AFTER 2 WEEKS HE WAS COMPLAINING OF PAIN & DECIDED TO GO TO THE DOCTOR. DR DID X-RAYS & FOUND A FRACTURE IN THE TIBIA. 1ST VISIT TO DR ON 4-23-04. EE WORKED FROM 4-9-04 TO 4-23-04.

EE WAS DRILLING MATERIAL USING A PNEUMATIC DRILL WHEN HE ALLEGED THAT THE DRILL BIT SLIPPED OUT OF THE DRILL HOLE & HE TWISTED HIS RIGHT SHOULDER.

EMPLOYEE WALKED BY A DRILL PRESS AND BRUSHED THEDRILL PRESS CAUSING A STEEL PIN TO ROLL OFF AND LAND ON HER TOE.
