Employee inadvertently failed to remove lifting strap from truck crane to excavator bucket. When excavator backed away it raised front wheels of truck with mechanic inside truck. Strap then broke and truck came back down causing employee to strain back.

Employee was cutting off a roller with a torch. When roller got cut through it accidentally fell on employees right hand and lacerated hand and fractured his index finger.
