2'6"x29' Steel Concrete form clamp system failed, allowing the form to seperate into 2 sections (2'6"x9' and 2'6"x21') as it was being lowered to the ground. 9' section broke away from main piece and dropped to the ground. 21' section remaining suspended by crane dropped to the ground impacting injured.
