Employee was drilling a hole in a new disconnect box. The drill bit got caught and pulled the box into employee's hand. Caused a cut and tuft fracture to right index finger.
Employee was using 1 inch impact gun on bolt under truck. Bolt was not coming off causing the employee to adjust position. As employee was adjusting the impact gun it bounced back hitting employees face causing a small chip top of *R* tooth. 4/10/25 EE did elect to repair tooth, dental work completed
Using hytorc gun on loader when it came loose striking the L middle finger causing laceration and fracture to the finger.
EE said EE was working on E9 belt on the north end side of the belt where the cleanup chute is. EE said that EE closed the cleanup chute door that goes down to E1 belt. Right after, a good-sized rock bounced over from the E9 belt. EE picked up the rock and was going to put it down the chute, EE said EE "closed the door to the clean chute and something above EE hit EE's hand"
Employee was rebuilding a mantel while setting the bottom liner with cable when the crane was lowered and pinched employee's right hand (4th digit) between C hooks and punctured with the latch. X-ray was performed on 8/22/23 showing a nondisplaced distal phalanx fracture of 4th digit.
Employees were using an air impact wrench and as they bled off air pressure a rod that held a swivel in place came off the fitting striking an employee in the chest. The employee was transported to an emergency room to have the rod removed. Employee was given medication during a subsequent follow up.
Employee was at booster station working on a pump installing bolts and nuts when right pinky was smashed between the casing and nut by another employee using a 1.5" impact. X-ray was performed on 7/25/23 confirmed fracture of right pinky and referred to medical specialist.
Employee was backing into 579 shovel in HT552 to be loaded when the dipper bumped the truck jarring the driver. treated as first aid. On 11/20/2023 company was notified recent symptoms are related to the work injury and physical therapy has been ordered.

Employee was removing the pivot pin with fellow employee using an air hammer when the bit chipped off and struck right forearm causing small cut. Xray was performed on 10/4/2022 4mm foreign body was discovered in soft tissue of Right forearm and referred to medical specialist.
Employee Was Working on haul truck tire. While using the RAD gun to torque the wheel the employee caught a finger between the RAD gun and the wheel causing a laceration to the left index finger. Required 12 sutures.
While using a grinder, it caught the corner and the grinder kicked back toward EE resulting in a break in the skin of EE's upper leg
Employee was in the process of removing adapters off of the 368 bucket when a J- hook they were using stretched out and came off the adapter striking the employee in the left forearm.
while rotating SAG mill to perform a gear alignment, a mill ball bounced out striking IE in mouth requiring 3 sutures.

Employee was repositioning right leg for leverage on the grinder when right foot slipped. While slipping they reached for the handrail in front of them to maintain balance which caused the grinder to turn sideways and cut into right thumb.
Employee was attempting to remove bolts from the lid of a Mine owned water truck, using a battery operated impact. While attempting to loosen a bolt, the employee's left index finger was lacerated between the lid and the nut.
Used power tool on exhaust flange and rust and debris went over safety glasses and into Left eye.
While employee was performing work on the press, a cathode shifted causing a laceration to right hand.
While trying to remove bottom drill steel, employee was attempting to place shim between jaws of deck wrench and drill steel. Operator thought employee motioned to close jaws pinning employee's right thumb between shim and drill steel causing an avulsion of right thumb.
When the pump was started, the discharge line rotated on the clamp causing the pipe being used to strike EE on the right side of the forehead/brow, lacerating brow. EE was knocked back into the sidewall of the thickener. EE fell to the ground on left side. Co-worker approached EE & rolled EE to EE's back, keeping EE still, & called a mayday over the radio.
While drilling out holes for fender with mag drill, the employee stepped on the cord, unplugging the mag drill causing the drill to demagnetize falling on employee's hand.
The joystick in the shovel failed and the operator could not stop the loaded dipper, and the dipper hit the haul truck, jarring the employee. Employee did not seek medical attention until April 20.
Mechanic was tightening up a few loose cutting edge bolts on rubber tire dozer. Employee was using a impact gun with a 1 1/2inch socket to tighten the bolts, holding the impact over head when the impact slipped off the nut striking the employee in the lower lip resulting in a cut to the outside and inside of lower lip. Employee received 4 stiches.
Employee was using a rivet buster to knock in bolts. One of the bolts gave which caused the liner to fall. From this the rivet buster shot up and hit the employee by right eye.
Employee had been welding earlier in the day and while in the process of wiping the sweat from EE's head EE used EE's sleeve and got foreign body in EE's eye. After it was reported EE was taken to an eye specialist who removed the material from EE's eyes.
Employee was operating the wet grind overhead crane for pulling the grinding rods out of #6 rod mill. The bundle was laying down on the grating on one end the other end was leaning up against the mill discharge box. A fan was blowing when employee was lifting the bundle of rods and that's when EE felt something enter eye.
Employee was using a clay digger to scale down the rock box and grabbed hold of the bit and pinched the tip of index finger.
While cutting pipe with a chainsaw, some residual water came out causing the mechanic to recoil striking another nearby mechanic's hand causing laceration requiring 6 stitches.
Employee was sliding the Knuckle boom extinction back in and pinched right ring finger and received 4 sutures.
Employee was installing a 20" ball mill feed elbow. One threaded studs fell out because the weld broke. Employee went to the repair bay to clean up this 1" threaded stud and to grind off the weld left over on the bolt. Employee was holding the bolt in one hand and the grinder in the other. The grinder bucked and blade made contact with thumb
The driller had plugged up the pipe, then pulled back a joint to the table and broke it loose and raised the top head. The driller noticed the inner barrel was plugged and leaned forwards towards the table a little when trapped air pushed drill cuttings out of the hole and sprayed into face and eyes.
Employee was using an impact wrench when the bolt caught glove. This caused glove and middle finger to wrap around the socket causing a fracture to right middle finger.
Employee was sanding a piece of angle iron. The sanding disc grabbed the angle iron which moved back to the employee's hand striking the top left thumb. Injury did not result in a Lost Time or Restricted Work. Employee received 10 stitches.
EE was changing drill steel on Drill 4. EE was guiding the steel over the hole in deck when steel swung over and smashed left hand between the steel and the carousel. Damaging left middle finger.
Employee was blading the ground around the #518 shovel. While in motion, the employee removed his seat belt and stood up to stretch his legs. While standing up, the blade caught a hard knob causing the employee to hit his head on the air-conditioning unit inside the enclosed cab. Employee sought medical treatment on his own 2 days later.
Hydroblasting stairway inside of shovel under very cramped conditions, waterjet penetrated boot injuring toe. Steel toe boots were in use.
Employee was working on a plugged up Syntron feeder (4) ee was using an air lance to bring down the feeder, while doing this the feed came down struck the air lance pinching left index finger causing a laceration on the index finger

Employee was holding a piece of duct & the drill bit got caught on the tin and went through the duct into his left thumb.
On the above date this employee was using a 1" impact gun and felt a sharp pain in upper left arm/shoulder. He has been treated for first aid since then and was out for personal illness for almost a year. Only now receiving any treatment for work injury. He had exploratory surgery and first day of lost time is 03/24/2016. The validity of this injury is in question.
Employee was operating a backhoe when he felt some discomfort in his groin area, around 11:30. The employee had lunch and went back to work. At 2:00 the employee requested medical care. Cause of injury is questionable.
Employee was working in basket of boom truck with LN-25 welder. The welder was knocked over and welding lead hit employee in arm causing injury.
Employees were using an air nail gun to frame the walls in the new ass in the truck shop. Employee attempted to nail his side when the nail gun went off and struck his lower forearm. Employee was checked by the nurse transported to clinic treated and released back to work.
Employee was guiding/signaling the crane operator that was hoisting a beam when the beam became lodged under another beam, when the tension was released the beam swung and hit the EE's hand breaking the first meta carpel bone in his right hand.
The employee was steam cleaning the drill. He was up on the mast of the drill. As he went to squeeze the handle to activate the cleaner, the nozzle jumped and he lost control of the wand. The high pressure water cut the tip of his boot and sprayed hot water onto his left foot causing 2nd degree burns to his foot.
Rigging strap shifted while moving load striking the employee in the right arm. Employee returned to work (Restricted Duty) on 04/12/2013.
Employee's left hand spun around the extension injuring the tip of the middle finger while working on HT426 installing the left rear tire. Employee was using a one inch impact. Employee became reportable on 4/26/2013 when he was diagnosed with a fracture of the middle left finger.
