Contact with hot objects or substances

Employee had fueled a small engine in the back of a Lube Trailer, set the gas can down, went to start the engine and there was an ignition. Flash fire burnt them on the Forearms, Hands and Face.

Struck by powered moving object

EE and co-worker were laying out the tow of slope. EE did not communicate with the operator of the Volvo 750 to let operator know EE was in the area. Operator swung the excavator around and struck EE with the bucket. EE received a cut above left eye.(around 6 stitches).

EE was attempting to extend a come a long to the span EE's task required EE to. While free spooling the come a long, EE's right index finger was pulled into the guide sprockets which caused the EE to sustain an avulsion to the tip of the right index finger. Work restrictions were issued as a result of injury.

EE was assisting contract mechanic to remove track tensioner from a Komatsu 65 dozer. Cylinder was stuck, so tension and sledgehammer was used to remove it. After striking tensioner several times, EE stated EE felt pain in right hand. EE was taken to Dr. on 11/6/2017 and given work restrictions and splint.

Struck against stationary object

I.E. was returning to control room, ducked below an I-beam, EE stumbled and lost EE's hardhat. Upon recovering EE's footing the crown of EE's exposed head struck another I-beam causing a gash across the width of EE's scalp. EE was treated with 6 stitches and tetanus shot. Returned to work with out restrictions.

Pulling parts off old dragline to repair loading dragline. Prying and sliding spider gear off shaft and when it came off end of shaft it pinched finger between spider gear and shoe.

employee stepped down on gravel covered culvert and the culvert / ground gave way and he fell. When he got up he said his left leg was cut above his knee

EE was gathering supplies to make a lace in conveyor belt when a bug flew in under his safety glasses and got in his eye. He took his glasses and glove off to remove the bug and he said his hands were a little dirty. Don't know if the bug or the dirt scratched his eye.

Struck by flying object

3 ee let off the tail pulley to make a lace. After making the lace they were trying to tighten the belt with the winch and noticed the brake was set on the winch. 1 ee was holding the handle of the winch with both hands while another ee released the brake and the handle slipped out of ee hand and hit him in left eye.

Guide roller on conveyor was not turning.Injury picked up a hammer to tap it with and as he approached the roller he said he tripped and fell into roller and conveyor.

EE was down inside the dragline between the brake drums in the chain case, cutting a bolt off to replace it, because the threads were stripped on it. The temperature had exceeded ninety degrees that day and he became overheated while in the confined space.

Mechanic was hammering on a coupling on dragline fan pulley shaft. Injury was standing behind him and when he swung the hammer up over his left shoulder to hit the coupling again The hammer head came off the handle and hit him in the chin and neck area cutting his chin.

DURING REPAIRS TO CONVEYOR, EMPLOYEE'S LEFT PINKY FINGER WAS MASHED BETWEEN THE CONVEYOR LEG AND THE BELT CLAMP WHEN ANOTHER EMPLOYEE PULLED ON THE BELT CLAMP, RESULTING IN A CUT AND BROKEN FINGER TIP.

He was getting bolts out of bolt bin at shop & tripped over a pallet in the floor. he went to the dr and the dr said it was only a sprain. after 2 1/2 wks. ee foot kept hurting so he went back to the dr on 8-24-04. upon x raying his foot they discovered a small bone had been broken in his ankle.

EMPLOYEE WAS OPENING DOOR OF CRUSHER TO EXAMINE INSIDE. HE WAS USING A PRY BAR TO RELEASE SAFETY LOCK. WHEN THE LOCK BROKE FREE HIS LITTLE FINGER WAS PINCHED BETWEEN THE PRY BAR & THE DOOR.

EE WAS STANDING AT THE NORTH END OF THE SHAKER. CO-WORKER WAS PUTTING A PATCH ON THE TOP DECK OF THE SHAKER ABOVE EE WHEN HE DROPPED THE PATCH & IT HIT EE ON HIS LEFT HEEL. THE WEIGHT OF THE PATCH BUSTED HIS HEEL OPEN REQUIRING 3 STITCHES.

EE WAS CLEANING OUT FROM UNDER HIS BELT WITH A SHOVEL WHEN HE FELT LIKE HE PULLED A MUSCLE IN HIS BACK.

THEY WERE MAKING A LACE IN ONE OF THE BELTS. THE COMEALONG WAS HOOKED TO THE BELT CLAMPS HOLDING THE BELT TOGETHER WHILE THEY LACED THE BELT. AFTER MAKING THE LACE, EE WAS RELEASING THE COMEAL ONG AND IT SLIPPED, PINCHING HIS HAND AND BREAKING HIS LEFT THUMB.

EMPLOYEE CLIMBED UP ON THE LOW BOY TRAILER AND FELL THROUGH A HOLE IN THE DECKING, WHEN HE CAUGHT HIMSELF WITH HIS HANDS HE RIPPED HIS RIGHT PINKIE FINGERNAIL OFF.

EMPLOYEE WAS STANDING ON THE LOWER PART OF HANDRAIL PUTTING A RAG IN A HOLE ON THE SANDBOX WHEN THE RAIL BROKE AND HE FELL ABOUT 1 1/2' STRIKING HIS GROIN AREA ON THE TOP RAIL. WHEN HE HIT THE RAIL IT MADE A TEAR IN HIS PENIS AND SCROTUM REQUIRING 11 STICHES.

EE WAS AT THE PLANT 1 SHAKER WELDING AND HIS CUTTING TORCH STOPPED UP, INSTEAD OF CLEANING IT WITH A TIP CLEANER HE WIPED IT ON HIS WELDING GLOVE TO CLEAN THE SLAG OFF. THE TORCH WENT OUT WHIL E HE WAS WIPPING IT ON HIS GLOVE AND HIS GLOVE APPARENTLY FILLED WITH GAS. WHEN HE RELIT THE TORCH IT IGNITED THE GAS AND BLEW UP HIS GLOVE.

EMPLOYEE WAS CUTTING THE BARRELL OUT OF AN OLD SHAKER AND A PIECE OF METAL CAME UP BETWEEN HIS GOGGLES AND HIS FACE AND GOT IN HIS EYE.

TRYING TO GET GREASE GUN PRIMED. HIT GUN ON RUBBER SLEVE OF FRESH WATER PUMP. THERE WAS A STUD STICKING OUT OF RUBBER SLEVE AND FINGER WAS HIT BETWEEN STUD AND GREASE GUN TEARING OFF FINGERNAI L AND BREAKING TIP OF FINGER.
