Employee was using a cutting torch working on a spare dozer blade when a piece of hot slag fell into the top of his boot causing a burn to his ankle.

On 01/18/10 EE's wife reported that the EE had injured himself while operating his dozer on 01/14/10 and not reported the injury. Now on 01/18/10 he was in pain and needed to go to the Dr. This accident is under investigation because it was not reported before leaving mine property and pending results of the investigation, this accident report may be requested to be withdrawn.

Employee stated the dozer he was operting got on a rock and slid sideways when the tracks got off the rock it came to a sudden stop causing a jar to his shoulder. He continued to work until 4/16/09 when he went to dr and was referred to farily dr for an MRI. He was placed off work on 4/21/09.

THE EMPLOYEE WAS USING A SHROTER DRILL TO DRILL ANCHOR HOLES IN THE MINE FLOOR, WHEN AN OIL SEAL BLEW ON THE DRILL, SPRAYING OIL INTO HIS EYES.

Employee stated that he was grading the haul road, when the blade of the Grader struck a tree that was imbeded in the berm of the road way. This caused the grader to stall, resulting in a sudden stop, which caused pain to his lower back. EE refused medical treatment, but went to his family doctor after work and was placed off work.

EE was operating a 1500 Komatsu Haul truck being loaded by an O&K 340 shovel and stated he received a jar from a rock that was being loaded. EE continued to work until 01/16/09 when he went to Dr. and was placed off work with a strain to the back.

Employee was in the process of removing a steering jack from a DBT Ram Car when the jack slipped catching his middle finger on his left hand between the jack and the frame of the car.

Employee was using a grinder on a grader blade. Grinder slipped striking him in knee causing a small laceration which required stitches. He returned to work on same shift.

The roof bolter operator was preparing to drill a hole to install a roof bolt. As he started to rotate his drill steel his glove got caught on the drill steel causing his left hand to be wrapped around the steel resulting in a laceration to the little finger and ring finger.

Employee (roof bolter operator) was preparing to drill a hole in the roof for a roof bolt when his foot slipped under the drill boom while he was lowering the boom, catching his foot. The area was dry with a small amount of loose coal.

Employee was using a air grinder on a bolt ont he pivot shaft of a d-10 caterpillar dozer. The grinder disc caught his glove and jerked his hand into the disc resulting in a laceration to the top of his hand. 5 stitches were required return to work the same day.

Drill operator stated he was tramming his drill on a drill bench when he crossed a hump and his drill rocked forward. The forward motion of the machine was stated to have caused pain in his back.

EE was operating his dozer on layers of rock when one of the upper layers broke causing his dozer to drop sudden resulting in a neck and back injury. This injury occurred on 12/2/06 and he started missing work on 12/20/06.

EMPLOYEE WAS DISCONNECTING A DISCHARGE HOSE FROM A 6" WATER PUMP. THE PUMP HAD BEEN RUNNING BUT NOT PUMPING & HAD BECOME VERY HOT, THE HOSE WAS FULL OF STEAM. WHEN THE LINE CAME LOOSE IT HIT THE EMPLOYEE CAUSING BURNS TO HIS RT. ARM & LEG.
