Struck by flying object

EE was working on the Primary #1 Feeder Mount. A spring was broken on the right 3/4 inch bolt. EE replaced the spring and was tensioning the guide nut, a worn portion of the bolt that was weakened failed, launching the weakened portion into EE's right cheek due to the stored energy of the upper spring.

Employee was checking the plant around #9 belt Heard something turned around to see what the noise was. Tripped and fell hit their head and right shoulder.

2'6""x29' Steel Concrete form clamp system failed, allowing the form to seperate into 2 sections (2'6""x9' and 2'6""x21') as it was being lowered to the ground. 9' section broke away from main piece and dropped to the ground. 21' section remaining suspended by crane dropped to the ground impacting injured.

Ee was conducting a pre-shift inspection on the dozer. It was raining and ee was standing on the track and he slipped and fell hitting his left shoulder on the ground.

EE WAS CLEANING BUILD UP MATERIAL IN THE CRUSHER. HE WAS USING A CHIPPING HAMMER TO LOOSEN MATERIAL AND GOT METAL IN EYE.

EMPLOYEE WAS HAULING MATERIAL TO AN ONSITE CONCRETE PLANT WHEN HE CAME UPON A DRILL PARKED IN THE ROADWAY. HE ATTEMPTED TO PASS THE DRILL ON THE LEFT SIDE WHEN HE CAME TOO CLOSE TO THE EDGE OF THE ROADWAY. THE GROUND WAS SOFT CAUSING THE TRUCK TO TURN ON ITS SIDE.

EMPLOYEE WAS STANDING ON A CRUSHER FRAME (APPROXIMATELY 3""-4"" HIGH) ATTEMPTING TO REMOVE A ROCK FROM THE HOPPER. HE MOVED ONE FOOT TO RELEASE THE ROCK WHEN HIS OTHER FOOT SLIPPED CAUSING HIM T O FALL ON HIS RIGHT SIDE.
