Two EE's were placing 6" PVC pipe at a tire wash. After gluing into place, they were back filling a Bobcat. As EE swung the backhoe for more material, injured EE reached down and pulled up on the pipe, the bucket of the backhoe passed, caught the tip of EE's right index finger between it and the end of the PV pipe, cutting a piece of skin.
Employee was greasing rollers on dragline with an air accuated grease gun. Hose developed pin hole & grease filled his glove and (L) thumb because of extreme pressure. Employee had surgery to remove grease from around bone. He is off work presently.
EE was working with a zip gun removing material from a pipe. He felt sore afterwards, which he later complained the day after. He was taken to the workers comp Dr. who mis-diagnosed him with B-Lat Carpal Tunnel. He the went to orthopedic surgeon who diagnosed him with tendinitis.
While working on removing bucket pin on a 992 loader, dust went into employee's eyes, possibly from the exhaust from the Impact wrench while working in a confined area (employee was wearing prescription safety glasses with side shields).
Employee got finger caught between a belt and a shieve (fan). Finger was injured while employee took eyes away from hands direction causing the finger to get caught.
EE was attempting to tighten a bolt on the drag friction with a hand wrench. The wrench was struck by a drum bolt causing EE's ring finger on L. hand to become pinched between the wrench and drag friction. A fracture to the last digit of the ring finger on the L. hand was realized.

OPERATOR WAS RAISING UP THE BOOM. HE GRABBED THE MASS PIPE AND THE BOOM STABILIZER CRUSHED HIS FINGERS.
EMPLOYEE WAS GRINDING A FLANGE WITH AN 11000 RPM ELECTRIC GRINDER. HE WAS PUTTING THE GRINDER DOWN WHEN IT BUMPED HIS RIGHT LEG JUST ABOVE HIS KNEE. IT TOOK SIX STITCHES TO CLOSE THE WOUND.
EE WAS GRINDING ON A SIDE WALL HOLDING GRINDER OVER HIS HEAD IN FRONT OF HIM. HE HIT THE FRAME OF UPPER DECK CAUSING GRINDER TO JUMP, TWISTING HIS ARMS CAUSING GRINDER TO STRIKE HIS WRIST, LAC ERATING IT REQUIRING STITCHES.
WHILE REMOVING A CYLINDER ON A P&H DRAG LINE A BOLT FLEW OUT AND HIT RIGHT CHEEK OF EE CAUSING LACERATION.
EE WAS CUTTING A PIECE OF 1 1/4" PIPE WITH A PORTABAND SAW. WHEN THE SAW CUT THROUGH, EE'S HAND WAS CUT BY PIPE.
EE WAS OPERATING DREDGE WHEN HE INADVERTENTLY STRUCK RIGHT ELBOW ON A LEVER CAUSING TEMPORARILY NUMBNESS TO RIGHT ARM.
DRILLING BOULDER, WENT TO MOVE THE BOOM OFF THE BOULDER, IT SLIP DOWN AND PINCHED HIS FINGER.
EMPLOYEE WAS LUBRICATING THE WALKING CAM OF THE DRAGLINE. EMPLOYEE'S HAND WAS PINCHED BETWEEN THE CAM AND FRAME.
EE WAS MOVING DRILL FROM ONE SHOT TO ANOTHER, HE STOPPED, AT THE TOP OF THE HILL FOR TRACKS TO MOVE, HE PROCEDED DOWN THE HILL ABD STARTED TO MOVE FASTER, HE ATTEMPTED TO APPLY THE BRAKES BUT DRILL WAS MOVING TOO FAST TO STOP. HE DROVE IT ONTO THE BERM TO STOP IT. DRILL STOPPED IN SLOW MOTION AND ROLLED OVER.
AFTER HOLED DRILLED, DRILLER FORGOT TOREMOVE RUBBER DUST COVER FROM ROD; LOOKED UP TO WATCH HEAD OF DRILL & SWING ROD INTO CAROUSEL. AS HE LOOKED UP THE FALLING RUBBER COVER STRUCK HIM THE FAC E KNOCKING OFF HIS SAFETY GLASSES & HARD HAT. HEWAS TAKEN BY CO. VEHICLE FOR EMERGENCY CAREA; WAS RELEASED WITH NO BROKEN BONES; NO STITCHES; NO CONCUSSION. WAS TOLD BY ATTENDING PHYSICIAN HE
WHILE USING A GRINDING WHEEL, THE WHEEL CAME OFF AND CUT EMPLOYEE ON LEFT FOREARM.
SUPERVISOR WAS HELPING ANOTHER DEPT TO DISABBEMBLE A PUMP. THE BRAKE ASSEMBLY BREAKING WERE STUCK. WHEN THE ASSEMBLY WAS LOOSENED IT WENT TO REVERSE CATCHING HIS THUMB DUE TO UNKNOWN PRESSUR E IN THE BASE OF THE PUMP.
EE WAS HIT ON HIS HARD HAT BY A ROCK WHICH WAS THROWN OUT OF THE CRUSHER.
EMPLOYEE WAS WELDING WHEN A PIECE OF SLAG FELL INTO HIS BOOT AND BURNED HIS RIGHT FOOT.
EMPLOYEE WAS CUTTING A BOLT OFF IN THE PLANT ANDA PIECE OF HOT METAL FELL INSIDE HIS RIGHT BOOT AND BURNED HIS FOOT.
DRILLER WAS PUTTING A NEW BIT ON THE ROD, THE WRENCH SPUN AROUND AND HIT HIS HAND CUTTING THE POINTER FINGER ON HIS LEFT HAND.
EE WAS WAITING TO BACK UNDER RIP RAP LOADER. HE WAS SETTING ABOUT 50 TO 100 FT AWAY FROM HAMMER BREAKER. EE HAD WINDOW DOWN ABOUT 6 INCHES ON PASSENGER SIDE. HE HAD PIECE(SMALL) ROCK FLY INTO SIDE WINDOW. IT HIM ON HIS RIGHT SIDE OF HEAD.
EE DEVELOPED BACK PAIN WHILE OPERATING BULLDOZER.
