While Traveling to the section on a 2 man Johnson. A drop of water fell from the mine roof and struck the employee in the right eye. The employee was wearing EE's safety glasses at the time of the incident.
EE was installing roof bolt. EE went to push bolt up but hit rotation by accident, causing bolt to spin and strike EE's wrist. This caused a fracture in EE's left wrist.
EE was adding knife blades in storage compartment on knife. A blade slipped out when closing compartment, cutting through glove and into ring finger on right hand. EE received four stiches in finger.
Roof fall occurred at 39+15 B entry on 7 Seam B belt line. The fall was the intersection, 17 feet wide, 20 feet long and 8 feet high.
While grinding on a steel pipe, a spark bounced off of the front of employees jacket, up under the welding hood, and safety glasses, contacting EE's left eye.
EE WAS SPOT BOLTING TRACK ENTRY WHEN A PIECE OF ROOF FELL STRIKING EE IN EE'S LEFT ARM AND HAND
EE WAS RECOVERING RAIL. LEFT MIDDLE FINGER WAS STRUCK BY A HAMMER CAUSING A FRACTURE.
Setting arches when co worker went to put a crib under support and let IE take the weight of the steel set.
EE WAS TRAVELING OUT OF THE MINE FOREIGN MATERIAL FELL FROM THE TOP HITTING THEM IN THE FACE AND EYES
EE WAS BOLTING. A ROCK CAME DOWN HITTING EE'S HAND ON THE ROOF BOLTER FRAME MASHING THE MIDDLE FINGER ON RIGHT HAND.
EMPLOYEE WAS PICKING UP MINER CABLE TO HANG FELT BURNING PAIN IN LOWER LEFT BACK
Two elevator repairmen rode down on the top of the elevator on inspection mode, the elevator went into a fault state and stopped. The repairmen were trapped on the elevator for approximately 45 minutes.
The Employee was shoveling the 1 Left B belt take-up. As the employee was shoveling material onto the belt, employee stepped back into the slack belt take-up ropes twisting right ankle.
The EE was hanging ventilation curtain when a piece of shale became dislodged striking the EE on the top of EE's Right hand. The resulting injury required stitches.
A roof fall was found in F entry of 7 south at 43+90. Fall dimensions 17 feet wide 50 feet long 7 feet high.
ROOF FALL f ENTRY AT 11+89 7 SEAM B INTERSECTION. FALL WAS APPROXIMATELY 7 FEET HIGH BY 17 FEET WIDE BY 30 FEET LONG.
Employee was roof bolting, the employee did not properly seat the finishing drill steel. When the employee started to apply drill feed the finishing drill steel flung out of the drill chuck and struck the employee on the left side of the head. This resulted in a contusion.
While roof bolting, a drill steel fell out of the roof striking the employee's right index finger. The result was the tip of the finger is broken. The employee was wearing gloves at the time of the injury.
At 5:30 pm the continuous miner intersected an uncharted gas well. The location of the well was 1 room off of an entry at spad location 3+45 on the 3 left section.
The Employee was changing out a belt top stand on North A belt. As they lifted the top stand into place, they felt pain in the lower back.
EE was helping hang miner cable, when EE felt a pop in upper back.
While loading Rail on car, the rail slipped out of the clamps. When the rail hit the ground the rail bounced striking EE on the foot.
EE was roof bolting, as ee was dropping the bolter head, the head struck the second drill steel ee was holding in right hand. The second drill steel punctured right thigh.
EE was roof Bolting, as EE was dropping the bolter head, the head struck the second drill steel EE was holding in right hand. The second drill steel punctured EE's right thigh. EE returned to full duty on 02/22/2019. EE went back off work 2/17/2020. There was not a Reoccurrence of injury.
EE was splitting a roof bolter tram chain, using a hammer and a chisel. After striking the chisel with a hammer, the chisel flew up striking the employee in the mouth. This resulted in a broken tooth.
Roof Fall occurred in the 7 Seam B Return. The Fall was located in B entry at 5+15. The dimensions are 40 feet long, 18 feet wide by 7 feet high.
Employee was working in the continuous miner contactor case when the employee received an electrical shock.
During a thunder storm, an electrical phase burned in two causing a power outage. The power line supplied power to the bath house and the elevator. An employee was on the elevator at the time of the power outage. The employee was stranded on the elevator for approximately 65 minutes.
An open flame was observed on the raw coal stockpile at the preparation plant.
Tramming bolter; rock fell from top striking IW on left shoulder
EMPLOYEE WAS CHANGING DRILL STEEL. THE DRILL STEEL STRUCK EE IN THE RIGHT HAND WHICH RESULTED IN A BROKEN BONE IN THE HAND.
Employee was working along the Section E beltline. Employee was dragging water/pipe line and tripped over other pipe line in the work area.
EE was stepping out of a mantrip, as EE got out EE stepped on a piece of coal twisting right ankle causing pain and swelling.
A roof fall above anchorage was found by the weekly examiner in B Entry of the 7 seam B section. This fall was 90x16.5x7.The fall blocked the airway for travel but did not alter ventilation. The area was dangered off on all approaches. There were no injuries to this accident. This area was mined in Oct. 2014. The travelway was rerouted around the fall.
When throwing a crib block under a shuttle car ee smashed right ring finger between the crib block and the shuttle car frame. This accident caused a laceration to finger and broken bone in the finger also.
EE was pulling cable off of a roof bolter and when ee turned to walk away ee ran into a roof mat that was bent down on the end causing a laceration to right ear which required 7 stitches to close the wound.
EE's were putting inserts in the lugs on the miner, they were using a sledge hammer to drive the inserts in when an EE hit the lug the sledge hammer glanced off the lug and hit this EE in the mouth. This accident chipped 1 tooth and knocked the cap off of another tooth.
A roof fall above anchorage was found by the Mine Foreman in the 7 Seam A Entry return @ 36+75. This fall was approximately 60'x 17'x6.5. There was no injuries with this accident. The travelway was blocked but did not effect ventilation. The area was dangered off at all approaches and travelway was re-routed around the fall. This area was mine 9-1-2014.
EE was riding on a 2 man rubber tired Johnson and ran over some gob in the roadway causing ee to jam head into a roof strap. EE felt pain in neck and shoulders and into both hands.
The 480 volt main feed power line going to the elevator had a phase burn into causing the elevator to stop. There was 1 miner on the elevator at the time of this accident. The line was repaired and the power was put back on at 12:00 pm 10/09/2017 and the miner was brought outside. There were no injuries with this accident. Schindler Elevator Company was called and on sight.
EE was walking down B Entry bent over going to scoop charging station when EE hit EE's head on a low brow, EE felt a pop in the left side of EE's neck with pain and had difficulty turning EE's head left and right.
EE was going down a set of steps holding on to the handrail when EE's helper threw a come-a- long over the handrail, the come-a-long had a broken cable in it which stuck out 3', the cable struck EE on the top of the hand causing a laceration to the top of EE's left hand requiring 7 stitches to close the wound.
A roof fall above anchorage occurred in the 003-0 7 South B-C entries at spad #44+05. This fall interrupted ventilation by knocking part of a return wall out. A new wall was built and the area was rebolted and floor to roof support was installed. This area was mined in April of 2017 and was due to normal weathering. There were no injuries with the accident.
EE was tramming the roof bolter from the side controls when stone fell from the rib line and the bolt knocking EE to the ground, then more stone fell and struck EE on the right leg and ankle causing a fracture to leg and a laceration to ankle requiring 2 stitches to close the wound.
Employee was carrying a box of resin glue to put on the roof bolter, when employee twisted to put the glue in the glue tray employee felt pain in employee's lower back and pain going down employee's left leg.
This employee was dragging a shuttle car cable around the block when EE caught EE's foot on a stone and started to fall. When EE tried to catch self EE jammed EE's leg into the ground, which jammed EE's leg into EE's hip area. EE also states that EE landed on EE's Proximity box that was on EE's belt. This fall caused 2 fractures to EE's left hip.
A fall above the anchorage was found by the weekly examiner in the 4 South intake escapeway blocking the travelway at spad location 4+67. This fall was due to normal weathering of the area as the roof bolts broke off. The area was dangered off and the escapeway was rerouted. This area was mined in 12/07 with fully grouted 6' bolts.The fall is 50'Lx6'Hx18W.
A roof fall above anchorage was found by the weekly examiner. This fall was due to normal weathering. The fall was 18x60x6 and blocked the travelway of the examiner. There were no injuries with this accident. The ventilation was not interrupted and the area has been dangered off at all approaches. This area was mined in Nov. 2014.
This area is part of the 7 day exam found by the examiner on their normal run. A roof fall was found which was 18x20x6. Travel way was blocked due to the fall, ventilation was not affected. The fall is due to normal weathering. This area was mined in March 2006. There were no injuries with this accident. This fall was located in the West B right side return at spad 37+64 H entry.
While just starting to mine in the F entry of 7 South 003-0, the miner cut into an uncharted oil or gas well. This well was uncased. There were no injuries with this accident.
