Showing all 23The employee was cutting angle iron with a 4.5" grinder. Employee shut the grinder off but, with the wheel decelerating they set it down and it made contact with the back of their hand causing a laceration wound.
Employee was plasma cutting on a loader bucket. EE then was needle scaling the slag off when a piece of slag shot off and got under EE's safety glasses.
Employee was welding grating down and was bent over when employee twisted employee's back causing pain in lower back
Installing the a burner pipe on Kiln 7, a tab needed cut off the pipe seal ring. Employee used 4 1/2 in grinder with a .042 cutting wheel to cut tab off. Grinder started to bounce, employee stopped. EE then when to cut the tab again and the wheel shattered. A broken part of the wheel flew back and cut employee left hand, middle knuckle.
Employee was grinding and welding all day and noticed EE had something in EE's eye when EE got home. EE tried flushing the eye at home. Reported injury the next day. Tried flushing on site, went to local clinic where referred to ER.
Employee was drilling a flange when the drill twisted his finger causing a fracture.
Employee was grinding the rough edges off of a 4" x 6" x 1/8 plate using a bench grinder. He tilted the plate at an angle and the wheel sucked the plate into the gap between the guard and grinding wheel, lodging his finger between the wheel and plate.
While operating a D-5 Oldenburg dual boom drill the drill slipped and lost traction and the rear of drill impacted a pillar. Employee was thrown into dash on impact.
Employee could see the steam coming off of the diesel pump and knew it was not pumping. Cold water running into the hot pump case created steam pressure that exceeded the hose clamps capability and it blew off the suction hose. Hot water then spilled out of the pump case and splashed onto the employees legs.

Employee was using an air impact wrench to loosen bolt on a piece of mobile equipment. Employee was holding onto socket so it would not come off and had hand in contact with rotating part of wrench.
Employee was bag loading RKP material. When the material entered the bag, dust splashed from bottom of bag and entered his eyes.
Employee was raking coating off of the cooler grates of #6 Kiln. While raking, the kiln puffed back from positive pressure inside the kiln, causing dust to fly out of the door, where he was raking from.

Employee was pulling on an air hose and the valve got turned on and it whipped and struck him by his left eye.
THE EE WAS CUTTING OFF A USED STAINLESS STEEL BURNER NOZZLE WITH A SLICE GUN THAT CONNECTS AN ARC WELDER WITH COMPRESSED OXYGEN. THE EE ISN'T EXACTLY SURE WHAT HAPPENED AS HE WAS BURNING HIS SECOND ROD WHEN IT FLARED UP BURNING HIS FINGERSON HIS LEFT HAND.
THE EE WAS WORKING ON 980C LOADER, CHECKING FOR ANTIFREEZE LEAK, AND WAS PUTTING SIDE PANELS ON THE RIGHT SIDE BACK ON WHEN HIS RIGHT HAND WAS HIT BY THE FAN AND KNOCKED IT DOWN ONTO THE GUARD , CUTTING HIS HAND.
THE EMPLOYEE WAS RUNNING A 4"GRINDER WITH WIRE WHEEL ON IT WHEN THE WIRE WHEEL CAME APART AND THE WIRE STRUCK ON THE INSIDE OF BOTH LEGS.
EMPLOYEE WAS REMOVING BRICK FROM THE KILN WITH AJACKHAMMER. WHILE HAMMERING OUT BRICK ON THE BOTTOM OF THE KILN A PIECE OF BRICK FELL FROM OVERHEAD AND STRUCK HIM ON THE BACK OF THE NECK. SAW A DOCTOR ON 10/18/00.
