Employee was using an etching tool when a metal fragment entered the eye. The item was removed and medication for the eye was prescribed. They received a full release to work.

The employee was injured on the 22nd of May and reported the accident on the 24 of May. The employee was taken to the clinic where a piece of metal was removed from his eye.

During the process of pulling a core barrel out of a 60 degree angle hole, driller lowered the head and employee's left index finger was pinched between the head and rail on the drill mast resulting in an open fracture to the tip of employee's finger.

Employee was working in a tight area grinding bad concrete on a wall with a 4" grinder. It had "bucked" back at him several times and the lead man had reinstructed him to not apply so much pressure. When he put the grinder back on the surface he applied so much pressure it broke the blade and kicked back into his left lower leg causing a laceration.

Per the operator's report the dozer was backing up and ran over a rock causing the operator to be "jarred" This incident was not reported until March 4th , which is in violation of our policy. We have reasonable doubt that this injury occured on our property. We will be forwarding more information in order to have this incident receive MSHA's "Category 10" designation.

Employee was stepping over a support and grabbed the handrail that he was heated up for support. He was repairing handrail and had removed his glove to put hearing protection on to use a hammer to put handrail back in place

Employee was placing the dishcarge trunion bearing into place on Mill #6. He was trying to remove a piece of cribbing when the cribbing shifted pinching fingers on his right hand between the cribbing (wood) and the casing on the mill. Resulted in laceration and partial amputation of mid and ring fingers of right hand.
