A roof fall occurred at 63 crosscut on 7B track. The fall was 4'high, 14' wide and 35' long. The area has been cleaned and re-supported.
A roof fall was discovered at approximately 6:14am. On the 1 Left B section belt. The roof fall is approximately 8' high, 16'wide, by 70' long
A roof fall was discovered in J Entry of the North A Belt area between crosscuts 3 and 4. The fall is approximately 8' high, 15' wide and 100' long. All accesses were dangered off.
EE was roof bolting a corner. As EE was retracting EE's side it caught the roof. Upon catching the roof it caused the slide out on the canopy to gap open EE caught EE's fingers in the gap.
Hopedale Mine elevator was down due to electrical problems. The elevator was repaired and placed back in service at 8:15 pm on 7/18/2025.
At approximately 12:30 pm A supply man discovered a roof fall on the mantrip bottom.
A roof fall was discovered on the East A Track at crosscut No. 9. The fall was cleaned and rebolted.
EE was removing stone that had fallen on the continuous miner. EE had both hands on a stone when their right hand slipped off causing the stone to fall. Which caused the stone to strike their left hand to strike the shovel.
EE was putting rub rail back on the continuous miner. EE caught EE's finger between the rail and the c/m.
At 11:20pm a call was made outside saying a roof fall was discovered on East A belt. At 11:22pm MSHA was notified. Upon investigation of the roof fall it was determined that the fall did not meet the criteria for reportable accident. Roof stone had fallen out from around the roof bolts. The area was approximately 14 feet long x 8 feet wide x 3.5 feet high.
What was originally thought to be a roof fall was actually a non-reportable occurrence. The immediate roof had fallen but was not above anchorage. The area was the track entry on 7 Seam A track at 55 crosscut. The area was cleaned and additional bolts installed.
A roof fall was discovered in the A Entry of the 7B belt line. The fall is approximately 16' wide, 8' high and 60' long. The fall has been posted and dangered. The fall will not need to be cleaned up.
EE was sitting on a 2 man johnson parked beside the miner. 2 other EE's was pulling the miner cable outby. They dropped the wrong loop of cable as they pulled the cable caught the johnson. The other EE that was on the johnson tried to get off it. EE was caught between the johnson and miner.

A unplanned roof fall was discovered at 7:10 am on the North A belt at crosscut 9. The fall was dangered off and posted. Rehab will be performed.
Weekly examiner encountered a roof fall in the primary escapeway ""Intake"" Located at 7 seam B at 35+11. Horizontal shifting. Approximate size 40 feet long, 17 feet wide and 9 feet high
Weekly examiner encountered a roof fall in the intake. Located at 43+67 F entry 7 seam B. Fall approx.18 feet wide x 18 feet high length is unknown due to an older roof fall in that area.
While checking the car buffer and counterweight buffer. Both buffers failed to work properly.
Roof fall was found on 7 seam B Belt. Fall measured 8 feet high x 18 feet wide x 30 feet long.
EE was traveling to the slope bottom. They encountered a roof fall in North A track entry. Weathering was the cause of the roof fall.
A fully trained miner was installing a conveyor chain on a miner when a rock fell from the top hitting the miner in the wrist. The accident was not the result of any compliance issues nor were any protective guards involved.
EE was bolting EE was drilling the second hole in the last row of bolts at the face. When EE was struck by a rock from the roof.
EE was running a shuttle car, came off a ledge, hit tailbone on the seat
EE was helping change a bearing on a tailpiece. Another EE struck a chisel which flew 5 feet striking the EE right side of EE's face and right eye.
Pre shift examiner discovered a roof fall in the 7 seam B track entry. Fall was approximately 5-6 feet high, 14-15 feet wide X 35-40 feet long.
While EE stepped out of the shuttle car EE twisted their left ankle.
Pre shift examiner encountered a roof fall on the 7 seam B track entry at 57+12. Fall was approximately 3-4 feet high 15 feet wide by 30 feet long.
EE was riding on a 12 man rubber tire personal vehicle. EE turned their head to talk to another EE. As injured EE turned back their head struck a brow. EE had pain in neck.
While moving C/M in cut, rock fell from roof striking lower right leg.

At 7:00 am a roof fall was found on the 7 seam A beltline. The roof fall was approximately 18 feet wide X 18 feet long X 3.5 feet high.
AT approximately 10:50 pm discovered a roof fall had occurred at 49+40 on 7B track entry . Fall was approximately 14 feet wide x 22 feet long.
EE was cutting the jacket on miner cable. Second EE had pliers on the jacket pulling it. First EE slipped with knife cutting second EE's fingers.
EE was walking down the tram road stepped in a hole felt pop in knee.
A water line burst underground. The situation was not discovered until Monday morning approximately 8:30am. The water line break caused the track and intake entries to flood stopping travel. The water is located 7 seam B from 23 to 28 x-cut and from 33 to 35 x-cut
Weekly examiner was making the weekly run. The examiner encountered a roof fall. The roof fall was at 19+00 in 7 seam B intake. The fall measured approximately 7 feet high, 18 feet high, by 60 feet long.
EE was putting crib blocks under the feeder. As EE was shoving a crib block underneath the feeder EE struck EE's left thumb. Which lacerated EE's thumb.
During the weekly exam the examiner encountered a roof fall in the return air course. The fall was at 42 x-cut 7A return. Fall measures approximately 60 feet long x 17 feet wide x 10 feet high. The fall had stack rock strata.
While traveling 5Left weekly air course, A roof fall was encountered just inby spad # 20+41 in F entry, measuring approximately 50' long by 17' wide by 7,high.
At 3:40am a roof fall was encountered on the 1 south section. Fall was inby the tailpiece. Location of the fall A entry @ 0+59 to 1+20. Fall is approximately 60 feet long,18 feet wide,by 12 feet high.
EE was plugging charger cable into a battery scoop. EE felt a pop and burning sensation in right elbow.
EE was roof bolting over loose roof material that had fallen. EE was lowering the boom on the bolter when EE's left knee slid under the boom striking EE's left knee.
EE was roof bolting. As EE reached for the drill a rock fell striking EE's right forearm. The rock was approximately 18 inches by 18 inches by 3 inches thick. The rock had sharp edges lacerating EE's arm. EE received 6 stitches.
EE was stepping out of EE's shuttle car. EE stepped on a rock with EE's left foot. Felt a pop and pain in EE's ankle.
EE was roof bolting. EE had installed 2 pattern bolts in the row. EE felt the roof needed an additional bolt in that row. EE swung over to EE's rib to drill the bolt hole. As the EE was putting EE's drill steel in EE's chuck. A kettle bottom fell from the roof striking EE's left hand.

Weekly examiner found roof fall at 1:55 pm in 1 Right section at 2+00 in F entry. Approximately 15 feet wide 20 feet long height is above the anchorage.
EE was in the process of applying Block bond on when some of it splattered into EE's left eye
EE was roof bolting. A piece of stone fell from the roof. Striking EE on right neck, shoulder, and arm.
EE was bolting on opposite side. The bolter operator went to move the bolter forward. EE did not notice EE's left foot was in the way the bolter tire ran upon EE's left foot.

While bolting, the EE's cuff of long sleeve shirt was caught by a burr on the drill chuck. The rotating drill chuck pulled the EE's arm into the drill chuck, fracturing the E's left arm.
North A belt entry at 4+65 had a roof fall at that intersection. Approximately 18 feet wide and 40 feet long.
A roof fall measuring approximately 17 feet wide by 35 feet long by 6 feet high occurred. The roof fall was located on 7 seam B, E entry at 14 crosscut.

While Traveling to the section on a 2 man Johnson. A drop of water fell from the mine roof and struck the employee in the right eye. The employee was wearing EE's safety glasses at the time of the incident.
EE was installing roof bolt. EE went to push bolt up but hit rotation by accident, causing bolt to spin and strike EE's wrist. This caused a fracture in EE's left wrist.
EE was adding knife blades in storage compartment on knife. A blade slipped out when closing compartment, cutting through glove and into ring finger on right hand. EE received four stiches in finger.

Roof fall occurred at 39+15 B entry on 7 Seam B belt line. The fall was the intersection, 17 feet wide, 20 feet long and 8 feet high.
EE WAS SPOT BOLTING TRACK ENTRY WHEN A PIECE OF ROOF FELL STRIKING EE IN EE'S LEFT ARM AND HAND
EE WAS RECOVERING RAIL. LEFT MIDDLE FINGER WAS STRUCK BY A HAMMER CAUSING A FRACTURE.
Setting arches when co worker went to put a crib under support and let IE take the weight of the steel set.
EE WAS TRAVELING OUT OF THE MINE FOREIGN MATERIAL FELL FROM THE TOP HITTING THEM IN THE FACE AND EYES
EE WAS BOLTING. A ROCK CAME DOWN HITTING EE'S HAND ON THE ROOF BOLTER FRAME MASHING THE MIDDLE FINGER ON RIGHT HAND.
EMPLOYEE WAS PICKING UP MINER CABLE TO HANG FELT BURNING PAIN IN LOWER LEFT BACK
Two elevator repairmen rode down on the top of the elevator on inspection mode, the elevator went into a fault state and stopped. The repairmen were trapped on the elevator for approximately 45 minutes.
The Employee was shoveling the 1 Left B belt take-up. As the employee was shoveling material onto the belt, employee stepped back into the slack belt take-up ropes twisting right ankle.
The EE was hanging ventilation curtain when a piece of shale became dislodged striking the EE on the top of EE's Right hand. The resulting injury required stitches.
A roof fall was found in F entry of 7 south at 43+90. Fall dimensions 17 feet wide 50 feet long 7 feet high.
ROOF FALL f ENTRY AT 11+89 7 SEAM B INTERSECTION. FALL WAS APPROXIMATELY 7 FEET HIGH BY 17 FEET WIDE BY 30 FEET LONG.
Employee was roof bolting, the employee did not properly seat the finishing drill steel. When the employee started to apply drill feed the finishing drill steel flung out of the drill chuck and struck the employee on the left side of the head. This resulted in a contusion.
While roof bolting, a drill steel fell out of the roof striking the employee's right index finger. The result was the tip of the finger is broken. The employee was wearing gloves at the time of the injury.
At 5:30 pm the continuous miner intersected an uncharted gas well. The location of the well was 1 room off of an entry at spad location 3+45 on the 3 left section.
The Employee was changing out a belt top stand on North A belt. As they lifted the top stand into place, they felt pain in the lower back.

EE was helping hang miner cable, when EE felt a pop in upper back.
While loading Rail on car, the rail slipped out of the clamps. When the rail hit the ground the rail bounced striking EE on the foot.
EE was roof Bolting, as EE was dropping the bolter head, the head struck the second drill steel EE was holding in right hand. The second drill steel punctured EE's right thigh. EE returned to full duty on 02/22/2019. EE went back off work 2/17/2020. There was not a Reoccurrence of injury.

EE was splitting a roof bolter tram chain, using a hammer and a chisel. After striking the chisel with a hammer, the chisel flew up striking the employee in the mouth. This resulted in a broken tooth.
Roof Fall occurred in the 7 Seam B Return. The Fall was located in B entry at 5+15. The dimensions are 40 feet long, 18 feet wide by 7 feet high.
Employee was working in the continuous miner contactor case when the employee received an electrical shock.
During a thunder storm, an electrical phase burned in two causing a power outage. The power line supplied power to the bath house and the elevator. An employee was on the elevator at the time of the power outage. The employee was stranded on the elevator for approximately 65 minutes.
Tramming bolter; rock fell from top striking IW on left shoulder
EMPLOYEE WAS CHANGING DRILL STEEL. THE DRILL STEEL STRUCK EE IN THE RIGHT HAND WHICH RESULTED IN A BROKEN BONE IN THE HAND.
EE was stepping out of a mantrip, as EE got out EE stepped on a piece of coal twisting right ankle causing pain and swelling.
Employee was working along the Section E beltline. Employee was dragging water/pipe line and tripped over other pipe line in the work area.
A roof fall above anchorage was found by the weekly examiner in B Entry of the 7 seam B section. This fall was 90x16.5x7.The fall blocked the airway for travel but did not alter ventilation. The area was dangered off on all approaches. There were no injuries to this accident. This area was mined in Oct. 2014. The travelway was rerouted around the fall.
When throwing a crib block under a shuttle car ee smashed right ring finger between the crib block and the shuttle car frame. This accident caused a laceration to finger and broken bone in the finger also.
EE was pulling cable off of a roof bolter and when ee turned to walk away ee ran into a roof mat that was bent down on the end causing a laceration to right ear which required 7 stitches to close the wound.

EE's were putting inserts in the lugs on the miner, they were using a sledge hammer to drive the inserts in when an EE hit the lug the sledge hammer glanced off the lug and hit this EE in the mouth. This accident chipped 1 tooth and knocked the cap off of another tooth.
A roof fall above anchorage was found by the Mine Foreman in the 7 Seam A Entry return @ 36+75. This fall was approximately 60'x 17'x6.5. There was no injuries with this accident. The travelway was blocked but did not effect ventilation. The area was dangered off at all approaches and travelway was re-routed around the fall. This area was mine 9-1-2014.
EE was riding on a 2 man rubber tired Johnson and ran over some gob in the roadway causing ee to jam head into a roof strap. EE felt pain in neck and shoulders and into both hands.
The 480 volt main feed power line going to the elevator had a phase burn into causing the elevator to stop. There was 1 miner on the elevator at the time of this accident. The line was repaired and the power was put back on at 12:00 pm 10/09/2017 and the miner was brought outside. There were no injuries with this accident. Schindler Elevator Company was called and on sight.
EE was walking down B Entry bent over going to scoop charging station when EE hit EE's head on a low brow, EE felt a pop in the left side of EE's neck with pain and had difficulty turning EE's head left and right.
EE was going down a set of steps holding on to the handrail when EE's helper threw a come-a- long over the handrail, the come-a-long had a broken cable in it which stuck out 3', the cable struck EE on the top of the hand causing a laceration to the top of EE's left hand requiring 7 stitches to close the wound.
A roof fall above anchorage occurred in the 003-0 7 South B-C entries at spad #44+05. This fall interrupted ventilation by knocking part of a return wall out. A new wall was built and the area was rebolted and floor to roof support was installed. This area was mined in April of 2017 and was due to normal weathering. There were no injuries with the accident.
EE was tramming the roof bolter from the side controls when stone fell from the rib line and the bolt knocking EE to the ground, then more stone fell and struck EE on the right leg and ankle causing a fracture to leg and a laceration to ankle requiring 2 stitches to close the wound.
Employee was carrying a box of resin glue to put on the roof bolter, when employee twisted to put the glue in the glue tray employee felt pain in employee's lower back and pain going down employee's left leg.
This employee was dragging a shuttle car cable around the block when EE caught EE's foot on a stone and started to fall. When EE tried to catch self EE jammed EE's leg into the ground, which jammed EE's leg into EE's hip area. EE also states that EE landed on EE's Proximity box that was on EE's belt. This fall caused 2 fractures to EE's left hip.
A fall above the anchorage was found by the weekly examiner in the 4 South intake escapeway blocking the travelway at spad location 4+67. This fall was due to normal weathering of the area as the roof bolts broke off. The area was dangered off and the escapeway was rerouted. This area was mined in 12/07 with fully grouted 6' bolts.The fall is 50'Lx6'Hx18W.
A roof fall above anchorage was found by the weekly examiner. This fall was due to normal weathering. The fall was 18x60x6 and blocked the travelway of the examiner. There were no injuries with this accident. The ventilation was not interrupted and the area has been dangered off at all approaches. This area was mined in Nov. 2014.
This area is part of the 7 day exam found by the examiner on their normal run. A roof fall was found which was 18x20x6. Travel way was blocked due to the fall, ventilation was not affected. The fall is due to normal weathering. This area was mined in March 2006. There were no injuries with this accident. This fall was located in the West B right side return at spad 37+64 H entry.
While just starting to mine in the F entry of 7 South 003-0, the miner cut into an uncharted oil or gas well. This well was uncased. There were no injuries with this accident.

The employee got left wrist pinched between the foot jack assembly and the frame of a roof bolter trying to put a keeper pin in the foot jack.
While filling the pod duster, the employee thought that the outside tank was empty as EE was not getting dust. EE then unhooked the fill hose from the duster and didn't know that the line was plugged and had pressure. The hose flew into EE's left hand causing a laceration. The employee was wearing gloves at the time.
At 10:13 am on 1/20/2016 a roof fall above the anchorage was found by the maintenance foreman on the 7left off 4North track at spad #8+59. The fall was approx.17.5' wide x 17' long x 6.5' high. The area had been mined in 8/14 and was supported with 6' fully grouted bolts and 10' cable bolts. There were no injuries or property damage. The fall is dangered off.

The weekly examiner found this fall on his weekly run in G entry of the North B return at spad #5+99 The fall was approximately 20'long and 17'wide and 6'high. This area was mined in 5/1/2006 and was supported with 6' fully grouted bolts. No injuries or property damage. The fall is blocking the return as is being cleaned up and supported.
During routine x-ray examinations, it was discovered that the rope on the supply hoist was bad. The hoist was immediately taken out of service and plans were made to install a new rope. There were no injuries or property damage due to the incident.
While knocking cement block out of an old regulator, the employee was struck by a piece of stone measuring approx. 3'x 4'x 3"". The stone knocked the employee to the ground and the employee received fractures to the sternum and back.
On 10/31/2015 at approx. 10:30 am a roof fall above the anchorage was fond on the 4 North track at spad #24+47. The area had been mined in 2009 and was supported with 5 ft. fully grouted bolts. There were no injuries or property damage. The fall will be cleaned up and the area re-supported. The mine was idle at the time of discovery. Normal weathering may have been a factor.
The employee bent over to pick the mine cable and waterline up to throw out of the tram road when EE felt pain in EE's lower back and radiating to EE's groin area. EE was on restricted duty from 10/16/15 and ended 10/25/16. EE was placed on S/A on 10/27/16. At this time BWC has considered EE a lost time accident as of 6/7/17 since EE has not returned to work in any capacity.
While installing the center bolt in the face area of an entry, a piece of stone fell from inby the T-bar and rolled backwards striking the employee on the back of the right foot as he attempted to move out of the way. The incident resulted in fractures to the employee's foot and toes.
At 10:00 am the weekly examiner found a roof fall above the anchorage in the 3 left off 4 North E entry return at spad #0+20. The area was mined in 12/2009 and was supported with 5' fully grouted bolts. The fall was approx. 25' long 17' wide and 6' high and was temporarily blocking travel. There were no injuries or property damage. Travel will be re-routed.
At approx. 3:50 am the weekly examiner found a roof fall above the anchorage in the 4 North F entry primary escapeway just inby spad #32+30. The fall was about 30' long 17' wide and 6' high. The area was mined in 11/2009 and was supported with 5' fully grouted bolts. There were no injuries or property damage. The fall is blocking travel and will be supported and cleaned up.
While drilling a hole for a roof bolt, a piece of stone broke off and struck the employee on the outside of the left leg bending the leg to the side. The employee continued to work but he had surgery done on his left knee on 11/24/2015 due to the injury.
At approx. 11:07 AM, a roof fall above the anchorage was discovered on the North A belt line. The fall was approx 30'long x 17.5' wide & 6' high. This area had been mined in Oct. of 2005 & was supported with 6'fully grouted bolts. There were no injuries due to this accident. The area was re-supported and then cleaned up. Weathering may have been a factor causing this roof fall.
While helping to clean stone off of the top of the continuous miner, a piece of sharp stone cut through the employee's glove and lacerated his right index finger. The laceration required 3 stitches.
While installing roof bolts in the x-cut 5-6 room, the employee installed a rib bolt and then swung in to install the middle bolt. When he swung the bolter head in, a piece of stone approx. 24""x12""x1"" fell from between his last bolt and the rib and struck him on the calf of the right leg. The accident resulted in a non-displaced fracture to his right fibula.
While walking past the slope belt tail piece, the employee slipped on some loose wet material and slid onto the gob clean out door. The door slid open and he fell approx. 5.5 feet onto the slope track. The fall resulted in three small fractures to his back.
After pulling the lead car to the surface, the hoist operator forgot to throw the switch for the supply spur and he disengaged a coal car used for clean up from the lead car. He then was trying to guide the car into the spur when the car shot down the slope and contacted another empty car on the slope bottom. There were no injuries due to this accident.
At approx. 3:45 pm a roof fall above the anchorage was discovered on the 7 Left 4 North section in the x-cut B-A at survey spad 34+99. The area had been mined in Feb. and was supported with 6 ft. fully grouted bolts. The fall was approx. 17 ft. wide 40 ft. long and 6.5 ft. high. There were no injuries or any property damage.
While trimming a belt splice with a utility knife, the knife slipped and the employee lacerated his right thumb requiring 8 stitches. The employee was wearing gloves at the time of the accident.
At approx. 4:40 am, the elevator malfunctioned with one miner on it. The malfunction was due to extreme cold temperatures knocking the heater off and not allowing the fluids to go through the pump to cycle the system. There were no injuries or property damage due to this incident. The miner was able to exit the elevator on the surface at 5:50 am.
At approx. 11:38 am, a roof fall above the anchorage was discovered in the 4 North B entry return at spad No. 59+64. The travel way was blocked and ventilation was interrupted. The area had been mined in 2013 and was supported with 6 ft. fully grouted resin bolts. Plans were made to re-route the return and re-establish ventilation. There were no injuries or property damage.
While bending a roof bolt to push it in the hole, the employees hand slipped off of the bolt and he struck his Rt. forearm on the bolt plate causing a laceration requiring 3 stitches. the employee was wearing gloves at the time of the accident.

While cutting a brattice board with a buck saw, the saw jumped and cut the top of the employees left hand requiring 5 stitches. The employee was wearing gloves at the time of injury.
While walking toward the face of the E entry on the 7Left off 4North section, the employee accidentally hit his head on a roof mat that was slightly bent down causing a laceration on the left side of his head. The laceration required 6 stitches.
Injured stated he was pulling down wire screen when a piece of wire went through his glove and cut his finger.
At 1:12 pm on 8/28/2014 a roof fall above the anchorage was found in the North A intake escapeway at spad #13+90. The fall was approx. 30 ft.long x 18ft.wide and 6.5ft.high. The area had been mined in 2005 and was supported with fully grouted resin roof bolts. There were no injuries or property damage.
At 7:10 am on 07/29/2014, a roof fall above the anchorage was discovered in the North A G entry intake at survey spad #13+30. There were no injuries or property damage. This fall will be rebolted and cleaned up. This area was mined in 2005 and is part of the 7-day exam. It was supported with 6ft fully grouted bolts and appears to have been caused by normal weathering.
Injured employee stated he was preparing to drill a bolt hole, put the drill steel against the roof and as he was withdrawing his hand a piece of rock fell and struck him on the right hand resulting in a laceration to his index finger.
Injured employee stated he was walking and hit his head on a roof bolt hanging down with plate attached.
Injured employee was assisting in installing track. A rail had been jacked up to line the other end up to bolt to another rail. While lowering the rail the jack slipped and the rail fell on the injured employees right foot.
Injured employee stated he was changing belt rollers, tripped and fell on a rock causing a laceration to his right hand.
At approx. 5:35 am, a roof fall at or above the anchorage was discovered in the A entry return of the 4North Mains. The fall was approx. 20 ft long 18 ft. wide and 6 ft. high. There were no injuries or property damage. The fall was dangered off and the return was rerouted.
Injured employee stated he was swinging the twin boom roof bolter boom to install another bolt when loose material fell and struck him on the back of his head and neck.
At approx. 3:00 am, an employee became trapped on the elevator due to extreme cold temperatures affecting the over travel switch. The employee was able to get off at 4:18 am with no injuries.
At approx. 6:30 am 11 employees became trapped on the elevator due to extreme cold temperatures affecting the over travel switch. The employees were able to get off at 7:11 am with no injuries.
At 3:10 am two EE's were trapped on the elevator when in went down due to cold weather.The car safety switch and counter weight buffer were adjusted and the elevator was repaired at 4:54 am. The EE's were able to get off at that time. There were no injuries.
At approx. 12:10 am on 1/5/2014 a roof fall above the anchorage was found in the F (intake) entry of the 6Left off 4North section. The roof fall was at survey spad #21+10 and extended inby to about 21+50. The fall was about 40ft. long 17ft. wide and 6.5ft. high. All areas were dangered off around it and the escapeway was rerouted. There were no injuries or property damage.

At approx. 4:10 pm, a roof fall above the anchorage was discovered in the 4North intake escapeway just inby spad no. 52+65. The fall was approx. 40ft long 16ft wide and 6.5ft high. There were no injuries or property damage. The fall was dangered off and the escapeway re-routed. The area had been supported with 6ft resin assisted bolts.
At approx. 5:35 pm, a roof fall above the anchorage was discovered in the intake entry of the 6Left off 4North section. The roof fall was in the intersection at 14+00 and continued outby for approx. 40 ft. The roof fall was approx. 40 ft. long x 17 ft. wide and 6.5 ft.high. There were no injuries or property damage. The fall was dangered off and the escapeway re-routed.
At approx. 6:30 pm a roof fall above the anchorage was found in the 6left 4North track entry at spad #12+71. The fall was approx. 40'long x 18'wide and 8'high. The fall was dangered off on all sides and extra cribs were set. There were no injuries or property damage.
At approx. 3:20 pm, a roof fall above the anchorage was found in the intersection of the A entry of the 4North 6Left section. The fall was dangered off on all sides and the return was re-routed. There were no injuries or property damage. The fall was approx. 70'long x 18'wide and 6.5'high.
After oiling the hoist rope, the EE disconnected the hose on the oiler. The hose still had slight pressure on it and the oil sprayed on his right index finger. According to his doctor, the oil was absorbed into the skin causing an infection which had to be cleaned out by a surgeon.
Employee stated he was roof bolting, changing a drill steel when a rock fell and struck his right middle finger.
Injured employee was walking through a x-cut when a piece of rock fell and struck him on the back of his head.
While scaling down a piece of stone, the employee was struck on the right foot by a piece that he did not see falling. This resulted in a fracture to his right little toe.
The employee was bending a 6ft. roof bolt using the ATRS and his hand slipped causing his left arm to strike the bolt plate resulting in a laceration requiring 3 stitches.
The employee was reaching over to get a roof bolt, that was leaning on the rib, and was struck on the top of the hand, above the glove line, by a piece of stone that fell between the bolt and the rib. The stone caused a laceration that required 4 stitches.
The EE was walking across the section faces. When he walked through the cross cut between the E and F entries, he ran into a bent roof bolt which cut the back of his neck requiring 5 stitches.
The EE was bending a 6 ft. roof bolt and twisted his left knee. There were no lost or restricted days at the time of the injury. This injury became worse after time and the EE required surgery on 05/01/2013.
While trimming the leads on a battery operated motor to install a new plug, the knife slipped and lacerated the employees left hand between the thumb and forefinger requiring stitches.

While walking in the neutral B entry of the 5 left off 4 north section, the EE heard a cracking sound in the roof. As he turned to observe a piece of stone fell and struck him on the left leg fracturing his ankle.
Employee was putting old rollers into the backend of mantrip when he hit his left hand with a roller against the frame of the mantrip fracturing his left middle finger,
While observing the roof bolters bolting a cut, the supervisor laid his on top of the bolter near the mid section to rest himself. A piece of stone approx. 12"" X 12"" X 2"" fell and struck his right thumb causing a laceration requiring stitches.
Employee was roof bolting in our 5 North Section when a stone fell out of the top striking his R hand and wrist, cutting his middle finger and ring finger and also cutting his R wrist. A total of 11 stitches were required to close the cuts.
The belt examiner found a roof fall 20'l.x 25' w.in the W. C return in the B entry at spad#21& 40 which damaged a return stopping. The area had been bolted with 6' fully grouted bolts. The area was dangered off and the stopping was rebuilt. No injuries.
At approx. 2:05 p.m. a roof fall occurred in the D entry of the 2 right section. The roof fall was approx. 20' x 20' and the height unknown. There were no injuries. The roof fall was dangered off on all sides.
A roof fall 20'W. x 40'L. X 16'H. occurred at approx 9:32 a.m. on the 2 right section causing the entrapment of two miners. The roof fall was in the E. entry at spad#21+35. The miners were rescued at 6:56 p.m. with no injuries. The miners were bolting the x-cut E to F.
While pulling a piece of stone off of the top of the continuous miner, the stone slipped in the employees hand causing a laceration on the right little finger requiring four stitches.
At approx. 12:10pm we mined into an uncharted oil well on the 2 right off 4 left section in the #5 return room airleg 5 to 1 at spad #1+85. There were no injuries or property damage. The well has been plugged and vented.
While helping set miner bits, the employee was resting his right hand on top of the shovel of the CM, when a piece of stone slid off the top of the CM and struck him on the right little finger causing a fracture.
EE just finished bolting a cut when he went to set down on the bolter he heard a cracking sound as he looked up a stone fell from the roof and hit him in the face, which caused a laceration to the bridge of his nose and to the left cheek requiring 2 stitches in each cut.
The EE was helping his foreman bolt a cut and when he swung his drill head out to install his first bolt a small stone slid off of the canopy and struck his Rt. hand cutting his middle and ring fingers. 4 stitches total.
At approx. 11:35 PM a roof fall above the anchorage occurred in the main west rooms @ the 2+50 of the #3 room intersection. The fall was approx 17' X 20' and 6 ft high. The area was dangered off. There were no injuries or property damage.
While getting on the Damascus personnel carrier the employee had a drill auger in his hand and put the auger on the machine against something solid and when he attempted to get on the machine his auger jammed him in the lower abdomen causing a large bruise.
While dropping the miner cable out of hi-line hooks, the miner moved and he caught his right index finger between the cable and the hook lacerating his finger. It required 7 stitches to close the wound. Note 4/24-25-26 2012 was regular scheduled days off.
At approx. 9:25 PM a roof fall above the anchorage was found in the E entry of the 3 right off 4 left off 4 North section. The fall was about 30ft long X 18ft wide and 6-7 ft high. The area was dangered off. There were no injuries or property damage. Fell at 8+15.
While securing a block stopping, he was hitting a wooded wedge cap when it split and a piece of it hit him in the upper lip which required 4 stitches to close the laceration.
At approx. 3:05p.m. a roof fall above the anchorage was discovered in the old #4 room of the 4 left off 4 North section return side. The fall was 17' W. X 300' L. X 8' H. The area was dangered off on all sides. Travel and air flow are not affected. No injuries or property damage.
At 9:10 AM a roof fall above the anchorage occurred inby the tail piece of the 3 right off 4 Left off 4 North section in the A entry intersection at survey spad #9+35. There were no injuries or property damage. The roof fall was dangered off.
A roof fall above the anchorage occurred in the neutral E entry of the west C mains at spad # 10+67. This area had been belted with 6ft fully grouted bolts. The area was dangered off and cribs set. The fall was 18' wide, 40'long, 7"" high.
While bolting the bolter operator put his first auger against the roof and hit his rotation. A piece of stone broke off and struck him lacerating his lip and chin requiring 5 stitches.
While bringing augers out of a drilled hole a piece of steel got embedded in his rt little finger which required 3 stitches to close the wound after removing the piece of steel.

At approx. 8:10 AM a roof fall above the anchorage was found in the old #4 room and extending to the #2 room of the 4 rt. off 4 North Section. The fall measured approx. 150' long by 18' wide and 7' high. The fall started at 1+00 in the old rooms.
While cutting wire ties with a utility knife he cut through tie while holding other end of tie and cut his left hand which required 3 stitches to close the cut.
A roof fall occurred in the 4 right/4 left North Return side rooms above anchorage point of roof bolts in the first intersection at 0-150 of the #2 room. All approaches to the intersection were dangered off.
At 4:30 pm a roof fall was discovered in the 3 Left/1 East section above the anchorage point of the bolts. This fall was in the old set of Return Rooms at 1+00 intersection of the #4 Room. This Room was turned off 'A' Entry at 28+07. This area is part of the 7 Day Examination.
While using a hand grinder a piece of material flew under his safety glasses and struck him in the left eye.
While kneeling down a stone fell out from between the bolts and hit him on the back and knocked him to the ground.
A roof fall above the anchorage was found in the 4 South intake escape way. The area was dangered off and the escape way was rerouted. The area had been bolted with 5' resin and 12' cable bolts. The fall was 18' X 80' and 6' above the seam.
On 9/19/11 at 10:25 a.m. we had dust and or methane ignition on our 4 right/4 north section in the first x-cut 4-5 room at 0+50.
While releasing the rope come-a-long it flew back and hit him on the (L) cheek bone causing a laceration which he received 5 stitches.
wHILE TALKING TO CO-WORKER THE INJURED PERSON HEARD A CRACK IN THE TOP AND HE TURNED AND LOOKED UP AND A STONE FELL FROM THE TOP STRIKING HIM IN THE LEFT CHEEK AREA AND LACERATING HIS CHEEKBONE WHICH HE RECEIVED 4 STITCHES
At approx. 3:00 p.m. a roof fall above the anchorage occurred on the 8 south section in old #5 rm. The fall is in old works and was dangered off on all sides. No clean up on the fall will be done.
At approximately 8:00 am on 8/13/11 a possible ignition occurred in the A entry of the Rt left section off the 4 left north section. There were no injuries or property damage.
Roof fall above anchorage in 4North main intake at spad 2 & 80 approx. 18 x 18'.
While trying to hold the shuttle car cable the shuttle car was energized - the cable pinched his hand between the cable and the frame of the shuttle car lacerating his right middle finger.
While roof bolting EE was adding drill steel to first auger and didn't get steel down into drill chuck, then raised drill head up. the steel caught on chuck which caused the steel to bow. He released pressure on steel causing the auger to spring back striking him on the right hand.
Climbing out of the kitchen of the Fletcher roof bolter when he twisted his right knee - originally diagnosed as a sprain. But got worse with time and then on 10-06-11 had to have surgery to repair knee.
Roof fall fell above anchorage in 4 south intake escapeway at # 2 & 50 of #3 room. Approx 16' x 18'.
Employee was helping scoop operator supply roof bolter when his hand was pinched between the scoop car and ejector blade and side of the bucket. Employee received 3 stitches to the back of his hand.
EE was helping splice a cable. The knife slipped and cut his left little finger, requiring 4 stitches.
Employee was suppling roof bolter when he backed away from the bolter rear tire and struck a 12"" strap and hit the employee under left arm. Requiring 2 staples.

While tramming a loaded shuttle car away from the face the car hit a bump and roofed out causing the operators head to strike the canopy.
Employee while bolting roof fall, swung his drill head out to install outside bolt, placed his left hand on the top of the roof bolter canopy and crushed his hand between the canopy and the brow of the roof fall, breaking his middle and ring fingers.
Roof fall 6 South Rooms (intake) 2 Room at 1+50 intersection adjacent to 19+28 F Entry. 17 ft wide, 20 ft long, 6-7 ft high.
Elevator's control cable was damaged entrapping 2 miners.
Roof fallat 32 & 45 B entry, west B return.Size of fall was 8' High X 25' Long X 20' Wide. Ventilation was not affected.
The continuous miner operator was struck in the back by a shuttle car while checking his sight in the belt entry.

At approx. 6:05 am on 4/24/2010 I was told that our elevator went down with 1 employee stuck on it. He was able to get off at approx. 8:30am. The problem was a short in a door switch due to a bad trailing cable.
