Employee was operating a D8 dozer and started experiencing neck and shoulder pain after the dozer landed roughly from climbing over a hardpan boulder that didn't break apart when employee was placing material being dumped by haul trucks.
Injured Left index finger with screw gun and became dizzy and fainted.
IW placed EE's hand between two sticks of rail. Eq. Operator moved the stick of rail that was being installed, without clearing red zone. Stick of rail being installed slid forward and caught the end of the index finger of the R/ hand causing laceration.
EE states EE was operating a bulldozer and there was some malfunction of the machine while EE was backing up that made EE lunge forward. EE was holding the gears with EE's left hand, which put oppositional force on EE's left arm/neck. EE states EE has pain in the left side of EE's neck, left shoulder and upper left arm. Identified restrictions by MRI on 11/5/2022
Employee was roof bolting in Area 7. As employee reached half the depth of the drill steel, drill steel guard was released as is standard procedure. The employee's left hand became caught on guard and in the process the left thumb was pinched between the guard and boom of the roof bolter.
The employee was cleaning a pedestal floor plate w/a grinder and wire wheel when foreign material got into EE's right eye. The employee was wearing Safety Glasses and a Face Shield at the time of the incident. EE immediately came down from the machine and flushed EE's eye to remove the material from EE's eye. I was notified and continued flushing the eye with eyewash.
Pressure washer wand hit index finger on right hand with pressure washer. Causing .07cm round hole in right index finger middle joint area. On May 27th employee went to hospital due to pain. EE had surgery on finger to release fluid in finger.
While using an overhead hoist to position the No.8 trummel for routine maintenance, the lifting hook pulled through the steel structure and struck employee on the right wrist. The injury became recordable on February 10, when the employee was treated with an injection of anti-inflammatory medication and placed on restrictions during an ongoing care visit with a physician
Contractor's EE was struck in the back of the head and neck by a clevis pin that came loose from the clevis and factory tow hook on a slider bucket truck. The pin penetrated through the D8 steel guard and window. The stuck equipment is owned and operated by another contractor of Mosaic.
Employee was operating a 657 scraper and hit a small rough spot on the haul road, resulting in back and shoulder pain. EE stopped the equipment, and when the scraper was inspected it was found that the threaded yoke and linkage rod threads stripped out at the cushion hitch valve. The employee is currently on restrictions.
Injured employees right thumb lodged between a 1/2 inch winch cable and a AWJ drill rod causing a compression injury.
While lining up pipe to bolt flange the pipe sprung back and hit miner in the knee. On 5/21/2020 miner had x-ray which was negative. After continual discomfort miner had a MRI on 6/29/2020. MRI results on 7/1 revealed a fracture to the end of the fibula.
Miner experienced pain in neck, shoulder & arm while operating bulldozer. On 7/27/2020 Neurologist deemed illness (pain/inflammation) was work related and prescribed physical therapy.
Employee was struck by cable reel machine when cable became tight due to not clearing the starboard winch during unspooling. On 11/19/19 employee was placed on restrictions after the MRI results revealed multiple tears in the right shoulder.
Employee pulled up to the work zone in forklift to help cherry picker remove material from Level 1. Employee was helping with the outrigger set, heard a clank and someone yell to run. Guard rail was knocked loose by the picker and fell, striking shoulder and forearm.
1) Operator distracted by cell phone. 2) Operator should not have been in the seat while greasing was ongoing. 3) Operator failing to notify anyone that operator was moving equipment.
Two emp were drilling a hole in the mine floor with a hydraulic drill. The drill bit caught causing 1 emp to stumble & lose grip on the handle of the drill. The drill spun causing emp hand to become caught in the hand guard with emp hand stuck on the trigger. This emp. fell into conveyor feeder. This emp. taken to dr. and placed on restrictions.
Two emp were drilling a hole in the mine floor with a hydraulic drill. The drill bit caught causing this emp to stumble & loose grip on the handle of the drill. The drill spun causing emp hand to become caught in the hand guard with emp hand stuck on the trigger. 2nd emp fell into conveyor feeder. This emp taken to dr and placed on restrictions.
Employee was operating a bulldozer watching other employees put together a 24" joint of pipe. EE exited the tractor and was walking toward the employees to assist in the task as they flagged the rubber tire operator to let the pipe down. When the rubber tire operator let the pipe down, the pipe shifted forward striking the employee in the lower leg causing EE to fall to the ground.
Tractor operators were working the tailings discharge line. The poly pipe at the discharge started to slide down the hill at the discharge. The tractor operators moved to hold the pipe in place with their blades. As the pressure on the line increased, it slid out from under the tractor blades and struck tractor
EE was using a hand-held air chipping hammer to remove build-up on the Granulator. As EE moved left hand down the towards the base of the chipping hammer, left middle finger was pulled into the tool & pinched. The employee was wearing leather gloves. EE taken to ER & treated for fracture to left middle finger with sutures. Pain Med was given & emp placed on restrictions.
After using a gas powered chop saw to cut cement, EE stated was not feeling well and wanted to go home. Once home, EE felt the dizziness, headache and nausea could have been caused by inhaling exhaust from the chop saw and sought medical treatment.
EE walked in between rock box and flex. EE signaled, crane operator understood to be "Boom Up" suction pipe, so final tie in could be made. After signaling crane, EE moved into the line of fire. When crane lifted suction, stored energy caused flex to swing back into the rock box. EE who signaled the crane operator was briefly caught between flex and rock box.
Employee was using a powered hammer drill with a hole saw attachment. Employee reached down with his left hand to steady the piece being drilled on. The hole saw bit attachment became bound up and jumped out of the drill hole and struck the employee's left pinky finger resulting in a laceration. Employee received 7 sutures.
Employee was welding and felt debris enter eye. Reported to supervisor and washed out eye. At 2:30 am taken to hosp. in Bartow, Fl. Employee returned to Tx. on 6/10 and went to doctor in Tx on 6/11/2015, 6/12/2015 and was released for work on 6/15/2015. The employee did not lose any time.
Emp raising the boom with the fast feed lever with right hand and holding 6 steels in the hole with his left hand. The end of the steel extended out of the hole 10" and as the boom was coming up to meet the end of the steel, the operator stated that the boom jumped & pushed his left hand up in the hole between the back and the turn table of the bolter.
While digging clays with the 14 D/L in the S1 clay dam the operator was dumping a bucket when a clay ball came back towards the operator cab. The clay ball shattered the window cutting the employees arms.
Emp. was loading an excavator and he went to get off and the equip. started to tip over and emp. jumped off and landed on his left heel. Emp. taken to the company Dr. He has a hairline fracture and was placed on restrictions. Emp. will return to work on his normal shift 7/24/13.
Equip. Oper. hit a rut, causing tractor to bounce, jarring operator. He reported discomfort in neck, seen by DR 6/4 sent for Xray, no fractures found. Oper. returned next scheduled shift w/no restrictions. Later seen for pain complaint, placed on work restriction w/RX. Initially reported 5/31,became reportable on 6/12 w/RX & restrictions given.
GRINDING DISC BEING USED BY ee SHATTERED CAUSING PIECES OF DISC TO FLY BACK HITTING ee. PIECE OF DISC BECAME LODGED IN ee RIGHT SIDE JUST BELOW RIBCAGE.
Two mechanics were installing a resistor bank on dragline 16. They brought it in to the machine house with the side winch and set it on the floor. They manually moved it across the floor to position it and pick it up with the main hoist. Employee caught his pinky finger on his left hand between the framework and a lube tank, cutting it at the first joint.
Employee was using a socket universal on a air impact to remove 1" bolts, as the mechanic was removing one of the bolts the swivel pin on the universal dislodged and the universal came apart in the mechanics hand, causing a laceration to the inside of his right ring finger.
Emp. welding when he felt a piece of hot slag fly into his welding glove on his left hand. Emp. removed glove and said he saw a small red spot on his finger where he had been burned. Emp. continued to work. Emp. reported injury on 10/5/11 because left hand ring finger was red and swollen with drainage. Emp. taken to company Dr. and given oral antibiotic and restrictions.
While using a pipe threading machine employee got his hand caught between the pipe and the machine, causing lacerations to right hand between thumb and forefinger.
Emp. noticed red, swollen, red irritation on the left side of his navel on his stomach. He stated he was welding on ventilation fans the day before 8/4/11 and during the process a chip of slag or a "pop" of hot metal entered his shirt and he felt a burning sensation close to his navel. He went to the dr. and received an antibiotic. Emp. did not report injury until 8/5/11.
Emp. cut back of his hand between the knuckle on the ring and middle fingers (left hand) while blowing the grinder table area off. Emp. cut his hand on a duct opening behind the grinder, the flashing around each duct opening has sharp edges. Emp went to the ER and received 2 stitches. No restrictions.
Employee was using a 1" drive impact wrench un-bolting a flanged coupling on a 24" joint of pipe. He was loosening the nut when it flew out of the socket striking the employee on the right side of his face above his lip.
Employee was welding/gouging under a dragline tub when a piece of metal particle entered his left eye.
Mechanic was removing bolts from a hammer mill door utilizing a 3/4" impact wrench fitted with a 3/4" to 1/2" adapter to accept the smaller socket. The adapter reportedly failed causing the socket to spin off the bolt head, striking the employee in the mouth.
The injured employee was holding the head of a bolt with an end wrench while another employee was getting an impact wrench in position to remove a nut. The air hose caught on work platform causing the air hose to break away. The male end of the coupling hose struck the employee in the mouth breaking 2 teeth and cutting his lip.
Emp. was working in #1 Shaft scaling. He was 708'deep. He was struck on the left shoulder with a piece of re-bar. The re-bar was 2 1/2" long by 1". The Constructors crew was cutting the re-bar and a piece that was being cut flew down the shaft and hit emp.
While boring out a hole in a basket hoist for the clevis, the drill bit hung up causing employee to twist right shoulder. Originally treated conservatively as a minor first aid, became a recordable on 02/19/10 after further diagnostic testing and consultation with the medical staff indicated a need for more treatment.
EE was welding and grinding in right side of ballest. Unknown how or when foreign body entered eye. EE was wearing PPE.
EE was relocating fan, stuck fingers through the guard and fan blades struck fingers.
Employee was at the 12 pit preparing to drill out broken stud on stuffing box when the drill mechanism that powers the drill was inadvertently contacted causing the drill bit to contact his left thumb resulting in an avulsion/laceration.
Emp was using a Hilti nail gun attached to a 3 ft extension to attach a red warning ribbon to a rib. He let the gun extension handle slide through his hand to rest it on the ground and it hit the top of his foot instead and the gun fired on contact with his boot causing a fracture to top of Lt foot. Went to ER, placed on restrictions.
Emp was cutting wood for a waterline job. While emp was aligning the wood against the rail with right hand he bumped the wood. Hand slipped, saw blade caught shirt sleeve and cut the flesh to his right wrist and tendon. Emp had surgery to repair tendon and close the wound.
Employee felt minor irritation in right eye while at work but didn't report to supervisor until he was driving home and the pain increased. He states he did use grinder earlier that day and thinks he may have got an object in right eye. He went to company doc and eye doc where imbedded metal was removed from his rt eye and antibiotics were prescribed. Released to full duty.
Emp was using a hand held drill to drill a hole in a metal fan control box. The drill hung in the metal box causing his right hand to twist and resulted in pain and swelling. Took to doctor, xray showed fracture of right hand. Placed on restrictions and referred to ortho.
Foreign body went under face shield and got into right eye.
