Employee was watching rocks drop from conveyor and a rock hit his left index finger. Employee received a laceration from the accident and the doctor placed the employee on restricted duty. Employee was also prescribed a splint to wear on his left index finger.
Employee was changing the propane tank on a fork lift and the propane discharged from the tank. The tank was found to be defective.
Employee was removing stone jammed in conveyor belt. Stone on top on pile fell on employee's right forearm. Restricted duty assigned. **Employee did not seek treatment until 8/13/11**

Employee pinched his finger when the linkage on the control for the bottom head broke causing it to rise by itself. At the same time, the coil in the joystick that controls the top head of the machine fell out. Without this coil, the top head could not be raised. Diagnosis: Finger contusion - restrictions assigned.
Employee was carrying a large stone to pallet and hurt his back. Diagnosis: Back strain; Restricted duty assigned. **Employee did not report injury or seek medical treatment until 2/11/08.

At that time, the Dr. told the employee he would probably have to have surgery on his right knee in the future.
Employee was using a cutting wheel to cut a piece of metal off a forklift rack. The wheel became jammed and fell out of employee's hands. Diagnosis: Right 2nd finger laceration. Restrictions assigned.
Employee was wrapping a pallet of stone and felt pain in his back. Lumbar strain - Restricted duty and prescribed medications.
Employee was placing stone on discharge belt to be recut. His finger got caught under the stone. Fracture/Laceration requiring stitches
Employee was next to another employee who was chiseling a stone. A stone chip flew up and became embedded in the employee's shoulder. Shoulder puncture; prescribed antiobiotics and pain killer.

Employee was carrying a stone and felt some pain in his back. *Didn't report injury until 2-8-07. Back strain. Off work.
The lever on the bobcat that is used for locking/unlocking the pin so that the operator can change from bucket to forks was frozen. The employee kept trying to get the lever to move and once it did, the lever pinched his finger between the lever & the carriage. Fracture & complex laceration right middle finger. THE EE SET A LARGE STONE DOWN ON TOP OF HIS HAMMER. THE HAMMER FLIPPED UP STRIKING HIM IN THE MOUTH. RESULT: SPLIT LIP AND BROKEN TOOTH.

EE BENT OVER TO PICK UP A STONE, WHEN RETURNING TO STANDING POSITION. HE HIT HIS HEAD ON A STONE THAT WAS HANGING OVER ROLLERS.
ROCK CUT ARM. DIAGNOSIS: SUPERFICIAL LACERATION.
EE WAS WORKING 9ON THE SPLITTER, HE WENT TO MOVE A PEBBLE OFF THE TOP OF A ROCK BEING CUT, WHEN THE SPLITTER TEETH CAME DOWN AND SMASHED HIS FINGER. HE LOST THE FINGERNAIL & RECEIVED STITCHES.
EE WAS PUTTING A STONE ON A PALLET WHEN HE FELT A SHARP PAIN ON HIS LEFT SIDE.
EE WAS PICKING UP A ROCK AND SLIPPED OUT OF HIS HANDS AND THEN HE TRIED TO CATCH IT IN MID-AIR. RESULT: PULLED SOMETHING IN HIS SHOULDER. DID NOT REPORT OR SEE DOCTOR TILL 3/5/01.
EE HAD COMPLAINED OF SORENESS IN SHOULDER AND DECIDED ON 12-20-00 TI SEE A CHIROPRACTOR.
EE WAS CUTTING ON THE SPLITTER AND REACHED BACK TO PULL ANOTHER ROCK TO MACHINE AND SOMETHING GAVE OUT IN HIS LOWER BACK RIGHT SIDE.
SPRAIN/STRAIN TO SPINE, CAUSED BY PICKING UP A ROCK. CYRIACK DID NOT REPORT INJURY OR WENT TO SEE A DR TIL 10-26-00.
EMPLOYEE WAS SETTING A STONE ON FLOOR, WHEN HE GOT UP HE FELT PAIN IN HIS BACK. HE WOULD HAVE BACK PAIN WHILE STANDING.
WHILE USING A SLEDGE HAMMER AND LIFTING CONCRETE SLABS, HE FELT PAIN IN RIGHT SHOULDER, NOW HAS CONSTANT ACHE IN SHOULDER.
PAIN STARTED IN HIS BACK AFTER LIFTING A BIG STONE, BEOFRE REPORTING (WAS REPORTED TO BUECHEL STONE'S HR DEPT 10/12/00) LOWER BACK STRAIN.
THIS INJURY OCCURRED ON 8-23-00. DIDN'T GOT TO SEE A DOCTOR UNTIL 8-29-00. HE HURT HIS BACK LIFTING A LARGE ROCK, AND TRIPPED ON A SMALL STONE, CAUSING HIM TO TWIST BACK WHEN HE FELL.
EMPLOYEE WA LIFTING A STONE AND A PIECE BROKE OFF AND FELL AGAINST HIS ARM CUTTING IT.
WOKE UP WITH NUMBNESS OF THE 4TH & 5TH FINGERS ON RIGHT HAND. PROGRESSED TO THE POING SHE WAS UNABLE TO MOVE FINGERS. DOCTOR NOTIFIED COMPANY THIS WAS A WORK RELATED INJURY ON 8/8/00.
EE'S HAND WAS CAUGHT IN SPLITTER TORE MAIN TENANT IN THUMB-RESULTED IN SURGERY TO REPAIR & REMOVE BONE CHIPS . OFF WK 12 WKS RECOVERY
PUT A STONE IN A PALLET AND WAS MOVING IT. ANOTHER EE PUT A STONE NEXT TO HIM AND DIDNT REALIZE EE S FINGERS WERE THERE. EE'S FINGER WAS PINCHED.
