Employee works indoors in the Chilton Manufacturing Center as a Splitter Operator. The employee was operating a splitter and a piece of rock fell off the table and struck foot. The employee was wearing steel toed boots with metatarsal protections.

Employee was working indoors splitting stone when a saw near his splitter, had a stone open at the seam. This caused the blades of the stone to force the stone out towards the operator of the splitter next to it.

Employee was working indoors on a Hydro Splitter and was cutting the ends off the stone to square them off. While lowering the head of the machine, the stone flipped up and flipped up pinching his left hand between the stone and top of the machine. This resulted in a left hand middle finger fracture.

Employee was making a cut on a stone, the stone kicked up causing his finger to be pinched between the stone and the metal frame of the splitter.

Employee was splitting a stone and lowered the head of the splitter instead of the cut cycle. The left index finger got caught between the dust collector and the stone.

The employee was working indoors and running a splitter machine. He got his hand caught between the stone and the table.

Employee was performing a finishing process on stone; a stone chip flew under employee's right eye causing an abrasion. Prescription given.

Employee was pushing stone forward through hydraulic splitter; stone got stuck causing the employee's left wrist to bend downward. Employee did not seek treatment until 4/12/11. Employee was diagnosed with ganglion cyst. No restrictions were given. On 4/20/11, diagnosis was changed and restrictions were given.

Employee was flipping stone over and pinched finger between stone and rollers.

Employee was using nail gun to nail boxes together. Nail went through wood and punctured employee's right ring finger. Splint required.

Employee was operating splitter; another employee who was loading the line moved the row of stones forward with the forks of his forklift causing the first stone on the line to move forward, pinning the employee between the machine and stone. Restricted duty assigned.

Employee had hand on stone while cutting stone on end trimmer. The blade tipped causing employee to pinch finger between stone and frame of trimmer. Restricted duty assigned.

Employee was using Hilti Hammer drill; the drill got caught and spun around her hand. She struck her hand against a racking system. Diagnosis: Fractured left finger

Employee pinched his finger when the linkage on the control for the bottom head broke causing it to rise by itself. At the same time, the coil in the joystick that controls the top head of the machine fell out. Without this coil, the top head could not be raised. Diagnosis: Finger contusion - restrictions assigned.

Trimming stone on end trimmer; instead of moving hand when making a cut, the employee held onto the stone causing the stone to rise up and pinch his finger. Diagnosis: Fracture; Crush right ring finger Stitches, splint and restricted duty assigned.

Employee was using a cutting wheel to cut a piece of metal off a forklift rack. The wheel became jammed and fell out of employee's hands. Diagnosis: Right 2nd finger laceration. Restrictions assigned.

Employee was opening a stone with the Splitter; a piece of the stone hit him in the elbow and jammed his right shoulder; Diagnosis: Right shoulder strain; right elbow laceration. Restricted duty assigned.

Employee was using a manual grease gun to grease a splitter when he felt something pop in his thumb area. Area burned and felt tingly. Diagnosis: Flexor tendon strain; restricted duty assigned.

Stone was being pulled by discharge belt. Employee reached through opening to pull stone back as the splitter was continuing to cycle. The head came down and smashed his left hand between the stone and upper teeth of splitter. Diagnosis: Crushed left hand; Status: Currently incapacitated.

Employee's saw blade got stuck in a stone. When the blade came free, it jerked the saw upward rapidly causing his arm to jerk. **Neuroproxia right upper arm **Off work

Employee was cleaning the splitter under the lift table. Another employee dumped stock on the conveyor and the splitter operator turned around to watch the stock come up the belt and at the same time lowered the lift table. His hand was still under the lift table and got pinched between the lift table and the frame.*Restricted Duty*

Employee placed a stone under the head of the splitter, lowered the head and started the cut cycle. The wedges came down but he did not get his hand out of the way fast enough. The wedges hit the last knuckle of his left index finger. *off work * amputation.

Employee cut a stone and noticed debris laying by the lower wedges. He reached with his left hand to clean it out and at that time the scrap guard bounced up and pinched his ring finger. *restricted duty *hemotoma/bruising left ring finger

Employee was placing a stone in the splitter to make a cut and jammed his finger between the stone and the splitter teeth.

Employee was reaching under the head of the splitter with both hands to reposition a stone. His right hand was on top of the stone when the splitter head came down, pinching his right hand between the wedges of the splitter head pinching his right hand between the wedges of the splitter head and the stone. Contusions to r. hand fingers 2-4 *light duty.

Employee was putting pressure down on a stone. The stone slipped on the table causing his hand to move upward striking the blade of the saw.

Employee's fnger was pinched between a stone and the rollers when he accidentally pushed the guide button and the guides came down.*Fracture of right middle finger. *Off work.

Employee was pachaging on 9/27/06 and complained of shoulder pain. He said it had been hurting since 9/25/06. *Muscular injury left scapular. *Restricted duty.

EE was sawing a stone and felt something fly under his glasses and into his eye.

EE was cutting stone, arm started hurting. Left arm swollen, left wrist pain--restricted duty.

EMPLOYEE'S SHOULDER STARTED HURTING FROM PULLING DOWN ON THE SAW TO SAW STONES.

EMPLOYEE WAS CUTTING STONE ON A SAW. THE BLADE DID NOT RETRACT HIGH ENOUGH & HE BUMPED HIS ARM AGAINST THE BLADE.

Employee was cutting stone on the splitter. He turned the feed rollers backwards to clear the stone, but it did not roll. He grabbed the back of the stone with his left hand and when it began to roll his finger got pinched between the stone & the roller. Tuft fracture on left ring finger.

EMPLOYEE WAS OPERATING HYDRA SPLITTER. A PIECE OF THE STONE HE WAS CUTTING FLEW & HIT HIM IN THE LEFT FOREARM CAUSING A LACERATION REQUIRING STITCHES.

EE WAS CUTTING STONE AND HE GOT DUST PARTICLES IN HIS EYES WHICH CAUSED IRRITATION - PRESCRIBED MEDICATION (VIRAL CONJUNCTIVITIS). EE NOTIFIED SUPERVISOR THE DAY IT HAPPENED. SUPERVISOR MAILED ACCIDENT REPORT TO HR, BUT HR NEVER RECEIVED IT. HR FIRST NOTIFIED OF ACCIDENT WHEN CLINIC CALLED ABOUT BILLING ON 1/20/06.

Employee was grinding & a piece of metal got in his eye. Employee did not seek medical treatment until 8/10/05.

EMPLOYEE WAS OPERATING HYDRAULIC SPLITTER WHEN A STONE FLEW BACK AND HIT HIM ON THE FINGER. SPRAINED FINGER.

Employee was using chop saw - felt pain in his right arm. Overuse injury - right arm.

A STONE ON THE CONVEYOR BELT BROKE. A PIECE FELL AND HIT HIM ON THE LEG, RIGHT LEG LACERATION

EMPLOYEE WAS WORKING BY HYDROSPLITTER. A STONE BROKE & FELL OFF CONVEYOR BELT ONTO HIS LT. FOOT. LEFT FOOT WAS FRACTURED.

EMPLOYEE WAS CUTTING A STONE ON A TABLE SAW. THE STONE TWISTED AND EMPLOYEE HAND CONTINUED INTO SAW BLADE. NEAR SEVERANCE OF EMPLOYEES 5TH DIGIT ON LEFT HAND.

Employee was cutting a stone on the table saw. The stone slipped from his grasp & his hand continued into the blade. Laceration to left hand.

EE WAS CUTTING A STONE ON END TRIMMER. THE STONE TWISTED, CATCHING EE'S FINGER BETWEEN THE STONE AND THE SIDE OF THE END TRIMMER. AMPUTATED RIGHT INDEX FINGER (TIP OF FINGER ONLY - NO BONE INVOLVEMENT).

EMPLOYEE WAS CUTTING STONE ON THE TABLE SAW. STONE MOVED & EMPLOYEE'S HAND CAME IN CONTACT WITH THE SAW. HE RECEIVED A 3CM LACERATION TO HIS RT. RING FINGER THAT REQUIRED STITCHES.

EMPLOYEE WAS WORKING AT SLOTTING STATION. HE WENT BEHIND THE MACHINE TO GIVE INSTRUCTIONS TO FELLOW EE'S. SLOTTING MACHINE CYCLED TO THE BACK & PINCHED EE BETWEEN THE MACHINE & A CONVEYOR. EE WAS ADMITTED TO HOSPITAL WITH ABDOMINAL PAIN.

Employee was using the end trimmer machine to cut a stone. A piece broke off and hit him in the face. He then fell to the ground laceration to face requiring sutures sprained right wrist.

Employee was sawing a stone on the table saw. The ee's hand grazed the saw blade as ee removed the stone. He received lacerations to his left hand that required stitches.

EE USES A SAW TO CUT STONE & BEGAN FEELING NUMBNESS & TINGLING IN HER RIGHT ELBOW & FINGERS FROM REACHING UP & PULLING THE SAW DOWN. DIAGNOSIS: MEDICAL EPICONDYLITIS W/ULNAR NEURITIS

EE WAS CUTTING STONE WHEN A STONE CHIP FLEW INTO HIS RIGHT EYE.

EMPLOYEE WAS TRIMMING A STONE WITH END TRIMMER. A STONE CHIP FLEW OUT AND CUT THE EMPLOYEES UPPER & LOWER LIPS REQUIRING STITCHES.
