Struck by falling object

While working in the shop, EE dropped a wrench on left hand causing a closed non-displaced finger fracture. The adjustable wrench landed on EE's hand which was caught between the wrench and metal container.

While walking from EE's pickup to the shop, the miner felt something snap in EE's right calf. EE's leg gave out and EE fell to the ground. The miner suffered a right calf strain and placed on walking restrictions.

Struck by falling object

While moving screen mesh from storage, the screen mesh slipped and smashed the miners right middle finger causing a contusion. The miner was wearing gloves, hardhat, and safety glasses during the incident.

Fall down stairs

Miner was walking down a ramp and tripped on something causing them to fall down stairs of a cone screen. Miner said they were not sure what they tripped on and there was no debris on the walkway. Miner fractured right ankle after the fall. Miner fell approximately 4-6 steps from the landing. The miner was not carrying anything during the incident.

Struck against a moving object

Individual was clearing dirt around the plant with a skid loader when loader hit a large rock. This pushed employee forward into the safety bar causing a bruise to chest on the left side. Individual was wearing a seatbelt.

EMPLOYEE WAS WALKING ON CONCRETE SIDEWALK WHEN HE STEPPED ON A ROCK AND IT SLIPPED CAUSING HIS ANKLE TO ROLL, RESULTING IN A STRAIN / SPRAIN OF THE LEFT ANKLE

WHILE WALKING THE PLANT DURING STARTUP, A RANDOM ROCK BOUNCED OFF OF THE SCREEN DECK HITTING HIM ON THE HARDHAT AND LANDING ON HIS SHOULDER RESULTING IN A CHIPPED CLAVICLE.

miner was standing on excavator track and while closing the door, fell off and struck his head on the ground causing lacerations to his head and hand. head laceration required eight sutures.

EE WAS CLEARING A JAM IN THE FEEDER. HE WAS STRIKING THE JAMMED ROCKS WITH A SLEDGE HAMMER. WHEN THE JAM BROKE LOOSE THE HAMMER WAS STRUCK BY THE FALLING ROCKS. EE WAS TRYING TO NOT LOSE THE H AMMER AND THE HAMMER TWISTED AND JARED IN HIS HAND CAUSING A STRAIN TO HIS FOREARM. RESTRICTED DUTY.

EE WAS CHANGING BELTS ON THE JAW CRUSHER, HE DECIDED TO CLIMB ONTO THE FRAME TO GET A DIFFERENT ANGLE TO WORK AND FELL OFF THE FRAME ABOUT 2 OR 3 FEET TO THE GROUND. HE HIT HIS CHEST/SHOULDER ON THE GROUND, RECEIVING A CONTUSION. EE RECEIVED RESTRCTED DUTY STATUS WHILE HEALING.

LOADER OPERATOR WAS SCALING THE BENCH. A ROCK FLIPPED OUT OF THE BUCKET, AND CAME THROUGH THE WINDSHIELD. THE ROCK PRESSED ON THE R STEERING JOYSTICK AND CAUSED THE LOADER TO REVERSE DIRECTION AND TURN AT THE SAME TIME THE ROCK WAS PRESSING ON THE OPERATOR'S LEG WHICH CAUSED HIM TO ACCELERATE. OPERATOR STRUCK THE OPPOSITE BENCH. APPEARED TO LOSE CONSCIOUSNESS MISSED 1 DAY OF WORK.

MINER WAS LOADING ROLLERS INTO A PICK UP TRUCK AND CAUGHT HIS HAND BETWEEN A ROLLER AND THE BED OF THE PICK UP. STITCHES REQUIRED.

EE WAS SHOVELING AROUND A SHUT DOWN AND LOCKED OUT CONVEYER. HIS SHOVEL HIT SOMETHING AND JAMMED HIS SHOULDER. HE SAW AN ELECTRICAL CABLE AND ASSUMED THAT HE HAD BEEN SHOCKED. (RESTRICTED DUTY AND RX MEDS) AN ELECTRICIAN WAS BROUGHT IN TO CHECK IF THERE COULD BE ANY LIVE WIRES OR VOLTAGE TO THE FRAME AND THERE WAS NONE.

EE WAS CHANGING THE TAIL PULLEY ON THE SCALPER BELT. AS HE ALIGNED THE PARTS THE TAIL PULLEY SLIPPED AND CRUSHED HIS HAND. THERE WERE NO BROKEN BONES BUT THE LACERATION REQUIRED 8 STITCHES.
