While the EE was removing the attaching hardware for a cutting edge on a loader he positioned the bolt with his left hand while the impact wrench was still on the nut. He accidentally hit the trigger on the air tool which caused the nut to spin, lacerating his finger. He was evaluated, received seven stitches and was released with restrictions.

Struck against stationary object

Employee was washing loader when he rolled his ankle on a hose. Did not report the injury to employer until 5/14/12.

Employee sustained a lower back strain as he was attempting to lift and slide an oxygen tank into back of the yard truck.

Struck by flying object

Crusher laborer was operating an air chisel when the bit broke. As he attempted to remove the broken bit, the air chisel was accidentally triggered and the bit flew out of the shaft and struck employee in the center of his forehead. He sustained a small laceration which required stitches.

Employee was using a rachet extension to drive wedges out of screen rails and a piece of steel chipped off and struck employee in the neck creating a puncture wound.

WHILE EMPLOYEE ATTEMPTED TO REMOVE AN OXYGEN TANK FOR HIS TORCH SET OUT OF THE BACK OF HIS PICK-UP TRUCK, HE FELT A PULL IN HIS BACK AND THEN THE PAIN SLOWLY GOT WORSE AS THE DAY WENT ON.

WHILE CHECKING ON A BLOWN HYDRAULIC LINE UNDERNEATH A LOADER, A DROP OF THE HOT OIL LANDED IN THE EMPLOYEE'S RIGHT EYE. THE HOT OIL BURNED HIS EYE BEFORE HE WAS ABLE TO START FLUSHING IT OUT.
