While the EE was removing the attaching hardware for a cutting edge on a loader he positioned the bolt with his left hand while the impact wrench was still on the nut. He accidentally hit the trigger on the air tool which caused the nut to spin, lacerating his finger. He was evaluated, received seven stitches and was released with restrictions.

Employee states he was welding a bar on the primary crusher when a piece of metal went into his right eye. Employee was wearing a welding shield but was not wearing eye protection.

Employee states he was using a grinder on a tailgate. The vibration of the grinder caused the chute door to slide down and impact head.

Crusher laborer was operating an air chisel when the bit broke. As he attempted to remove the broken bit, the air chisel was accidentally triggered and the bit flew out of the shaft and struck employee in the center of his forehead. He sustained a small laceration which required stitches.

Employee was using a propane torch and sustained 2nd degree burn when he turned it on.

Employee was installing the cutting edge on the loader when the bolt he was tightening with an impact gun spun and cut his left middle finger.

EMPLOYEE WAS TORCH CUTTING AT BOTTOM OF ELEVATOR WHEN SPARKS BLEW BACK AND FELL ON HIS SHIRT CAUSING A BURN TO HIS NECK.

EMPLOYEE WAS ASSISTING ANOTHER PERSON ON THE CRUSHER TRYING TO DISLODGE A STUCK STONE. WHILE HOLDING A METAL BAR TRYING TO FREE THE STONE, IT CAUGHT IN THE MOVING PARTS OF THE EQUIPMENT. THE M ETAL BAR WAS PROJECTED OUT OF THE CRUSHER AND STRUCK THE EMPLOYEE ON THE LEFT SIDE OF HIS HEAD ALSO CAUSING INJURY TO HIS HAND.

WHILE EE WAS GRINDING OUT CRACKS IN BUCKET LOADER THE GRINDER KICKED BACK & STRUCK HIS LEFT KNEE
