Employee was in the process of dismantling a gear drive when the steel pivot base frame on which it was setting collapsed onto EE's chest.

Employee was was working on a final drive (mobile equipment). Employee struck the housing to loosen it. A shard of metal broke off striking employee's right shin.
Employee was tightening a bolt on Crusher 2 (CR2) with a torque wrench and torque multiplier. After tightening bolt, the torque multiplier came loose and dropped. Employee reacted by attempting to catch the multiplier. The multiplier jammed employee's left hand.
The employee attempted to break up frozen clumps of sand in a hopper with their hand and while doing so, their hand twisted in an awkward position resulting in a wrist sprain.
Lifting floor grate and felt a pull in their back.
While doing some service work to a skid steer, employee was reaching into tight area of the machine to remove a hose and EE felt a pop in the front side of their right shoulder.
Employee hit a bump while driving a haul truck. As a result, the employee was jolted (was wearing seat belt) and felt pain in upper back.
EE was cleaning up the work area and went to move some jugs of fluid, approximately 2.5 gallons, when EE twisted and tweaked their back.
Employee was attempting to move a wheelbarrow, when they felt a pull in their back. EE received medical attention at a panel physician the day of the incident and was cleared to return to work with no restrictions but on follow up apt on 7/31/23, EE was put out of work until 8/2/23 when EE had their next follow up apt and was allowed to return to work but with restrictions.
Employee was finishing service job when closing engine bay door on excavator and pinched finger. Resulted in laceration to finger.
Moving a 4'x4' metal plate of off truck bed and plate slid, catching EE's finger between plate and weld equipment on truck. Fractured tip of left 1st finger.
Employee (mechanic) was changing cutting edge on 982M loader. Edge was on the forks of telehandler and employee was prying up edge with a bar to get the right side up. When the edge started to go over employee reached out to grab the edge to keep it from falling and felt a pull in EE's back. Reported immediately, but did not seek treatment until next day 3/21/23.
Bucket truck parked in front of connex, lifted bucket and traveled forward, coming in contact with connex, moving it 6". Another employee was inside of the connex hanging up a hose at the time of contact. Injured says was "thrown around", but did not fall, hit head or come in contact with anything. Next day reported rib and back pain; taken out of work until follow up appt.
Employee was walking catwalk on Primary 2 access the ladder at the bottom of the conveyor. As EE crossed a section of catwalk under head pulley of the return belt, EE stepped on a weak section of the catwalk. The weak section of catwalk gave way causing EE's right foot to break through the catwalk. Sprained right ankle; working restricted duty.
Employee was doing a plant start up at the wet plant and was shoveling wet material out from manual drop magnet/bucket elevator and felt a pull/sharp pain in lower back.
Employee notified mgr on 10/26 that EE needed to see a dr because EE's left hand has been bothering EE for a month. Employee was given anti-inflam and muscle relaxer scripts, light duty of limit use of left hand and to PT. The action of shifting from forward to reverse in loader is aggravating, so employee has been temporarily transferred to haul truck, which PT has agreed with.
Employee got finger pinched between aerial lift basket and a wall fracturing the left middle finger tip.
While driving haul truck, employee felt pain in lower back.

The employee lost grip of tail pulley and struck their left big toe. The employee suffered a fractured left big toe.
Employee got finger pinched in between two guardrails, while re-installing a catwalk.

Employee was pulling a piece of steel out with a come along to divert a material when the steel plate tilted and caught employee's left hand between steel and angle iron. Employee fractured left metacarpal shaft in hand.
Employee was removing a 3 by 5 foot piece of steel plate from an old discharge chute. While moving the plate out of the chute, the employee caught EE's right middle finger between the plate and a structural support beam. Employee fractured tip of finger.
Crew was working to remove an old screen. 2 crew members were on a platform below the screen removing old hydraulic lines & storing them on the platform. While trying to roll the hydraulic lines on the platform, one of the connection ends of a line was stuck on a wall. While pulling the line, the connection end came off the wall striking one of the employees in the nose/face.
While employee was getting into the Pick Up, they didn't realize the seat was moved forward, it caused the employee to be move forward in the truck and employee jammed their neck against the roof of the pick up causing a sudden pain in the left side of the neck.
EE was operating a Caterpillar 992C loader in the South Pit. As they were driving the machine, they reached up with left hand to adjust the airflow coming out of an overhead air conditioning vent. As they reached up, their hand hit the adjacent vent and cut the middle finger resulting in sutures to repair.
Employee inadvertently failed to remove lifting strap from truck crane to excavator bucket. When excavator backed away it raised front wheels of truck with mechanic inside truck. Strap then broke and truck came back down causing employee to strain back.
Employee assisted another employee lifting a drain pan after emptying diesel fuel into pan. When EE lifted pan EE felt a pull in back. Diagnosed with a muscle strain in the back.
An employee was descending the cab access ladder of a haul truck. The light illuminating the ladder timed out and went dark. The employee continued to descend the ladder in the darkness. The employee thought ee was on the bottom rung of the ladder and stepped down, causing ee to fall and strike knee on the bottom rung of the ladder
Employee was cutting a piece of rib rubber for the MGL screen when ee inadvertently lacerated left forearm about 3 inches above wrist with utility knife. Employee received 6 internal stitches and 3 external stitches. Released back to full duty with no restrictions on same day as injury.
Employee was cleaning the tracks of excavator off with a shovel and felt a pop in left shoulder.
Employee was operating a 980G loading a customer truck. While the loader was still in reverse backing away from the truck. Operator depressed the brake / clutch and the loader shifted from reverse to neutral to forward inadvertently. Sudden shift caused pain in lower back.
EMPLOYEE WAS RETRIEVING AN ACETYLENE BOTTLE FROM CAGE WHEN THE BOTTLE SLIPPED AND PINNED EMPLOYEE'S FINGER BETWEEN THE BOTTLE AND CAGE. CONTUSION AND LACERATION TO RIGHT INDEX FINGER.
