Employee was working to get EE's hammer from the welding truck. While holding the tool box open the hammer slipped from EE's other hand and hit EE's left middle finger.

Employee was inside the screen box replacing the shaft for the shaker, all equipment was locked out and tagged out. While putting the new shaft inside the shaker, Other ee assisting him could not hold the shaft and it fell approx. 6 to 8 inches and struck him on the left side of his head. It put a cut of about 2 inches on the left side of his head. He did not lose consciousness
