Employee was tramming bolter up when the canopy caught cable that was hung from the roof. He reached out from under the canopy pushing the cable up with his left hand. The canopy caught employee on the left hand mashing it against the top resulting in a fracture to the end of his left middle finger and a laceration requiring stitches.

Showing all 33Investigation indicates employee was entering Roof Bolter tram deck when he inadvertently lowered canopy onto body, resulting in multiple fractures.
While trying to straighten a bent bolt that was being installed, the bolt spun in the operator's hand causing a strain to his abdomen. Employee started losing time on 03/24/2010.
EE was installing the second bolt from rib on last row in #3 entry. Had bolt in hole, trying to align bolt head with drill chuck. Used rotation to align, plate spun and cut EE's forearm.
Employee was helping anchor tailpiece and while holding one of the hydraulic jacks got his right hand caught between the jackbase and roof pad.
Employee stated that he was installing a roof bolt, caught his finger between the mine roof and bolt plate causing a laceration to his finger.
While installing roof bolt, rust off of roof bolt plate managed to get into the employee's left eye. He was prescibed a topical anitbiotic for preventative measures.
Employee was in process of connecting finisher and pusher steels when he inadvertently hit the rotation causing the pusher to fly out of the chuck. It hit him below his left eye resulting in a cut which got infected due to the type of sutures used.
Employee was grinding and welding on top of discharge gate, a few days later he realized he had a small piece of metal in his eye. Eye presciption medicine was administered.
Moving miner, a loop of cable slack pulled across, striking him and knocking him to the mine floor.
EE was moving pinner when pinner cable got caught on a roof bolt lying on top of pinner, cable jerked bolt off pinner, striking EE on top of rt. ear. Went to dr. 10/04/07.
EE was drilling bolt hole-head on pinner stopped up dropped head reached over to check suction on head. Drill steel fell out of hole, striking him on finger. Went to dr on 6/21/07-ret'd 6/22/07.
Employee was moving a piece of rock off the side of the continuous miner. Another piece of rock fell off striking him on side of Rt. foot. Went to Dr. on 3-13-07 (started his 4 off days on 3-14-07) Started losing time on 3-18-07.
Employee was swinging drill boom over to put up rib bolt, catching finger between canopy jack housing and boom. Went to doctor on 12-09-06. Employee on 4 on 4 0ff schedule. Returned to work on 12-13-06 10:00 PM.
Employee alleged he hurt his back while operating a d-10 dozer on the raw coal stockpile on 10-12-06. Injured did not reporat this accident until 10-20-06.
EE was spinning a roof bolt that he was installing, bolt bent, EE reached to get bolt and the bend in bolt struck him on the fingers and hand. Started losing time on 6/2/06.
Laceration & Fracture above Rt thumb - 3 stitches and immobillzation. Struck by pin from impact wrench. No lost time - employee performing light duty since date of accident. Total duration of LD may be up to 5 weeks.
INJURED EMPLOYEE WAS WORKING ON A PIPE JOB. ANOTHER EMPLOYEE WAS GRINDING IN THE AREA NEXT TO HIM. HE STATED THAT HE FELT A STING IN HIS RIGHT EYE AT THE TIME BUT WAS UNAWARE OF ANY METAL HITTING HIS EYE. HE DID NOT REPORT THIS UNTIL 3/15/06. THE DR. HAD TO REMOVE THE METAL SLIVER FROM HIS EYE.
Employee was helping to move the roof bolter machine out of #4 entry-the roof bolter operator backed machine upon a rock bolter slid off rock-causing it to move forward and toward rib bolter t bar striking him and pinching him against t bar and rib starting losing time on 1-23-06.
EMPLOYEE WAS CHANGING OUT THE FEED JACK ON A ROOF BOLTER MACHINE, GETTING HIS ARM CAUGHT IN A PINCH POINT AREA OF THE DRILL POT HEAD BOOM. STARTED MISSING WORK ON 8/15/05
Employee was bolting top - put a piece of pusher steel on the steel already in bolt hole - pusher steel bent and he reached to get steel - steel slipped into chuck of pinner and steel struck employee on end of thumb.

EE STATED THAT HE HAD MOVED BACK TO THE 2ND ROW OF BOLTS & PINNER OPR. LET T-BAR DOWN TO MOVE PINNER UP- ROCK FELL OUT BETWEEN BOLT & RIB, STRIKING EE ON SHOULDER & FOOT.
EE WENT BACK TO SIDE OF ROOF BOLTER MACHINE TO PICK UP SOME ROOF BOLTS WHEN A PIECE OF ROCK FELLOUT BETWEEN BOLTS STRIKING HIM ON LOWER PART OF BACK. WENT TO DR. ON 6-4-03, RETURNED TO WORK NE XT SHIFT THEN STARTED MISSING WORK ON 7-14-03.
EE STATED HE WAS PUTTING (2) PIECES OF STEEL TOGETHER, CATCHING HIS GLOVE & LITTLE FINGER ON RT.HAND, BETWEEN THE COUPLING POINT OF THE STEEL.
INJURED WAS USING A GRINDER TO GRIND THE SLAG OFF OF AN EXCAVATOR BUCKET. WHILE GRINDING THE GRINDER KICKED BACKWARDS STRIKING HIM ON HIS WRIST.
