EE was installing roof bolt. EE went to push bolt up but hit rotation by accident, causing bolt to spin and strike EE's wrist. This caused a fracture in EE's left wrist.
While grinding on a steel pipe, a spark bounced off of the front of employees jacket, up under the welding hood, and safety glasses, contacting EE's left eye.
Employee was roof bolting, the employee did not properly seat the finishing drill steel. When the employee started to apply drill feed the finishing drill steel flung out of the drill chuck and struck the employee on the left side of the head. This resulted in a contusion.
While roof bolting, a drill steel fell out of the roof striking the employee's right index finger. The result was the tip of the finger is broken. The employee was wearing gloves at the time of the injury.
EE was roof bolting, as ee was dropping the bolter head, the head struck the second drill steel ee was holding in right hand. The second drill steel punctured right thigh.
EE was roof Bolting, as EE was dropping the bolter head, the head struck the second drill steel EE was holding in right hand. The second drill steel punctured EE's right thigh. EE returned to full duty on 02/22/2019. EE went back off work 2/17/2020. There was not a Reoccurrence of injury.
EMPLOYEE WAS CHANGING DRILL STEEL. THE DRILL STEEL STRUCK EE IN THE RIGHT HAND WHICH RESULTED IN A BROKEN BONE IN THE HAND.
The employee was bending a 6ft. roof bolt using the ATRS and his hand slipped causing his left arm to strike the bolt plate resulting in a laceration requiring 3 stitches.
The EE was helping his foreman bolt a cut and when he swung his drill head out to install his first bolt a small stone slid off of the canopy and struck his Rt. hand cutting his middle and ring fingers. 4 stitches total.
While bringing augers out of a drilled hole a piece of steel got embedded in his rt little finger which required 3 stitches to close the wound after removing the piece of steel.
While using a hand grinder a piece of material flew under his safety glasses and struck him in the left eye.
While roof bolting EE was adding drill steel to first auger and didn't get steel down into drill chuck, then raised drill head up. the steel caught on chuck which caused the steel to bow. He released pressure on steel causing the auger to spring back striking him on the right hand.
Employee while bolting roof fall, swung his drill head out to install outside bolt, placed his left hand on the top of the roof bolter canopy and crushed his hand between the canopy and the brow of the roof fall, breaking his middle and ring fingers.
EE was grinding on c/m bit lug, a peice of metal bounced off under his safety glasses into his right eye.
EE was cutting a piece of Conduit with an angle grinder. The fringing wheel was pinched resulting in the grinder jumping out of ee's hand. The grinding wheel struck the ee above the right knee requiring 6 stitches.
The roof bolter operator was guiding his drill steels into the drill head and inadvertently pinned his right index finger between the mine roof and the roof bolter head breaking right index finger.
Employee was installing 6' roof bolts with a Fletcher roof bolting machine when he bent a 6' bolt. He went to straighten the bolt and accidently hit rotation handle spun the bolt striking his right hand.
Had hand under metal cutting with torch. When ball released metal dropped and punctured hand (7 stitches) bolt was holding 2 pieces of metal together.
EE was installing a roof bolt on rib area. He slipped on loose coal, while trying to recover his balance, he grabbed the lower boom lever and pinned his foot. X-rays revealed 2 broken toes.
While grinding on a shuttle car bed, a piece of dust or metal entered the left eye.
EE was drilling a hole for a roof bolt installation. Drill steels stuck together. While trying to get them apart, the bottom one came loose and struck him in the face. (nose)
Ee was setting up to put bolt in. As he was lowering the drill pot his foot slipped on loose coal and he dropped drill pot on his rt. foot. He was taken to hospital x-ray showed no fractures. He went to another dr these x-rays also found no fractures. He went to a 3rd dr. These x-rays show a hairline fracture on 2/28/07.
While drilling a hole to insert roof bolts, a piece of stone became incircled around the auger. The rotation caused the stone to come apart and a piece struck him in the upper lip.
Employee was inserting a 6'bolt when it bent. He attempted to pull the bolt out & accidentally hit the rotation lever causing the bent bolt to strike his right hand & wrist. EE is on restricted duty.
EE was attempting to clean mud from drill pot using a lenght of wire. As he rotated the drill pot the wire wrapped around his left thumb causing a severe laceration and avulsion.
EE was kneeling with his back to the bolting machine, extending the line curtain. The bolter trammed forward running over his foot - The operator alerted him that the machine was going to move, EE thought his foot was clear. No lost time - on restricted days.
EE was installing a cable bolt, positioning the bolt plate so it would not hit T-bar. He got his right index finger pinched between the plate and the roof.
EE was in process of installing 6' bolt, as he picked up the bolt bent pinching hand between plate & bolt - Cause- safety awareness and improper hand positioning. Protective equip. not a factor, regulations not a factor.
