While loading material onto the conveyor of TXS 3 in the NTV Flats dept, the stone tipped over, pinching EE's finger between the stone and the conveyor.

Employee in the NTV area was moving a stone off a conveyor belt when it fell on EE's foot.

While trying to move a stone that was jammed on the feed conveyor of splitter #9 inside the CMF, the employee's hand slipped on some ice that was on the stone, and the employee's left middle finger was pinched between that stone and another stone that was on the conveyor. The finger was bruised, numb, and swollen after making contact between the two stones.

Employee was working inside of the CMF building on splitter machine #7. While removing a stone from the splitter, the scrap guard between the discharge conveyor and the bottom row of teeth popped up, then came down on finger, pinching it between the scrap guard and bottom row of teeth. The left index finger was cut and fractured.

Employee was working in the CMF on the splitter nearest the office. Was helping the operator move a rock standing on the machine leg. The employee's foot slipped and went into a slit. The employees right foot then got stuck between two pieces of iron when a waste tub was slid into place by the material handler.

Employee reporting pain/injury in upper left shoulder from repetitive use over time operating forklift steering wheel in the CMF.

I lifted a stone from the conveyor belt that weighed about 100 pounds to place it on the pallet, and I immediately felt a crack in my back. I told the material handler about the accident & asked the material handler to let the supervisor know. I continued working slowly and after I took my lunch break and went back to the work area, the pain in my back was worse.

I took a stone from the splitter table that was 10"" to change its position, and when I held it in my hands, the thumb of my right-hand bent.

The injured employee was cutting rocks and they smashed their left ring finger with a rock. IMPACT/BLUNT FORCE INJURIES.

I was picking up and moving a stone piece when it slipped out of hand and slid down my arm. It cut my arm.

EE had split a piece of stone in half, slid the top half back off of the bottom half, and pinched their finger in the process.

When moving a stone from one pallet to the finished pallet for packaging with a suction cup tool employee reached down to move some packing material and at the same time the stone fell from the suction cup onto EE's hand

Employee was taking a stone from the pallet to cut it on the machine and another stone from the pallet fell on their right thumb resulting in a nondisplaced fracture of distal phalanx of the right thumb.

EE had to crouch down to pick up multiple rocks that fell over, causing pain to EE's left lower back.

EE was working on machine #2 and a rock piece stuck under the lower belt guard and the belt that carries the split stone away. There is a metal plate that acts as a belt guard which protects the belt from having rocks get under it. It appears as the belt was running/advancing the rock piece was dislodged and the guard came down against the belt which pinched EE's finger.

Employee was stacking rock near a drain, employee tripped on slope and twisted foot.

Another employee threw a snowball in the production facility and struck employee in the ear.

Employee was work on splitter numbers 8 and 7 when EE picked a rock off the line and EE was opening a seam on the rock and a piece fell off and struck EE's right foot.

Employee works inside our Chilton Manufacturing Building in the Splitter Department. Employee operates the splitter and moves stone before it is split open.

EE is a Splitter Operator in our Chilton Manufacturing Facility at our Chilton Quarry. EE was leaving the shift early at 5:00pm as EE was feeling ill. As EE was walking out to the car in the parking lot, EE slipped landing on right side. A salt/sand mixture and salt is used regularly in winter to limit slippery spots in our parking lot and employee walkways.

Employee is a Splitter Operator at our indoor Chilton Manufacturing Facility. Employee was handling two stones when EE pinched EE's left pinky/5th finger between the stones. The pinch caused an laceration to EE's left pinky/5th finger.

In the process of setting it on the table next to EE, EE caught EE's finger between the rock and the corner of table resulting in a laceration and break to the right middle finger.

The nail gun then jumped and landed on top of EE's left hand. When the gun hit EE's hand, a nail fired into the top of EE's left hand as EE's finger was still on the trigger.

Employee had pulled into the quarry and parked semi in yard to hook up flatbed trailer. Fell of top step of truck & landed on gravel lot. Broke left wrist, right thumb & needed stitches in forehead.

Employee was working indoors on a NTV Corner Machine cutting stone. Employee loaded stone and had his hands place incorrectly and ended up lacerating is right index finger. Employee ended up getting sutures.

The stone broke in half and the employee tried pulling part of the stone out with a pry bar. While trying to pull out the piece of stone, the employee lost balance, tripped over a scrap tub and hit his head on the splitter machine.

Employee was turning over a large stone and felt pain in his right shoulder. Diagnosis: Shoulder strain. Restricted duty assigned.

Trimming stone on end trimmer; instead of moving hand when making a cut, the employee held onto the stone causing the stone to rise up and pinch his finger. Diagnosis: Fracture; Crush right ring finger Stitches, splint and restricted duty assigned.

On 11/13/07 employee was stacking stones on pallet and felt a pain in his shoulder and pelvic area. He took aspirin and continued to work. He did not report it to his supervisor. On 11/14/07, employee was doing same task and felt pain again. Diagnosis: Left shoulder strain and right groin strain; Restrictions assigned.

Employee was cleaning the splitter under the lift table. Another employee dumped stock on the conveyor and the splitter operator turned around to watch the stock come up the belt and at the same time lowered the lift table. His hand was still under the lift table and got pinched between the lift table and the frame.*Restricted Duty*

Employee placed a stone under the head of the splitter, lowered the head and started the cut cycle. The wedges came down but he did not get his hand out of the way fast enough. The wedges hit the last knuckle of his left index finger. *off work * amputation.

EE began to have back pains from driving the lift truck on rough terrain. Off work. Right thoracic back strain.

Employee was lifting an icy stone and it fell and landed on his finger. Fracture right middle fingertip. Restricted duty.

Employee cut a stone and noticed debris laying by the lower wedges. He reached with his left hand to clean it out and at that time the scrap guard bounced up and pinched his ring finger. *restricted duty *hemotoma/bruising left ring finger

Employee was placing a stone in the splitter to make a cut and jammed his finger between the stone and the splitter teeth.

Employee was reaching under the head of the splitter with both hands to reposition a stone. His right hand was on top of the stone when the splitter head came down, pinching his right hand between the wedges of the splitter head pinching his right hand between the wedges of the splitter head and the stone. Contusions to r. hand fingers 2-4 *light duty.

Employee was putting pressure down on a stone. The stone slipped on the table causing his hand to move upward striking the blade of the saw.

Employee was walking from one building to another and slipped on the snow and landed on his buttocks. *Was not determined to be work related (per Doctor) until 12-29-06. Lumbar disc displacement* *off work*

Employee tripped over a cord as he was walking back to his work station. He landed ont he floor and hurt his shoulder. Left shoulder injury-restricted duty.
