Employee was arc gouging bulkhead weld seam off Matrix pump sled. While cutting the last of the weld seam the wall shifted pinching a finger between the floor and head of the gouging torch. Employee received laceration to right second finger. Employee was transported to Nutrien site first aid office and then ER and returned to work on 08/22/25.

Mechanic (M1) was tasked with working with an automated welding machine. During welding activities M1 began experiencing pain/itching in both of their eyes. M1 was diagnosed with flash burns and received prescription medication for both eyes.
Operator struck a dead tree with bucket. Top of dead tree struck Excavator, bounced and broke top glass. Part of tree top entered cab but did not touch operator. Operator closed eyes and did not see what happened. Operator stated may have lost consciousness/doesn't remember. Two medical professional examined operator, no sign of head trauma or loss of consciousness.
Operator was digging into an Ore stockpile loading a screen plant. At approximately 2:26 a.m. the Ore pile peak shifted from East to West and slid down striking a 336 Excavator and a Screen Plant. During the slide the Ore entered the front glass of the Excavator covering the legs, waist and lower chest of the operator.
Employee was placing rope over a tooth on the bucket of an excavator. While employee was positioning rope, the excavator operator started to curl the excavator bucket. This movement caused two fingers on the employee's right hand to be caught between the bucket tooth and the rope.
While pushing out a pin in the boom dipper with a 200 ton hydraulic jack one of the spacers came out and struck the employee in the head.
Crane was being used to remove pump impeller, placing it in bed of truck. Rigging failed resulting in impeller falling into bed of truck. The impeller rolled off bed of truck and struck employee in right leg and foot. Employee transported to local medical facility.
Employee was using a jigsaw to cut siding on #11 conveyor in area. While cutting Employee was taken to first aid where the eye was flushed again. Employee was then taken to local eye care center where debris was removed and antibiotic eye drops prescribed. Employee was wearing required PPE at the time of incident.
Employee was adjusting the air flow on the green float bank B-Cell when the middle finger on his right hand got caught between a moving paddle wheel and the guide frame for the tee handle used to adjust the air.
Mashed little finger between impact wrench and steel plate on clean out tee.
Employee was placing body bolts with a port a power when the shim plate used to distribute the load pressure fell on his hand causing a fracture to his ring finger.
Employee was struck by a torch causing a 2nd degree burn to his left wrist.
The backhoe operator was pulling power cables with a backhoe. The employee was standing between junction box and backhoe. The cable became tight and hit him.
Employee was removing conveyor belt rollers on the prestripping system conveyor with the assistance of a backhoe. Employee placed his hand on the roller support bracket to remove it from the handler peg once the weight of the roller was supported. He signalled the backhoe operator to lift the roller. As the roller was lifted, the hanger bracket rotated and pinched his thumb.
Employee was removing steel grating in the Flotation Bldg. He rigged the hoist hook to the connecting rod of the grating and bumped the hoist up. Not knowing that the grating was not free, he bumped the hoist again, causing the connecting rod to break. The hoist hook released from the grating and struck him in the left hand, causing a cut and a hairline fracture.
Grinding bolt off of a flange. The grinder snagged and it was torqued out of his hand. Grinder fell and cut his side.
Assisting in changing out a section a tailing piping. He was using a 1" impact wrench to remove bolts from the flange. The impact wrench slipped off of the bolt head. This caused the employee to lose his balance and he used his left hand to catch himself. This resulted in a laceration of his left palm.
Welding on 2570 boom, noticed pain in right elbow.
Employee was welding and lifted his shield up. Hot slag "popped" and went over safety glasses and into left eye.
EMPLOYEE USED DOZER TO PUSH A 60' LENGTH OF RAILROAD RAIL OUT OF THE WORK AREA. HE POSITIONED THE RAIL PERPENDICULAR TO THE DOZER BLADE. THE RAIL STOPPED MOVING FORWARD AND THE DOZER CONTINUED TO TRACK. THE RAIL BOWED AND SPRANG UP OVER THE DOZER BLADE AND CAME DOWN THROUGH THE LEFT SIDE CAB DOOR GLASS AND ONTO OPERATOR'S LEFT KNEE.
THE METAL WAS CUT THROUGH WITH NOTHING HOLDING IT FROM THE EMPLOYEE. THIS PUT HIM IN THE LINE OF FIRE FOR THE FALLING METAL.
EE USED DOZER TO PUSH 60' LENGTH OF RAILROAD RAIL OUT OF THE WORK AREA. HE POSITIONED THE RAIL PERPENDICULAR TO THE DOZER BLADE. RAIL STOPPED MOVING FORWARD & DOZER CONTINUED TO TRACK. RAIL BOWED & SPRUNG UP OVER DOZER BLADE & CAME DOWN THROUGH LEFT SIDE OF CAB DOOR GLASS ONTO OPERATORS LEFT KNEE.
EMPLOYEE WAS HOLDING AN OBJECT WHILE THE WELDER WAS TACKING. EMPLOYEE HAD CUTTING SHIELD ON. HE WOKE UP AT 3:50 AM ON 12/16 COMPLAINING OF RED, TEARING EYES. HE FELT LIKE HE HAD ROCKS IN BOTH EYES.
DRILLING OUT A FITTING THAT WAS IN A DRILL PRESS VISE. DRILL BIT GRABBED, SPINNING VISE AROUND STRIKING EMPLOYEE'S RIGHT THUMB.
PUTTING IMPELLOR ON PUMP. EE WAS REPLACING SLEVE AND FINGERS GOT CAUGHT ON BACK KICK BETWEEN BELTS AND SHEAVE.
EE WAS WELDING BRACKETS. AFTER WELDING A BRACKET, EE LIFTED HIS WELDING HOOD. DICAL DUST ON HOD FELL DOWN BEHIND HIS SAFETY GLASSES INTO EYE.
EMPLOYEE WAS PUTTING PLUGS IN PIPE. APPROX 10-15 FEET AWAY OTHER EMPLOYEES BEGAN WELDING.
WENT INTO A TANK TO GET A GLOVE. WHILE IN THE TANK AN ELECTRICIAN STARTED BLADE (AGITATOR) TO CHECK ROTATION. STRUCK EE ON LEFT KNEE, SHOULDER, BACK ANKLES AND RIGHT UPPER LEG.
PULLING UP IMPACT ROLLER WITH A BOOM TRUCK. WENT TO PUT KEEPER IN PLACE. OPERATOR OF BOOM TRUCK LEFT ROLLER DOWN TOO SOON AND CAUGHT FINGER UNDER KEEPER. MASHED INDEX FINGER ON LEFT HAND.
WORKING IN BACKHOE AT PIT CAR SUMP WHEN WATER FROM PIT CAR CANNON HIT WINDSHIELD, SHATTERING GLASS. GLASS FLEW IN FACE.
