Employee was changing a piece of steel on the DTH. To keep balance, employee laid left hand on the track. Employee's glove got caught in the chain and pulled hand into the gear. The pinky finger and ring finger were completely severed.

Employee was using an excavator to pull over at C Sector. Was looking up, watching the wall and the bucket slipped which jarred employee and the machine. Employee woke up the following morning, 9/16/16 with neck pain.

EE LEANED OVER TO LOOK OUT OF THE WINDOW OF THE HEAVY EQUIPMENT HE WAS OPERATING. HE FELT A 'PULL' IN HIS MIDDLE BACK. EE DID NOT REPORT THE INCIDENT TO MANAGEMENT UNTIL 2/24/09.

Attempting to straighten a 12" anchor pin with drill press. Pin popped out of hydraulic press and hit his right hand - 5 stitches.

Transferring diesel fuel from transportable tank to generator tank, fueling hose was inhibiting operation of crank-style pump. Employee moved hose to allow cranking, hose slipped off nozzel, spraying fuel at other employee in face, eyes, front, causing ingestion of fuel. Immediate vomiting. Employee was sick 2 hours, finished shift to 10:00 p.m.
