Employee was running dryer. Needed to grease bearing. Didn't shut down dryer. Went under guard & tried to grease while dryer was running. Fore arm was caught on something and EE's skin was removed.

Showing all 5Employee climbed on excavator and operated it for approx 3 hrs & when EE climbed off EE felt pain in lower center to right of back. EE thought it was just stiff from operation of excavator but has gotten worse over the last 3 days.
AFTER CUTTING A PIECE OF POLY-PIPE, EE STEPPED ON THE OTHER SIDE OF THE PIPE TO DETERMINE WHY THE PIPE HADN'T RELEASED. JUST AFTER HE WAS ON THE OTHER SIDE, THE TENSION RELEASED AND THE PIPE HIT EE IN THE LEFT LEG.
EMPLOYEE WAS USING A TORCH TO SHRINK-WRAP A PALLET OF SAND BAGS. HE HELD THE TORCH TOO CLOSE FOR TOO LONG AND THE FILM CAUGHT FIRE. HE USED HIS HAND TO PUT THE FIRE OUT, BURNING HIS HAND.
EMPLOYEE WAS GUIDING AUTOMATIC SWEEPER FELT PAIN IN LEFT SHOULDER.
