Showing all 4Miner working next to a cable winch mechanism. The winch gear mechanism released causing the winch handle to spin. The rotating winch handle struck the miner on the left forearm causing swelling and bruising.
Miner was using pneumatic tools while performing post shift equipment maintenance. Dirt/debris was blown into miners right eye. Eye wash solution was used to flush the debris out of the eye. Eye was still irritated the following day which prompted the miner to see a doctor. An abrasion on the eyeball was diagnosed.
When setting up cone crusher, the lock pin of the chassis support leg came out causing the support leg to drop (extend) and hit the employee's right lower arm. A contusion of the right lower arm resulted.
