Caught in, under or between a moving and a stationary object

large rock got stuck in the jaw of the crusher. A TM attempted to use a chain to wedge between the rock and the jaw dies to break it up. As the rock and jaw pulled the chain down the TM tried to pull the chain up, resulting in smashing EE's hand between the chain and the railing causing laceration to left palm and a fracture to the 5th metacarpal

WHILE EXITING SKID STEER, EMPLOYEE FELT POP IN STOMACH.

Over-exertion in lifting objects

employee lifted the motor up to put it down where we could use the Crain to hoist it the rest of the way out. while lifting the motor employee felt something tear in the abdominal area. incident occurred 8/26/24 - it was reported 8/28/24 and found out EE was going to receive surgery 9/20/24 then surgery was moved up to 10/7/24

Employee was moving a conveyor belt onto a stand, upon moving the belt EE's fingers were caught in between the conveyor belt and the screen guarding. the screen went through EE's glove and cause a laceration to the right index finger. resulting in three stitches. employee was released for work with no lost time.

Miner was coming down stairs of button house and felt pop in left knee area. Muscles around knee area are inflamed and weak.

Miner tripped and fell, hitting back of head, causing laceration on back of head that required 7 staples

Muffler stack broke, striking and cutting right thumb. 5 stitches on right thumb.

Sampling aggregate from flowing stream, was holding a 5 gallon bucket with both hands, sweeping the the bucket through the aggregate stream from side to side. Noticed pain in lower back. Pain gradually increased.

Struck by flying object

Miner had a sliver of metal removed from right eye. At first EE thought it was an arc burn, but when it didn't get better EE went to eye doctor and had metal removed. No lost time.

Flash burns (welding)

Employee was assisting welder, holding a piece of metal for welder to weld together and received arc burn to eyes.

Employee was cutting a piece of pipe when another employee driving a skidster hit the other end of the pipe knocking the pipe into the knee of the employee cutting.

Employee carrying drill hammer to drill. Slipped from hands and broke leg.

Struck against stationary object

Miner using a chain with a loader bucket tooth attached to the end to free a large rock that was stuck in the jaw crusher. Rock was lifted but when rock settled back down it pulled the chain downward. The end of the chain the miner was holding cut the miners right hand which required stitches.

Miner installing belly pan underneath Cat D-9 dozer. Chain being used to hold pan in position slipped pinching miner' s hand between pan and dozer undercarriage. Left little finger was fractured.

Struck against a moving object

The articulated haul truck the miner was operating lost power traveling up a ramp. The truck rolled backward and was stopped by a topsoil pile. The abrupt stop jarred the miner's right arm causing a contusion. The contusion caused be a possible muscle tear or vessel tear.

Miner was holding metal punch used to knock out a pin in a dozer swing frame. Hammer used to drive in the pin hit miner's hand. Laceration to miner's right middle finger. Required stitches.

Struck by falling object

Miner was attaching bucket on skid steer loader. While bucket was raised, miner was hammering in pivot pins. His foot was below the raised bucket while hammering. The bucket slipped out of the attachment point and fell on his left foot. A contusion of the left foot occurred.

Fall down stairs

Injury occurred 01/20/2012. Miner slipped going down button house stairs. Was in a hurry. Fractured right thumb. Miner did not inform supervisor. Miner obtained medical care on 01/28/2012. Safety Director notified on 02/01/2012. No lost time has resulted.

Struck against stationary object

Miner was walking on a variety of ground surface conditions and on different equipment. At some point during his shift, the miner stepped on a rock, equipment surface or the like and broke a bone in his left foot.

Fall to the walkway or working surface

Miner slipped on ice while walking outside at gravel pit. Fell backward and landed on shoulders/back. Strain to neck and torso muscles.

Over-exertion in lifting objects

Miner notified company Safety Director on 08/03/2010 of injury to his shoulder that occurred on 07/20/2010. Miner injured shoulder while lifting electrical cables during crusher set up. Has been working since injury date. Miner now feels he needs to see physician. Safety Director will investigate validity of injury claim.

Struck by flying object

Miner performing maintenance on equipment. Dust/dirt blown into eye by wind. Miner was wearing eye protection. Dust/dirt embedded in eye.

Struck against stationary object

Miner was shoveling under belt. Shovel contacted belt and whipped back across left knee. Bruised knee resulted. Miner submitted injury report to safety office on 8/15/2008. This 7000-1 is a resubmission of a report form submitted 8/15/2008.

Caught in, under or between two or more moving objects

Miner working next to a cable winch mechanism. The winch gear mechanism released causing the winch handle to spin. The rotating winch handle struck the miner on the left forearm causing swelling and bruising.

Miner was shoveling material out from underneath a belt. The miner twisted his left knee in the course of his shoveling motion. Injury did not appear serious at the time. The miner went to a doctor two day later due to swelling.

EE WORKING TO HELP SET UP JAW CRUSHER, POSITIONING LEGS WHEN LEG SWUNG FREE & SMASHED HIS HAND. RIGHT HAND LACERATION BETWEEN THUMB AND FIRST FINGER REQUIRING MINOR SURGERY TO REPLACE DEAD SK IN AND THERAPY.
