employee was repairing a shaft. While tightening the shaft, the employee's hand slipped out of the wrench, and the employee hit EE's left eye

Struck by flying object

Employee was carrying out routine maintenance on the dryer drum drive belts. While changing the 2nd belt, the belt backlashed & pulled right hand causing a cut on pinky & ring finger

Employee walked out of break room. EE stumbled for unknown reasons and fell. A few seconds later EE started having convulsions. This is when EE struck head on the ground causing EE to bleed. EMS was called and available within 7minutes. The employee was taken to the hospital and received treatment.

Employee was cleaning up area around dryer. As they backed up the wheel barrow started twisting causing them to loose balance & roll their left ankle.

Employee was trying to clear a stuck flow hole in the bin knife gate, when it released and caught EE's hand.

Employee was stacking 50 lb bags onto a pallet. Employee over extended while stacking a bag and twisted/strained back

Struck against a moving object

Operator moving 2 pallets of material stacked on top of each other, the material weight exceeded the forklifts moving capability. Operator raised the load too high from the ground and when the forklift was moved forward the forks tilted forward lifting the forklift and once the pallets fell off the fork the lift landed back on the ground.

Employee was removing drain plug on oil pan on tractor. Felt shoulder pop when he pulled on wrench.

This was not an accident/injury. The employee collapsed while leaning on bagging machine table. Fellow employee found ee on the floor and called 911 and administered CPR. Employee later died at the hospital (see Death Certificate #9 - place of death).

Fall from machine

EE was moving pneumatic tanker after loading and grabbed handle on side of truck to enter drivers cab, handle was wet with dew and hand slipped. EE fell hitting right inner thigh on truck step and left knee on ground.

EE WAS ON LIFT CAGE OF LULL FORKLIFT, REMOVING TRIM FROM BLDG. DAMAGED BY HURRICANES. FORKLIFT BACKED UP TO LOWER LIFT CAGE & WENT FROM CONCRETE TO SAND. WHEN THIS HAPPENED, THE LIFT CAGE SHIFTED & EE HAD HIS HAND AT THE BACK OF THE CAGE, OVER THE HANDRAIL. WHEN LIFT CAGE SHIFTED IT PINCHED THE END OF EE'S FINGER BETWEEN CAGE HANDRAIL & FORKLIFT PALLET GUARD.

EE WAS CHANGING A SHEAVE. HE PLACED THE SHEAVE ON TOP OF A PUMP FACE AND THE SHEAVE SLIPPED AND SMASHED HIS FINGER. FINGER WAS LACERATED.

EE WAS ASSISTING IN CHANGING A CABLE ON THE DREDGE FROM THE WORKBOAT. EE STEPPED OVER THE SEAT, SLIPPED AND SCRAPED RT. LEG - SHIN.

AFTER CUTTING A PIECE OF POLY-PIPE, EE STEPPED ON THE OTHER SIDE OF THE PIPE TO DETERMINE WHY THE PIPE HADN'T RELEASED. JUST AFTER HE WAS ON THE OTHER SIDE, THE TENSION RELEASED AND THE PIPE HIT EE IN THE LEFT LEG.

EE WAS TAKING 0PROPANE TANK OUT OF RACK AND PLACING ON FLOOR, DOR TO RACK CAME BACK AND CAUGHT HAND BETWEEN TANK & DOOR CUTTING HAND.

EMPLOYEE WAS USING A TORCH TO SHRINK-WRAP A PALLET OF SAND BAGS. HE HELD THE TORCH TOO CLOSE FOR TOO LONG AND THE FILM CAUGHT FIRE. HE USED HIS HAND TO PUT THE FIRE OUT, BURNING HIS HAND.
