Employee was replacing conveyor chain in a truck dump feeder. The chain was being pulled out by a hoist truck. The flight keepers were unknowingly wore out and 1 flight fell out of the chain and struck the employee on the left hand and knocked 2 more flights out of the chain as it was being pulled out.
Attempting to clean torch tip, employee burnt left hand with a lit torch.
Employee was installing a roof bolt in #1 entry, when he lost his footing and fell into the joystick. This engaged the rotation and his right hand was caught by the bolt. The resulting injury was a small laceration and ulna fracture to the hand.
The employee was drilling roof bolt hole his glove became caught between the two pieces of drill steel and twisted his hand and arm
Employee was removing drill steel from the drill pot when he felt a pain in his left shoulder.
Operator was attempting to retrieve a roof drill bit out of the mud with his foot while lowering operator side canopy. His left arm was resting on the bolt tray fully loaded with bolts when the canopy pinched his arm between the canopy and the stack of bolts.
Employee was using oxyacetylene torch to cut a metal pipe when spark blew back in his right ear. Employee sought additional medical treatment on Sept. 22, 2012 and began missing days from work on 09/23/2012.
EE was cutting trees for powerline. He had been cutting trees all day. He cut one small tree not foreseeing pressure from another tree pushing on the tree he was cutting. The saw kicked back chain brake was working correctly but apparently the angle he was cutting at prevented his arm from hitting the brake.
Employee was operating Roof-bolter, while installing roof bolt, employee hit rotation and the bolt hit him in the left wrist.
Employee was Roof bolting, and drill steel hit left hand and fingers.
While roof bolting employee put hand on bolt & was trying to spin it to get it in the top. His hand got caught on the bolt & the plate spun around and cut hand and twisted wrist & arm. Employee was wearing gloves & arm guard at the time of accident.
Employee started missing days from work on 4/4/11 from a previous injury. Employee stated he was using a hydraulic wrench to remove 2.5 inch bolts which attaches the vibrating box to a feeder. Employee states that the hydraulic wrench slipped from a bolt and he strained the left shoulder while attempting to catch the wrench.
While bolting top, he put his (L) hand on pot of bolting machine and crushed it with drill pot and top.
Employee was changing seats in the shuttle car after his car had been loaded.The continuous miner was backed up striking the car and scooting it into the employees feet. His feet were caught between the shuttle car frame and rib.
While reviewing accidents, management concluded that this accident had not been reported. Employee was grinding on excavator bucket when cup wheel exploded and a piece hit him in the chin.
Employee was drilling a roof bolt hole when a piece of drill steel dropped off the drill tray to the inside of the drill boom. He instinctively reached for the steel and got his finger pinched between the long and short drill boom leveling arms. This caused a laceration to the end of his right index finger.
EE was tightening hose pad bolts on a D6R with impact wrench, foreign body went into ee's left eye.
Employee complained of consistent pain in his back while engaged in various work activities over a two day period.
The miner operator was tramming the miner to a new cut and as the miner crossed over a dip in the mine floor, it slid and the boom contacted the operator's right leg. The miner operator's extent of injuries was a puncture wound caused by the miner boom. Sutures to close the wound was applied.
Employee was adding pusher steel. After coupling the two pieces of steel together, he started the rotation. He had rested his left hand on the drill chuck and his glove caught on a burr. This wrenched his hand resulting in injury to his little finger.
Employee was using a 10 ton porta power to reinstall the drive chain on the drive sprocket. The porta power worked loose and struck the employee on the left thumb.
Employee was installing a 6' glue bolt at a surface crack and misjudged the elevation of the bolter boom for a 5' bolt. The boom needed to be lowered 1' and while doing this the operator lowered the boom on the right foot. The employee continued working until 01/09/09 when he sought medical attention. Work continues in a restricted duty capacity.
Injured was pulling steel to align in head, hand slipped, got caught between pusher and finger while head was still going up.
Employee bent drill steel while drilling hole and it became stuck. He was going to drop the drill head when he accidentally engaged the rotation. The bent steel struck him on the left forearm.
EE was tightening up tire, when wheel wedge broke causing EE to fracture his arm at an old break site. EE did not know arm was broke until he went to ER at 1730 7/14/08.
Employee was putting a bolt plate against the roof during installation of the bolt and got his right index finger caught between the plate and the mine roof while he applied up pressure on the bolt. Employee was wearing meta-carpal gloves.
Employee was pushing a roof bolt into a hole when he got his finger between the drill pot and the bolt.
Employee was installing a roof bolt. He bent the bolt and inserted it into the drill hole. When he pushed on the bolt to straighten it up, he felt a sharp pain in his left side and shoulder.
Employee was observing his coworker move the roof bolting machine into a right hand turn when the back of the machine fell off a small ledge in the mine floor and slid onto his left foot.
Employee swung the head of the roof bolting machine out and pinched his knee against a rock.
The operator of the roof bolting machine moved the back of the machine down catching injured's foot between the frame and the mine floor.
The injured employee was positioned alongside the roof bolting machine operator's deck and had finished conversing with the operator who was in the deck. The operator started the machine and the injured employee was caught between the deck of the machine and the rib.
Employee was drilling a roof bolt hole. The hole was drilling crooked so he stopped drilling and was lowering the drill pot. The drill steel came out of the drill chuck under tension and sprang against the employee's right hand injuring his little finger.
Employee was tramming a roof bolting machine towards the face and turning into a right crosscut. He had his right hand outside of the deck area and when the machine turned his hand was caught between frame and the coal rib, mashing his right hand.
Employee was holding onto drill steel with his right hand as he attempted to start a hole in the roof. He "bumped" the rotation and his glove was caught on the rotating steel. This caused his hand to be pulled around the drill steel. He became a restricted duty case on 6/9/06.
Employee was using a cutting torch to remove a primary scraper from an old 42" belt drive. He cut one side loose, and while cutting the other side, the scraper swung back and struck his left knee.
Employee was setting a rib bolt when the steel stuck, his helper pulled on the steel and the steel came out and struck the small finger of the right hand causing a laceration. The laceration required sutures, employee returned to normal duties on his next shift.
Employee was using a cutting torch to cut bit lugs off of the miner. A piece of hot slag popped off and fell down between his cutting glasses and his eye causing a scratch on his left eye. The employee sought medical attention and was given a perscription. He did not miss work as the result of this injury. This is a medical case only due to the perscription.
Employee was installing a roof bolt which contained the metal tag and wire containing ASTM information. As he was rotating the bolt to seat it into the chuck, the wire caught the little finger right hand, severing the finger. The remainder of the finger was later amputated at a local hospital.
Employee was struck by a rock measuring 42" wide 41"long and up to 3" thick while operating a continuous miner. The rock fell from between permanent supports and the right rib. Injured employee complained of pain in back and ribs and was later Diagnosed as having NON life threatening injures.
EE WAS OPERATING A CONTINUOUS MINER WHEN A ROCK MEASURING 3' WIDE 3' LONG AND 8" THICK FELL FROM THE TOP STRIKING THE TOP OF THE CONTINUOUS MINER. THE ROCK TRAVELED ALONG THE CONVEYOR BOOM AND ROLLED OFF THE END OF THE BOOM STRIKING THE MINER OPERATOR ON THE LEFT KNEE.
EE REPORTED THAT A ROCK FELL FROM THE TOP OF THE ROOF BOLTER BOOM CANOPY, SLID DOWN THE BOOM AND STRUCK A BOX OF ROOF BOLT GLUE. EE STATED THAT HIS RIGHT HAND WAS BETWEEN THE GLUE BOX AND THE BOOM. EE RECEIVED MEDICAL TREATMENT FOR HIS INJURIES.
ON 3/5/04 OPERATOR WAS PROVIDED ADDL INFO ON THE ACCIDENT FROM 1ST AID TO MEDICAL. ON 6/19/03 EE WAS BLOWING OUT A TOOL BOX IN THE PLANT SHOP & A PIECE OF RUST WAS EMBEDDED IN EYE. INITIALLY THE REPORTED MED TREATMENT WAS WASHING OUT THE EYE. HOWEVER IT WAS DISCOVERED THAT A MED TREATMENT WAS GIVEN USING SPECIAL INSTRUMENTS TO REMOVE THE RUST IN EYE
EE WAS STRUCK BY A ROOF BOLT PLATE ON THE LEFT ELBOW AS HE WAS ATTEMPTING TO SEAT THE BOLT IN THE BOOM. EE SUFFERED A LACERATION WHICH REQUIRED (3) SUTURES TO CLOSE THE WOUND.
