Fall to the walkway or working surface

Employee was sitting in a chair working on a wire saw control box and the chair shifted and EE fell out of the chair to the ground from a seated position. Review of the incident inconclusive of chair malfunction or employee weight shift could have caused the fall. Chair was disposed of regardless.

Currently showing a recordable STS for the right ear. Retest was recommended following initial testing. Retest was performed on 1/24/22. Employee stated employee has fluid on the ear. Employee also has a history of excessive wax build up in ears. Scheduling medical evaluation per doctor recommendation.

Employee was stung on the elbow by a wasp. They had a severe allergic reaction. Used an epi-pen onsite. Was rushed to the ER. Received fluids and medication via IV.

Struck by falling object

EE was attempting to use a bar to stop a slab from falling on EE's foot. The slab knocked the bar out of EE's hand and the bar came down across right foot, on top of the steel toe area of boot. EE has bruising on right big toe and second toe.

EE was on the edge of the B1//B2 highwall near the substation trailing a power cable along the highwall edge. EE stated that back popped when EE bent over.

Employee was changing a piece of steel on the DTH. To keep balance, employee laid left hand on the track. Employee's glove got caught in the chain and pulled hand into the gear. The pinky finger and ring finger were completely severed.

Struck against a moving object

Employee was using an excavator to pull over at C Sector. Was looking up, watching the wall and the bucket slipped which jarred employee and the machine. Employee woke up the following morning, 9/16/16 with neck pain.

Employee was working on a Volvo L330D Loader from a temporary platform constructed for the purpose of gaining access to the area in need of repair. The employee attempted to re-position the equipment by moving it, while standing on the platform and steering. Platform collapsed and employee fell.

EE WAS PASSENGER IN A PICK UP TRUCK TRAVELING DOWN THE QUARRY #33 RAMP. BRAKE LINE BROKE AND DRIVER COULD NOT STOP THE TRUCK. DRIVER APPLIED EMERGENCY BRAKE AND ATTEMPTED TO PUT TRUCK IN PARK, TRUCK CONTINUED TO MOVE, PASSENGER OPENED DOOR AND JUMPED FROM TRUCK. DRIVER CONTINUED DOWN THE RAMP AND DROVE TRUCK INTO PILE OF EARTH, NO DAMAGE TO TRUCK OR INJURY TO DRIVER.

EE WAS WORKING IN WHITE GEORGIA QUARRY OPERATING A QUARRY BAR. HE WAS HOLDING THE GUIDE BAR WITH HIS LEFT HAND, STARTED THE MOTOR SLOWLY STEEL TWISTED AND CAUGHT HIS LEFT THUMB BETWEEN THE GUIDE BAR ADN THE ROTATING STEEL. CUT TO THUMB REQUIRING 5 STITCHES.

EE WAS ATTEMPTING TO REMOVE A BRASS RIFLE NUT FROM A QUARRY BAR PISTON IN THE SHOP. THE PISTONWAS IN A VICE, EE WAS PULLING WITH A TOOL AND A CHEATER PIPE, NUT CAME LOOSE AND THE EE LOST HIS BALANCE, FELL BACKWARDS ONTO HIS LT. LEG & BROKEBONE IN LOWER LEG.

EMPLOYEE BENT OVER TO REMOVE LOSOE ROCK BENEATH LEAD DRILL, WITHOUT TURNING OFF DRILL ROTATION. HOOD ON RAIN COAT FLIPPED OVER AND ENTANGLED IN MOVING DRILL STEEL, RESULTING IN MUSCLE STRAIN TO UPPER RIGHT ARM, AND SOME HAIR LOSS.
