Employee was removing steel liners from hopper. A chipping hammer was used as wedge while cutting and got very hot. After completing removal of liner, he sat the hammer down in front of him and began grinding the old welds. He inadvertently knelt on hot hammer while grinding and jumped releasing hold on grinder. Grinding wheel came in contact with left thumb causing small cut.

Employee was welding, when welding the employee received flash burn to both eyes from another welder in the same area. The employee reported the flash burn 16 hours later, we are not exactly sure on the time of exposure.
