EE and co-worker were laying out the tow of slope. EE did not communicate with the operator of the Volvo 750 to let operator know EE was in the area. Operator swung the excavator around and struck EE with the bucket. EE received a cut above left eye.(around 6 stitches).

Showing all 33EE was working on closing a lid on a VSI Crusher when it became stuck on descent. EE states they placed hand between lid slot and ear for the lid as they were asking the operator to raise stop and raise the lid. The lid released unexpectedly causing the EE's left middle and ring fingertips to be severed.
EE was performing hot work on a crusher roll, as EE stepped back to review the cut EE had made, the limestone fines found in the crevices of the roll burst outward, causing dust to enter the employee's left eye. Employee was seen by Dr. and a corneal abrasion was found and prescribed eye medication.
WHILE GRINDING ON CRACK ON DISCHARGE OF DEWATERING SCREEN, GRINDER HUNG ON CRACK AND KICKED BACK; KNOCKING OFF WELDING HELMET AND HITTING EMPLOYEE'S MOUTH, CUTTING LIP.
The employee was removing bolts on a screen spring with a torch when a piece of debris got behind EE's glasses and into EE's eye.
While removing a fly wheel for maintenance, a 3/4" bolt broke. Flying debris hit a worker and bruised his left arm and right hand.
While removing flywheel for maintenance, a 3/4" bolt broke, hitting worker above the eye and making a minor cut requiring stitches.
Employee was using an impact wrench to tighten a J-bolt. His left index finger was struck by the spinning j-bolt resulting in soft tissue damage and a slight fracture. First aid treatment only. X-ray positive for fracture is the reason for reporting.
Pan was in down position that would cause tire to elevate off of ground by 4"-6". Tire has stored energy (potenetial energy) prior to breaking the tire free from the structure. When tire was freed, tire dropped, pivoting about left side putting employee's hand in a pinch point between the frame and the tire.
Employee's left index finger was caught between an electric motor and motor base as the motor was being lifted by a mobile crane. The employee was trying to prevent the motor from swinging.
The employee was grinding inside a shaker when a tiny particle bounced off the side of the shaker and went behind his safety glasses. The particle was removed with a cotton swab but antibiotic drops were administered to prevent infection.
EE was using a grinder and a piece of metal fell into his right eye. He was wearing required PPE and conditions were windy.
Employee was cutting loose a grizzly when it shifted pushing him up against a support beam. Bruising to the neck, shoulder, and chest.
Employee was working on a slope with a dozer and laid it on it's side at the bottom of the slope.
Employee was cutting metal with a torch. A piece of hot metal fell and burned through his pants and fell into his boot.
Drill steel fell out of carousel. Driller couldn't get it back in by himself so he had EE come help him. Pipe was stuck so the driller started the rotary head down to unstick it but it hit the pipe smashing EE's hand.
Employee was operating a dozer pushing material. As he was backing the dozer, he struck a large rock, jarring and straining his neck.
EE was cleaning slag from a weld with a grinder and the dust or debris go into eye
Employee struck by water discharge hose, 10", 65 psi, when hose seperated from fitting.
Employee was grinding on a weld when he was struck in the left eye by a foreigen object
Employee was helping another welder, received welders flash to both eyes.
EE was assisting the foreman sort railroad iron with a backhoe. The foreman was pulling a piece of iron with the bucket of the backhoe when it became stuck & then suddenly came loose & flew striking the EE about 8" below the left knee; this resulted in a broken leg.
Crew was trying to get engine on drill to start. They pulled driveline to pump box and at same time pulled guard that covers driveline & alternater. For some reason, inj EE reached down to check tension on alt belt at which time engine turned over, catching his finger between belt & pulley.
Flat bar, employee was attempting to weld, shifted away from him and employee jerked back and hit his left elbow on T-Bar.
Ignition switch on pump engine not working properly. EE tried to turn crankshaft to get flywheel & starter in time. He grabbed fan with l. hand & pushed fan belt in with r. hand. The engine started with key in off position & pulled his r. hand fingers (index & middle) in between fan belt & crankshaft pulley. Severed fingers (index: middle phalange; middle: distal phalange).
DRILLER & HELPER WERE IN THE PROCESS OF REMOVING BROKEN DRILL STEEL FROM CAROUSEL. BAD STEEL WAS ATTACHED TO WINCH LINE PICKING IT UP TO LAY ON GROUND. HELPER WAS PUSHING STEEL AWAY FROM RIG WITH ONE HAND WHILE THE OTHER WAS ON THE RAILING FOR SUPPORT. DRILL STEEL SLIPPED FROM HELPERS GRIP AND SWUNG BACK AND SMASHED THE OTHER HAND AGAINST THE RAIL.
Injured employee was assisting in moving mats for scraper and a miscommunication between the IE and the dozer operator caused the operator to lower the pad which smushed the IE left little toe and resulted in a fracture.
Employee was in the primary crusher welding on the hammers with a hand welder. The fan they use to blow the smoke away from where they were welding blew a welding spark up under his helmet into his eye.
While drilling, employee wiped his (L) eye with his sleeve and got a piece of metal in it.
Employee was operating tail winch on D6D Dozer, operator engaged winch & got out of cab in order to guide winch cable into the correct position onto the winch drum. He did this by using his left foot as a guide while operating the winch with his right hand. This caused his foot to become caught in the winch resulting in a cracked bone in his left foot.
Three employees were moving the 7400 dragline which is powered by a large power cable in which a Dozer is used to move while walking the dragline to the new location. The ground man was directing the dozer operator were to go when the cable was caught under some material and caused the cable to pop and struck the employee under the right knee.
Employee was welding on a 12" pipe in the sand plant and was grinding on root pass when the wind gusted and particles got into his eyes.
Employee was grinding and when he raised his face shield, accumulated metal shavings on the frame went into his eye.
