Miner slipped stepping off excavator. Injured left knee. Failure to file 7000-1 discovered during 01/19/2010 inspection. Disclosed to Wyoming inspector on 01/20/2010. Citation has been issued.

When setting up cone crusher, the lock pin of the chassis support leg came out causing the support leg to drop (extend) and hit the employee's right lower arm. A contusion of the right lower arm resulted.
