Showing all 8Two employees were standing on the plant structure using weed burners, there was an employee standing on each side of the conveyor leading to the hopper. One of the employees was wearing ice cleats, which caused that employee to lose balance but not fall. That employee then lost control of the torch and it passed in front of the injured employee's face.
Employee was grinding loose packing material from a cone crusher and in the process of setting down EE's grinder it caught EE's pants and cut EE's leg as it hadn't come to a complete stop before this happened.
Employee was operating a D8 Dozer pushing up material, spun his tracks, this caused and buildup of material behind the tracks. As the employee backed up over this material the rear of the dozer slammed to the ground once it broke the center of the pile. The employee was twisted the right, looking back as this happen, jarring his back.
EMPLOYEE WAS USING A CUTTING TORCH & A SPARK BURNED HIS KNEE.
EMPLOYEE WAS WELDING IN THE OXFORD GARAGE & CAME TO WORK THE NEXT DAY & REPORTED THAT HE HAD AN INJURY TO HIS EYES CAUSED BY A LOOSE LENS IN THE WELDING SHIELD. HE WENT TO THE CLINIC AFTER WOR K ON 12/5/02. HE WAS GIVEN TWO PRESCRIPTIONS AND ADVISED TO DO NO MORE WELDING UNTIL NEXT RECHECK ON 12/6, AT WHICH TIME HE WAS DISCHARGED FROM CARE WITH NO RESTRICTIONS.
EMPLOYEE WAS DOING A LOT OF WELDING ON THIS DAY. HE WAS OVEREXPOSED TO WELD FLASHES AND WAS NOT WEARING HIS PERSONAL PROTECTIVE EQUIPMENT. HE DID NOT REPORT THIS INCIDENT TO HIS SUPERVISOR UNT IL 10/30/02 OF WHICH HE THEN WENT TO THE CLINIC AND WAS DIAGNOSED WITH KERATITIS. NO RESTRICTIONS WERE GIVEN AND HE RETURNED TO WORK DOING HIS REGULAR JOB.
GRINDING STEEL, FLECK OF STEEL GOT IN RIGHT EYE.
