Showing 50 of 93On February 4, 2026, an ACME field employee reported to EE's foreman that EE's right forearm felt strained but did not believe medical attention was needed. EE later requested medical attention, stating EE's forearm began hurting after using an air hammer for two hours removing excess concrete.
EE was washing out the raw mill posimetric chute with a water lance. The water hose became entangled with the door, causing the door to begin closing. The door struck the employee's right right wrist resulting in a contusion. The injury became reportable on 11/18/25 when the doctor performed professional medical treatment during a follow up visit.
While using an air lance to clear a chute the employee got clinker dust into EE's eyes. EE's eyes were flushed and EE was administered an antibiotic eye drop medicine.
Employee was grinding a beam when it grabbed on the beam and the hand holding the handle slipped off. The grinder slipped back into their wrist as they came off the trigger with their other hand. The disc caught their left forearm and required 4 stitches. EE was wearing proper PPE and all guards were in place.
Employee was removing roll over protective structures of wheel loader, the right side was clear when the left side was hanging from crane ROPS and hung up on filter housing, they put a hand on the cab to see what was stopping it when they touched the cab the ROPS shifted pinching their finger against the cab.
EE was welding inside of FM6 classifier distribution table. The Welding mask kept slipping, obstructing EE's vision. The welding mask was altered and the lower section was cut off. EE completed task using altered PPE and finished EE's shift. 4 hours later EE was treated at local ER for welding flash burns to eyes.
Piece of shafting fell from lathe causing a fracture to right foot metatarsal bone.
While checking the torch on K1, employee noticed an air lance inserted into the cyclone on K2 hood and when walking to check why it was there the material blew out of the pipe and onto their arm and face.
While blowing down an air slide, Miner was not wearing proper PPE and material blew back into the Miner's face causing a right eye injury.
Was feeding a steel strap into a rolling machine and the glove on left hand was caught by the roller and pulled in. The ring finger on left hand was crushed up to the first joint and removed by Doctor at the first joint.
Employee was using a hydraulic puller to remove a gear from a shaft. EE had a hand on the body of the puller. The puller came loose from the gear and EE's finger was momentarily caught between the arm and the body of the puller causing a fracture to the little finger.
Worker was loosening allen bolt from cylinder with 1" air impact attached to swivel with a 24mm allen socket, when socket slipped off bolt and came into contact with workers leg resulting in a contusion on middle inside right thigh.
Employee was operating a vac hose when left arm was sucked into the hose. The arm was swollen from the blood being pulled to the hand.
Employee was pressure washing a loader and some material went around employee's safety glasses and got in eye.
Employee was using a cutting torch in the mill. A piece of slag went under EE's face shield and over EE's safety glasses and got into EE's left eye. ER doctor said there was evidence of a burn and gave prescription numbing drops.
EE was using an air lance to blow cement dust out from underneath a conveyor belt and the dust went into a gap that was between the dust mask and goggles that the employee was wearing. Cement dust went into the employees eyes resulting in an eye abrasion.
While using a chipping tool to remove material buildup between mill jack arm, EE smashed finger between jack arm and arm plating.
While a refractory pump was being worked on an employee reached hand in to remove some rocks that were visible. The pump was running and the swing gate on the valve closed causing three fingers to be dismembered.
EE was cleaning the riser with a pressure washer. When EE tilted EE's head to the left dust fell into EE's right eye.
The employee was using a jackhammer to attempt to free another jackhammer that was stuck in a pile of material. EE's right ring finger was pinched between the two jackhammers. EE fractured the tip of the finger when it was smashed between the them.

Employee was cutting plate with a grinder ans while taking off a little to fit into place the grinder came off the steel and struck employee in the left thumb.
Employee was drilling holes in screw conveyor covers overhead when the drill bit caught and twisted right arm.

Employee was dressing up a piece of metal when the hand held grinder kicked back and struck employee in the lower lip requiring 7 stitches.
Employee received a burn to left eye from the welding slag while sitting beside a coworker. Received a prescription.
Employee injured EE's finger requiring 3 stitches to the index finger. The maintenance mechanic attempted to remove metal shaving from drill press while the drill press was on and drill bit slowly turning. The metal shaving penetrated through glove cutting right hand index finger.
Contractor was rolling #2 mill, subcontractor was hooking cable to bolt on to manually turn mill. Mill was backspinning & cable got stuck. Subcontractor attempted to slide cable back on to bolt, while trying to prevent backspin of mill, when cable tightened and pinched subcontractors left hand/fingers between cable & mill.

Employee was grinding. after grinding EE reported debris in EE's left eye. All PPE was in place.
Employee did not report to work on 2/13/2016. Supervisor called EE at the beginning of shift to check on EE and employee stated to supervisor that EE went to the doctor the evening of 2/12/2016 for eye pains and EE was diagnosed with welder flash. The employee reported to work on 2/15/2016 with no symptoms. EE was taken to company doctor on 2/16/2016 for evaluation.
Employee's auto vision lens on his welding hood malfunctioned causing flash burns in his left eye. Employee did not report the injury until 11/2/13 and denied medical treatment until 11/3/15.
Employee was using a cutting wheel to cut a piece of pipe, felt something in right eye. Didn't report injury to supervisor until 9:30 a.m. next day.
Employee was working on Pre-Heater tower painting. at some point he thought he had something in his eye. Employee rinsed his eye and stated that he thought it felt better. two days later he went into the safety office and stated he felt that something was in his eye. Employee was transported to the clinic and it was determined to be flash burn.
While drilling a hole in a cable tray, employee stated he felt a piece of metal shaving hit him in the face causing discomfort to his right eye.
Employee and crew of 9 had the job task of removing beams in the cooling area. The beams were to be temporarily removed for other job tasks to be performed. As the employee began to loosen the bolts the extension or swivel on the impact kicked back and came into contact with his left forearm causing a laceration.
Employee was installing Tillman roller in the cooler frame in the clinker cooler area. As he was under the walkers to roll forward he utilized the hard back stand to lift the cooler. As he left a little pressure on the cooler he reached over to get the other Tillman roller. At the same time the porta power came off the stand and hit the employee on his left hand.
Employee was performing welding task on a column on the fourth floor elevation number 2674'9. While chipping slag off his weld a small piece came off and became embedded in his right eye. Another employee observed that he was using proper PPE eye wear when this incident occurred.
Employee was assisting in loading a trailer with scrap metal. The scrap was being lifted by crane. A piece of scrap got caught on nearby 10" I-beam. This lifted the I-beam off the ground. When the I-beam fell out landed on the employee's ankle.
Miner was assisting an electrician drill out a sighting hole. Miner used his gloved hand to support the drill bit. The miner's glove was grabbed and bent his right middle finger back. Diagnosis: Strain middle finger, right hand. Employee took a short cut to support the drill bit, rather than use the tool provided.
Employee was grinding stainless steel flange when a piece of metal blew into his eye. The employee was wearing eye protection: safety glasses and face shield. The employee flushed his eyes to remove the metal but he scratched his cornea before the metal was removed.
The employee was using a rivet buster to remove castable around the 2nd stage thimble. He was working on top of a scaffold. The employee said that he felt a pop n his left arm (forearm area) and pain. The supervisor contacted the Safety and Health Manager on Saturday morning 2/07/2015, to report that the employee is needing medical attention for his (L) arm.
Employee was using a mig welding gun on a vessel/ tank and set the mig gun down to chip off slag. As he was doing this he accidently fired off the gun. His welding hood was up and the arc flashed his left eye.
Employee installing 6" steel transport line for #2 FM FK pump. While welding line in place, employee had to lift off welding hood to inspect work. While inspecting the weld, a piece of slag popped off weld going around safety glasses & into left eye.
Employee was improperly using a 4-1/2" grinder (whereby the guard thus became ineffective) to grind welding slag off of a metal ring. While holding the grinder with his left hand improperly, the grinder became out of control striking the tip of his right thumb. Employee was wearing all required PPE.
