Miner was dislodging accumulated material in a log washer with a ½" steel rod measuring approximately 8ft in length, which was inserted through an opening in the expanded metal cover of the feed head box on the log washer. When the material broke free the rod went further down the chute and caused the employees' left hand to strike the metal cover.

Employee was working on breaking bolts free on the waste pump to inspect inner components. When the second bolt and nut broke free it jarred employee forward causing a slight pop and pain in right shoulder. Employee was using an adjustable wrench for the job because employee did not have a big enough wrench/socket for the job.

Employee was exiting a motor grader, EE was facing the steps of the grader while holding onto both handles. As EE was completing the dismount to the ground, EE's right foot slipped on uneven surface, and EE twisted right ankle which caused EE to fall.

Employee was performing EE's pre-shift inspection on Loader 944K2 and went into the lube shed to get a few rags to clean the windows on the loader. As EE exited the building and was walking back to EE's loader, EE struck EE's right shin on a large set of wheel chocks that were stacked together in front of the building.

Mechanic was torqueing boom bolts on a crane to 2100ft lbs., the employee switched from torque wrench to a 24" swivel head ratchet to remove the pressure from the torque multiplier. As EE pushed down, the ratchet handle slipped out of EE's hand and struck EE in the face resulting in a fractured and lacerated nose.

Two employees were standing on the plant structure using weed burners, there was an employee standing on each side of the conveyor leading to the hopper. One of the employees was wearing ice cleats, which caused that employee to lose balance but not fall. That employee then lost control of the torch and it passed in front of the injured employee's face.

Swinging a 4# sledge hammer with both hands to free up a cut piece of metal from beam. Metal broke free and upon completion of swing the left hand middle finger contacted the metal beam, resulting in a fracture of the middle finger on the left hand. Employee is right hand dominant.

Employee was rinsing the mud off of loader and was walking backwards pulling the water hose towards the back of the machine when they tripped over a wheel hub assembly that was on the ground. They reached backwards to brace themselves for the fall, and in doing so their hand contacted the top of the hub assembly causing a cut to the palm of the left hand.

Employee was moving a roll of skirt board rubber that was on a pallet into the lube shed. EE squatted down and pulled the top roll to the edge of the bottom roll and pulled it half way off so it tilted to the ground. As EE was standing the roll up on it's side it slipped from EE's grip. As EE reached out to grab the roll EE felt a pull in lower left side back.

Employee arrived at work at 6:45 and was having trouble breathing, was cold, clammy, sweaty, disorientated, and was having pains in abdomen. Prior to starting work employee took 2 prescription nitrile pills and used inhaler to try to minimize the pain. Employee has pre-existing heart issues.

Employee was removing chutes from the twin screw area preparing for its removal and replacement. EE was moving a section of a chute that was cut off; it was suspended by a 1-1/2 ton chain hoist. As EE was lowering the chute EE grabbed the bottom of it with right hand to guide it in place. The chute swung left and pinched EE's right middle finger.

Employee was helping complete a belt splice and was removing a clamp from the opposite end of the belt section that was installed, when the belt clamp slipped and traveled over a deck roller and struck him in the chin.

Two employees were performing maintenance on a hazmag crusher and while flipping a blow bar, it fell off of the stud on the lifting mechanism towards ee who was standing on the belt facing the rotor; he was able to keep his hands on it as it fell, however at approx 600 lbs it did hit his left leg which pushed him back up against the back wall of the crusher.

Employee was attempting to remove a bearing housing from an AC screen when his left thumb got pinched between the lifting chain and the bearing housing. The edge of the housing caused a cut on the inside of his thumb requiring 14 stitches and a cut on the outside requiring 4 stitches.

Employee was operating a D8 Dozer pushing up material, spun his tracks, this caused and buildup of material behind the tracks. As the employee backed up over this material the rear of the dozer slammed to the ground once it broke the center of the pile. The employee was twisted the right, looking back as this happen, jarring his back.

Employee was pulling on electrical cable for the hot sand plant when he felt pain in his groin area. Employee did not report this incident until 11/4/2009 three weeks after injury and after going to his own doctor. Employee was taken to the clinic on 11/9/2009. Currently employee is on restricted duty on his regular job and will see surgeon in two weeks.

1/28/09 is date event was reported to supervisor. EE alleges that operating his loader & years of working caused his neck to hurt and bone spurs. EE has been treating with his personal doctor until now. He was scheduled for surgery by his physician on February 20, 2009.

Employee slipped off ladder while climbing to get on top of a compressor room to get fluorescent light bulbs. As he slipped, his right wrist snagged on garage door rail cutting approx 2" of skin back which required 20 stitches.

Employee came to work with a brace on right wrist- said he'd been to his doctor-told he has carpal tunnel. Our HR dept sent him to a company referred doctor on 9/20 at which time he was given Ibuprofen (800 mg) 30 tablets & put on work restrictions which make him unable to perform his regular duties as a loader oper. He is currently working as a plant oper.

Installing belt scraper on stacker head pulley. Pipe of scraper barely in mounting fork, went to slide collar into place in gear box, caused pipe to fall off fork. Trying to keep scraper from falling, grabbed pipe too close & the force of it falling pinched his hand between the pipe & mounting fork.

Employee was trying to "stand up" a rear tire from a 1987 Ford service truck, which was lying flat on the ground. Employee knelt down and took hold of tire, then attempted to stand tire up and as he did, he claims to have heard a "snap" in his back.

EE told supervisor he felt small lump in chest & went to see personal dr. Xray revealed 1 suture from prior surgery undone. EE released w/weight restriction. Company did not feel this was work related. 3/15 EE went to see different dr., because his was out of town. Dr would not allow EE to return to work w/o stress test. EE has not done this.

While in the break room, EE said he wasn't feeling well & wanted to go to the clinic. Discharge papers from Lapeer Regional Med Ctr state he was prescribed a benzodiazepine tranquilizer. On 10/11/06 he went to the POH Med Ctr. He is being referred to Dr. Franckowiak for blood pressure treatment & was advised to be off work until 10/13/06 for blood pressure treatment.

16 yard Rohr Dredge located in lake on N Hummer Lake Rd in Oxford flipped over (cause unknown at this time). Employee drowned. Employee's body was recovered at 5:35 pm. Incident is under investigation by MSHA inspector Steve Field.

EE was walking around fueling station to do some clean-up & twisted his right ankle. He said no specific cause & the ground was level. Our Safety Director spoke with a Coder at MSHA Office of Injury & Employment Information on 4/6/06 about incident & was told it would not be reportable. Site was inspected on 4/25/06 & MSHA Inspector told Plant Manager that 7000-1 must be completed.

Employee picked up a 1-1/2" water hose with a spray nozzle attached to it and began pulling it up some stairs. As he started pulling, his finger came in contact with the end of the hose clamp. As his finger passed over it, the clamp caused a 1" laceration of his left index finger. Employee was taken to POH Medical Ctr where he was given a tetanus shot and 4 stitches.

While installing side bits on a loader bucket, employee hit the bit with a hammer and a piece of steel came off hammer and hit left index finger. Employee went to clinic on 4/25/05. He was dispensed 800 mg of Motrin, 500 mg. of Ciprofloxacin and given a splint to wear on finger. Has follow up visit scheduled for 4/29/05.

Incident reported on 5/2/05, stating that lower back started hurting & pain increased thru the week. A new seat was ordered & installed. Restrictions issued by doctor on 5/2/05 did not inhibit him from performing his regular job duties as a plant operator but our company safety director contacted MSHA in Colorado on 5/19/05, and it was determined to be reportable to MSHA.

Employee struck his right elbow on a 3/8" spray nozzle in the spray bar while installing a steel liner on the feed end of sand screen. X-rays at clinic revealed no positive results. He was placed on restrictions by doctor but was able to perform normal job duties within restrictions. Visit to doctor on 1/10/05 said no work until 2/14/05.

While exiting front end loader, employee was climbing down ladder and he fell backwards hitting his head on the ground. Employee had multiple objects in his hands which caused him to loose his grip.

A customer truck was overloaded and driver had to dump off some of the load. The truck driver was holding his tailgate partially open and did not see loader operator come over and grab on to the other end to help. The truck driver went to slam the tailgate shut and employee's left hand was smashed in the latching mechanism of the trailer.

Employee was cleaning crush plant floor. He picked up the 41/4 crusher feedbox support post to move out of way to clean. Post is approximately 5' long, 3" diameter steel, 120 lbs. While dragging water hose to clean area, he felt a sharp pain in his right groin area. He states that he initially felt pain when he lifted up post.

Started having tingling and numbness in right hand on Wednesday, June 30, 2004. Felt soreness in right hand from loading trucks, moving tilt and raise/lower levers with right hand. Notified manager, worked through shift. Tingling was bad during the night, went to clinic next day.

Employee noticed a sheet of plywood that had blown over and could cause a hazard, so he bent over and stood the plywood back up against the control tower. In doing this, he felt a pain in his groin area on the right side. Wixom Occupational Health Clinic diagnosed him to have a right inguinal hernia.

EE WAS CUTTING ON THE OUTSIDE OF THE 6A HOPPER WITH A TORCH WHEN HIS LEFT GLOVE CAUGHT ON FIRE BETWEEN HIS THUMB AND FIRST FINGER. HE TOOK HIS GLOVE OFF & PUT SNOW ON THE BURN TO COOL IT OFF. HE STATED HOW UNUSUALLY FAST & HOT IT WAS. ANOTHER EE WHO WITNESSED THE INCIDENT INSPECTED THE TORCH TO SEE WHY THIS HAPPENED. A HOLE WAS FOUND IN THE HOSE. IT WAS COVERED W/ELEC. TAPE.
