EE and another employee were replacing cover grates in the CC-404. They were using grinders with cut off wheels. The 2 employee's were working close together, and the EE's right forearm was struck by the other employee's grinder wheel while it was still spinning, causing a cut on EE's forearm.

Showing all 42EE was inspecting a chute and gate area. EE entered the gate area, and the gate cycled. EE was pinned between the gate and the chute. EE sustained some crushing injuries. The employee sustained a fractured rib.
Incident on 16SEPT2022 was originally a first aid only/no medical case after a 35lb. jack hammer went thru some loose material pulling down on EE's r-shoulder. On 19OCT2022 EE requested medical evaluation because EE still had pain. EE was medically evaluated and released to work the same day (19OCT2022) with restrictions awaiting follow up appointment.
EE reported to supervisor on 08APR16 that EE had been welding on 07APR16 and EE was o.k. at the end of EE's shift but by bedtime EE's eyes began to bother EE. EE was taken to the eye Dr. where EE was diagnosed with Superficial Keratitis and foreign material was removed from EE's left eye. EE was issued a prescription to prevent infection and returned to work without restrictions.
He was loosening a bolt with another employee when the bolt fell out of the impact wrench and hit him in the mouth.
The JTT miner was doing demolition. When chipping, the miner lost his grip on the chipping gun, causing him to lose his balance. Stepping forward to regain his footing, the miner bumped his right knee on a scaffold pipe.
EE was drilling a hole in a piece of steel and the drill bit stuck and the drill twisted out of his hands. He was diagnosed with a metacarpal bone fracture in his right hand and returned to work with restrictions.
EE was rolling a piece of metal and his glove got caught between the metal and roller and his finger was pulled in. EE was diagnosed with a fracture to the right index finger and released to work with restrictions.
Hand grinder slipped from control, twisted and hit the right hand causing a contusion.
While splicing a hose, his hand was sucked into the hose, up to his shoulder.
Welding nozzle on a pipe, a joint weld broke and pipe fell off the stands. Pipe swung into his leg and he received a laceration. He received 4 stitches.
He was cleaning the DC Pipes with a jackhammer which created excessive dust particles which entered his eye.
As he started the grinder the guard fell of and hit him on his left thigh.
He was jackhammering out BC-203 transfer chute from the outside. The jackhammer fell back and smashed his finger between the chute and the jackhammer.
He cut his thumb using the lunch room can opener.
While jackhammering above his head to loosen build-up in BM-302 feed bin. He lowered the jackhammer back to grating and felt a pop in his wrist.
WHILE PALLETIZING, A BROKEN BAG CAME DOWN ON THE PALLETIZER AIR FLOAT BLOWING CEMENT DUST UP INTO THE EMPLOYEES EYES.
He was loading a cement bag onto the packer spout when suddenly it blew cement into his R- eye.
THE BAG SLID OFF FROM SPOUT #3 BLOWING AN EXCESSIVE AMOUNT OF CEMENT TO THE MID SECTION OF HIS BODY AND THEN DIRECTLY INTO HIS FACE AND EYES.
Employee was running a paletizer to stack cement bags on pallet; ee opened slide door to drop bags on to pallet, when slide door started to close it caught the left arm and wrist between a cement bag and slide door.
GRINDING STEEL, HE LIFTED HIS WELDING HOOD & FELT SOMETHING IN HIS RT. EYE.
WHILE DOING SOME GRINDING ON BOTTOM TRACTION OF BE-501 GOT A PIECE OF METAL IN EYE.
GRINDING ANGLE IRON USING 4 1/2" HARD GRINDER. GRINDER SLIPPED CUT HIS LIP.
EE WAS USING AN IMPACT GUN TO TIGHTEN BOLTS ON A SECONDARY CRUSHER - FELT A STRAIN TO HIS SHOULDER. WENT TO DOCTOR ON 7/22/02. SURGERY ON 9/18.
EE WAS WORKING AT FINISH MILL CUTTING HAND RAIL. AS HE WAS FINISHING THE CUT HE GRABBED PIECE WITH LEFT HAND & BAND SAW WENT THROUGH & CUT LEFT THUMB. 7 SUTURES WERE REQUIRED TO CLOSE LACERATI ON.
ON 4-23-02 EE COMPLAINED OF NECK AND SHOULDER PAIN WHICH STARTED ON 4-16-02, HE THOUGHT IT WOULD GO AWAY, BUT IT GOT WORSE. SENT HIM TO DOC ON 4-23-02. EE WAS SACKING AT THE PACK HOUSE.
EE WAS STANDING ON A PLATFORM BY W-345 IN A AWKWARD POSITION DUE TO CONFINED AREA (SUPPORT SYSTEM) WHILE EE WAS AIR LANCING THE CHUTE HE WRENCHED HIS BACK.
LABORER WAS DIGGING A SMALL EXCAVATION (~30" WIDE X 36" LONG X 14-21"DEEP) USING PICK AND SHOVEL HE HIT A LARGE ROCK AND USED A JACKHAMMER TO BREAK THE ROCK. WHILE USING THE JACKHAMMER AN UNDE RGROUND ELECTRICAL CONDUIT WAS PENETRATED CAUSING A FLASH FIRE.
WHILE EE WAS JACKHAMMERING A TRANSFER CHUTE, HE LIFTED THE JACKHAMMER AND THOUGHT HE PULLED A MUSCLE INHIS SHOULDER.
RECIEVED A SMALL CUT ON LEFT KNEE REQUIRED A FEW STITCHES, AND THEN RETURNED TO WORK THE SAME DAY.
ON 6-27-01, EE WAS TRYING TO REMOVE A STUCK BAG IN PALLETIZER. SUPERVISOR CAMY BY AND TURNED MACHINE ON WHICH CAUSED EE'S ARM TO GET PINCHED BETWEEN BAG AND FRAME. ON 10-16-2001, EE COMPLAINED THAT HIS SHOULDER WAS BOTHERING HIM--AT TIME OF ACCIDENT HE SAID HE WAS O.K.
WHILE CLEANING OUT CHUTE BELOW 103 SCREEN AT THESECONDARY, THE LOWER END OF THE CHUTE FULL OF MATERIAL, IT BROKE LOOSE & PINNED EMPLOYEE'S LEG AGAINST THE SIDE OF THE CHUTE. ALSO, HE HAD HOLD WHEN THE JACKHAMMER BROKE FREE. WHEN THIS HAPPENED HE FELT A STRAIN IN HIS RIGHT ELBOW.
INJ WORKER & HELPER WERE REMOVING SLING FROM IBEAM PLACED ON DUNNAGE. THE CRANE OPERATOR MISSED THE HAND SIGNAL & BOOMED UP INSTEAD OF CABLING UP-THE SHADDE WAS NOT REMOVED FROM THE SLING PRI OR TO USING THE CRANE TO PULL THRU THE SLING FROM UNDER THE LOAD. THE BEAM FELL OVER & STRUCK THE HELPER ON THE RT TIBIA.
EE NOTICED A SORENESS IN NECK AND BACK AFTER JACKHAMMERING THE DAY BEFORE SO HE FILLED OUT AN ACCIDENT REPORT. ON 2/20/01 HE TOLD HIS SUPERVISOR THE PAIN WAS STILL BOTHERING HIM AND REQUESTED TO SEE THE DOCTOR.
EE HAD HAD AN ONGOING REPETITIVE MOTION INJURY IN HIS LEFT SHOULDER. AT THE END OF THE DAY ON 1-4-01 HE ASKED ME IF HE COULD SEE A DOC. IT WAS REALLY BOTHERING HIM. HE WORKS IN THE PAK HOUSE P ALLET RING AND SACKING.
EMPLOYEE GOT METAL IN RT MIDDLE FINGER. TRIED TO REMOVE IT, TURNED OUT TO INFECTED. REPORTED TO SAFETY COORDINATOR ON 11-13-00. SENT HIM TO DR. INFECTED-REMOVED FINGERNAIL-ANTIBIOTICS AND PAIN KILLER PRESCRIBED.
WHILE UNPLUGGING BUELLS ON #3 KILN. PUFF OF HOT AIR BLEW OUT OF PORT. TURNED HEAD TO AVOID AIR.
WHILE LOADING KILN GUN A PIN CAME OUT OF ELEVATION ADJUSTER CAUSING GUN TO FALL DOWN WHICH CAUSED FINGER TO GET HIM BY THE ADJUSTING WHEEL.
EMPLOYEE PREPARED AREA ACCORDING TO PROCEDURE HOWEVER HE DID NOT CONSIDER MATERIAL (COAL) DUMPING FROM ABOVE. HE WAS IN A CONFINED SPACE REPLACING A LINER WHEN MATERIAL CAME DOWN ON HIM. AS HE WAS EXITING THE AREA HE DROPPED THE WELDER CAUSING A SPARK WHICH CAUSED A SMALL FLASH FIRE.
EMPLOYEE WAS CLEANING A CHUTE WITH A JACK HAMMER. MATERIAL BROKE FREE AND THAT CAUSED THE JACK HAMMER TO FALL AGAINST HIS FINGER SMASHING IT. HE WAS WEARING GLOVES AT THE TIME OF THE ACCIDEN T.
EMPLOYEE WAS INSIDE CLINKER COOLER REMOVING GRATINGS; AIR FLOW CAUSED DIRT TO DRIFT UPWARDS INTO HIS EYE.
DOZER WORK IN HARD ROCK OVER THREE DAYS CAUSED EMPLOYEE DISCOMFORT WHILE TURNING HIS HEAD.
