Employee placed hands on the fork of the hydraulic bowl adjustment ram in an attempt to free it and reposition it. Another employee actuated the hydraulic cylinder of the ram, and it retracted before the employee could move a finger out of the way. The tip of ring finger was crushed between the fork and the cylinder.

Employee was welding screen 1 roolback plate when EE noticed some irritation in right eye. EE removed contact and kept working. EE went home and eye began to swell. EE texted supervisor and reported the issue. EE was taken to ER which referred EE to eye doctor.

Injured employee was removing a wear plate from a feeder. As a weld was being cut free with a torch, a secondary weld failed, allowing the steel plate to release, striking the employee on top of the left foot. The employee sustained broken bones to the left foot.

Employee was passing a power drill to another employee when EE accidentally pressed the trigger. This caused the drill to rotate, striking the employee's left ring finger, resulting in a fracture to the finger.

Employee was troubleshooting a Furuwaka DCR 20 Drill and finger was caught between moving parts. EE received a finger laceration requiring sutures. EE was wearing cut protective gloves but the impact cut through the gloves.

Employee was utilizing a small/dremel type tool to grind a shaft when a piece of debris entered employee's left eye. The employee did not report the injury until the following day. Employee received treatment at a local clinic and was prescribed eye drops.

Employee was adjusting the set screw that controls the RPM's on the grinder in an attempt to lower RPM's. While the grinder was being adjusted the disk broke apart striking employee on left hand between middle finger and ring finger.

A mining foreman was operating a Cat D6K2 LGP dozer blading a haul road. While blading he struck a rock, buried in the subgrade. This stopped the machine suddenly, causing the operator to lunge forward and strike his head against a steel grab bar in the machine. Injury resulting was a left forehead laceration above the left eye, requiring 10 stitches.

Drill techs shirt sleeve got caught between rotating rod and chain sling being used to load rod on the drill. Individual sustained a broken elbow, cracked rib, cut on his forehead and abrasions to his chest, arm, and neck from his clothing.

Employee was removing bolts from a wear plate segment on a loader bucket. One bolt dropped down out of its seat causing it to spin when the employee applied the impact wrench. The employee used his left hand to lift the bolt back into its seat and applied the impact wrench. The bolt then spun causing a laceration to the left middle finger that required stitches.

Employee was positioning hydraulic ram on 4 1/4 cone crusher to tighten bowl down. As ram retracted employee did not realize his finger was in the way of the moving cylinder. As the cylinder closed the tip of his right index finger was crushed.

Welding with mig welder and accidentally hit the trigger switch to on position. The welding gun was near his left leg when this happened. This caused the welding wire to penetrate into his left calf.

Employee was assisting a mechanic to manually turn the drive on a Telsmith 68S crusher to prime the oil system when he accidently pinched his right little finger and ring finger in the v-belt against the sheave. The injury resulted in the amputation of the tip of his little finger and reconstructive surgery to the tip of his ring finger.

Technician inspecting mobile crane and unaware of boom position in regards to positioning of technician's hand. Boom was retracted and sliding section of boom injured technician's hand. Cuts to fingers requiring stiches and fractured bone in right ring finger requiring 2 pins.

While clearing trees with a bulldozer, the employee opened the cab door to look at his track while moving when a broken tree branch struck him on his left hand. He suffered a laceration to two fingers requiring sutures. He returned to work with no restrictions.

Employee had been welding and line boring to repair a front end loader. After completion of work while cleaning up he began to experience discomfort in right eye. He went to dr 11-8-06 and had small metal fragment remove from eye.

A plant maintenance welder was welding in a confined area at our jaw crusher feeder box when he was subjected to welding flash. Due to the precarious position he was in, his helmet got hung up on a support beam above him as he was flipping it down to cover his face. Subsequently striking an arc and receiving enough ultra violet rays to cause some cornea burns of the left eye.

A Hammer Operator breaking a boulder in the pit. The boulder was large and his point was approximately 6 feet off the ground. When the hammer point broke through the rock, the base of the hammer hit the edge of the rock, jarring the excavator and the operator. The operator expressed that his neck hurt and was taken to the doctor. He was diagnosed with a strained neck.

Employee was cutting a bolt with a torch. Torch brushed across the employee's finger and burned through glove, causing a second degree burn to the employee's left middle finger.

EMPLOYEE CUT A POLYDEX SCREEN PANEL WITH A CHOPSAW. HE BECAME ILL LATER THAT EVENING AND DEVELOPED A MILD COUGH. 2 DAYS LATER HE WENT TO THE EMERGENCY ROOM BECAUSE OF SHORTNESS OF BREATH. HE WAS DIAGNOSED WITH CHEMICAL PNEUMONIA, CAUSED EVIDENTLY DUE TO FUMES FROM THE SAWING OPERATION.

EE WAS HOLDING AN ALLEN WRENCH WHILE 2ND EE USING AN AIR WRENCH TO TIGHTEN NUT. THE WRENCH SPUN THE NUT UNTIL IT CAUGHT, SPINNING THE BOLT, WHICH CAUSED THE ALLEN WRENCH TO SPIN. THE ALLEN WRE NCH CAUGHT THE EE'S FINGER BETWEEN THE FRAME AND THE WRENCH. THEY WERE REPAIRING A FEEDER PAN LINER.

EE WAS CLEANING CATWALK AROUND 68S TELSMITH CRUSHER. THE EE LAID HIS HAND ON TOP OF THE CRUSHERSHOUSING AND BENT OVER TO PICK UP SOME ROCK OF THE SURROUNDING CATWALK WHEN A ROCK OF UNKOWN SIZE BOUNCED DOWN OFF THE ABOVE 6X16 SCALPING SCREENAND HIT THE EE'S LEFT HAND.

EE WAS HELPING CHANGE SCREENS WHEN HE STUCK HIS FINGER INSIDE THE SOCKET ON A 3/4"IMPACT WRENCH TO REMOVE A NUT STUCK IN THE SOCKET & ACCIDENTALLY HIT THE TRIGGER ON THE IMPACT WRENCH CAUSING THE NUT TO SCREW UP THE END OF HIS FINGER AND INTO THE FIRST LOWER FINGER JOINT. THE LACERATION REQUIRED STITCHES.
