Grinder angle was incorrect, causing it to bind up on a narrow area on angle iron. Grinder bounced at employee causing lacerations on chin and neck.

EE was unplugging the 1651 DC, when material blew back striking EE in the left eye. EE was using air lance above the rotary valve slightly cracking the valve when fine material blew back striking EE's left eye. EE radioed for assistance to nearest eye wash. Following the initial flush EE was taken to the plumbed eye wash where additional material was removed.

EE was using an pneumatic air hammer to chip lime scale away from the truck side loading scale. The EE reached out over head while lying on the ground and over extended, causing an acute pain in the right shoulder and numbness in the right hand.

Employee was air lancing around a lime silo when lime came in contact with eyes. EE immediately flushed eyes for 10 minutes at an eye wash station, they used 2 bottles of saline solution. Eyes felt better however they wanted medical treatment. Employee was sent to urgent care this morning. EE received 3 prescriptions, one of which was tylenol with codeine.

Injured employee was in a boom lift using a cutting torch. The worker turned the torch off and draped it over the handrail. The acetylene valve knob bumped against the mid-rail of the basket opening the valve which allowed acetylene gas to penetrate the employee's pant leg. A spark from the grinder ignited the gas causing a flash fire and burn to the worker's left leg.

Operator and rigger were replacing steel covers using a loader and chain rigging. The rigger removed the chain from one side of cover and signaled the operator to back up. Rigger did not signal the operator to stop and as they backed up the other rigging pulled across the riggers right foot. Fracture of the 5th metatarsal. Fracture boot for 6 weeks.

Employees began to lift up a block of refractory with a hoist & choker. The load was not centered below the hoist & it began to swing. One of the employees was in the path of the block. EE attempted to get out of the way. The block struck a crossbeam and continued to swing into lower left leg. EE was diagnosed with a hair line fracture in left leg.

Right angle grinder was used to dress-up the edge of a steel plate roof repair when the wheel wedged between the plate and the supporting channel iron kicking back into the workers face resulting in ten stitches in cheek and six damaged teeth.

EE was operating the East Side Crusher. Several rocks had come to rest on the jaw of the Crusher. EE was attempting to remove them by hand when a rock had come loose from the feeder/shaker (crusher was not operating) and struck his right index finger.

Employee was lifting hydraulic wrench on to pier when left pinky finger was pinched between timber and wrench when resting tool. Finger was lacerate at tip requiring three sutures to close wound.

Multiple welders in same area; redness in left eye with moderate discomfort. Employee notified supervisor of discomfort at 9:30 AM 05/13/2009; received medication from doctor. Noticable relief within a couple hours, will return to doctor for check-up 05/15/2009; employee was released with no limitations or restrictions 05/13/2009

Injured was stacking empty bags behind the #3 spout packer when lime discharged from the #1 spout getting into left eye. Injured was wearing safety glasses. Immediately flushed eyes and taken for emergency care at Northeast Methodist Hospital, released after treatment with followup appointments to make sure no permanent injury to cornea as result of chemical burn incurred.

Employee was welding on the CL- 300 system when he received cornea abrasions due to welding burns involving both eyes! Contributing factors include failure to inspect tools and equipment prior to performing task assigned!

EE WAS UTILIZING A RIGHT ANGLE GRINDER TO REMOVE ROUGH EDGES IN PREP. OF WELDING TASK ON THE CL-520 SCREW IN THE QUARRY/MAINTENANCE SHOP. EE INDICATED THAT SAFETY GLASSES WERE BEING USED AT TH E TIME OF THIS ACCIDENT HOWEVER A FOREIGN BODY ENTERED THE LEFT EYE REQUIRING MEDICAL TREATMENT TO BE REMOVED.
