Employee removed lid from manhole to pump out water from scales. While waiting on water removal process to be completed, EE picked up an air hose and began walking backwards to remove slack from the hose and stepped into the manhole causing employee to partially fall into opening striking lower back

EE was assigned to build a platform and hopper. EE was wearing a harness but did not tie off and fell through the unfinished platform. The fall was approximately 13 feet. Injuries include a cut about the eye, wrist fracture and teeth knocked loose, and a cut on lip.
Employee was removing roll over protective structures of wheel loader, the right side was clear when the left side was hanging from crane ROPS and hung up on filter housing, they put a hand on the cab to see what was stopping it when they touched the cab the ROPS shifted pinching their finger against the cab.

Employee was working on the 300 elevator on FM7 torquing the splice. Torque wrench slipped and caught EE's fingers and smashed EE's right hand thumb and index finger in between the bucket elevator housing and the torque wrench handle
The pump is moved with a remote but sometimes gets stuck and the smooth metal wheels spin in place and the pump is ""stuck"" in place. The historical EE response has been to use a crow bar to lift and push the pump forward. While moving using the crow bar, the EE twisted awkwardly and then pain started in the lower groin area
EE was descending inside staircase to exit building. EE missed a step, slipped and fell down approx 5 remaining steps landing at the bottom of the staircase resulting in a fracture of the left ankle

EE was loading up tools and equipment into the back of the work van. As EE was stepping out of the back of the van, their right foot slipped off of the step causing them to fall forward with their left leg underneath them twisting a knee.
As employee was installing bolt on the jack of the raw mill. As the employee began installing bolts the padeye gave way. Causing the rams head to fall back down onto the employees right index, middle, and ring fingers. Crushing the tips of the fingers. The employee was taken to the hospital, and the tips of each finger were severed between the last knuckle and the nail bed.

EE completed task using altered PPE and finished EE's shift. 4 hours later EE was treated at local ER for welding flash burns to eyes.

EE was using a man lift to pick up roofing debris in the old warehouse EE could find. EE picked up a panel of about 6'X3' that was under some roofing debris. EE tossed the panel but didn't lift it high enough. This caused the L side of the panel to come down and hit the handrail and then the R side came down and struck EE in the R wrist. Resulting in a 3/4"" cut, 3 sutures.
Employee was moving gas cylinder bottles. While lifting bottles, employee strained knee while in motion.
Employee was dismantling a platform assembly when one member of the assembly fell and struck EE's leg. The injured employee was treated and taken to a local hospital.

Employee was picking a c-channel to cut into smaller pieces to weld. Employee asked for medical treatment the next day and was placed on restrictions.
Employee reports waking up on the 13th with pain in their left eye around 3:00 a.m. They woke up again at 5:30 a.m. still with pain at which point employee took out their contacts.

Employee was putting brick conveyor together when one section shifted pinching his finger between the two sections. Employee was wearing protective gloves.
The employee along with two other MZP employees were installing two U-shape (half) rings around a 4 foot diameter X 15 foot long duct pipe. After tack welding the first half of the ring, the second half was being fitted around the pipe when employee pinched the tip of the middle finger between the pipe and the second half of the ring. The employee was transported to the hospital.
Employee tripped over HDMI cable and fell onto right knee.
Employee's auto vision lens on his welding hood malfunctioned causing flash burns in his left eye. Employee did not report the injury until 11/2/13 and denied medical treatment until 11/3/15.
EE was uncoupling the product and struck hose that comes off the pump when he lost his footing and the hose was full of product and struck him on his thigh. There was no bruising or any sign of injury was visible, he was taken to be checked out to be safe because he was complaining of pain in his leg.

EE was standing in line near Taco Stand located near parking lot. EE felt something crawling when suddenly he was stung by a bee. Fifteen to twenty min later the ee experiences radiating pain up his left elbow. EE was transported by company vehicle to Sweatwater hosp. EE received an injection by doctor and was told to rest for day. EE retuned back to full duty next day.
Miner was clearing a horizontal screw conveyor. The lid pinched his right ring finger between the lid and motor guard fracturing his distal phalanges and requiring 6 stitches to close the wound. Miner was wearing PPE, including Kevlar gloves.

Employee failed to wear safety harness and tie off while working at height. Miner fell approximately 40 feet into a raw material bin. Initial medical assessment is broken bone proximal to the coxics bone.
Employee started feeling dizzy and was having trouble using his tools. We brought him up to the office to cool him down when he stated that he felt that he was going to pass out and was feeling numb. We called 911 due to his symptoms and after they arrived he was feeling a bit better. The EMTs took him to the hospital in an ambulance to check him out further.
While drilling a hole in a cable tray, employee stated he felt a piece of metal shaving hit him in the face causing discomfort to his right eye.
Employee and crew of 9 had the job task of removing beams in the cooling area. The beams were to be temporarily removed for other job tasks to be performed. As the employee began to loosen the bolts the extension or swivel on the impact kicked back and came into contact with his left forearm causing a laceration.
Employee attended morning toolbox meeting on job site. After the meeting he began to walk to the tower and noticed a bug flying around him. All of a sudden he tried to move the flying bug with his hand and the bug flew into his ear.
The employee got his thumb caught between two pieces of metal. While assisting with the placement of steps to access the tool trailer by sliding it into position, the unit unexpectedly shifted into the trailer and the employees thumb got caught between the metal steps unit and trailer. This caused a laceration and fracture to the thumb.
Employee reported that during the cleaning duty on his workplace hi transported an I-beam in his shoulder. While he use a stairs he slipped and the I-beam hit his right knee and cause him a injury. Employee was installing Tillman roller in the cooler frame in the clinker cooler area. As he was under the walkers to roll forward he utilized the hard back stand to lift the cooler. As he left a little pressure on the cooler he reached over to get the other Tillman roller. At the same time the porta power came off the stand and hit the employee on his left hand.
Employee was moving a welding machine inside of a tool trailer when he felt pain on the left side of his groin.
Employee was performing welding task on a column on the fourth floor elevation number 2674'9. While chipping slag off his weld a small piece came off and became embedded in his right eye. Another employee observed that he was using proper PPE eye wear when this incident occurred.
Employee was kneeling onto his kneepad when a piece of rebar went through his kneepad and punctured his right knee. CCCG placed the employee on unofficial restrictions from 4/16/15 to 5/1/15.
While loading an I-Beam, the beam shifted on the trailer while being placed for transport and came off landing on the employee's right hip.

Miner was climbing down from loader. He had a water bottle in one hand. He failed to maintain 3 points of contact as trained. He fell from the 2nd from bottom step onto concrete pad.
Miner was assisting an electrician drill out a sighting hole. Miner used his gloved hand to support the drill bit. The miner's glove was grabbed and bent his right middle finger back. Diagnosis: Strain middle finger, right hand. Employee took a short cut to support the drill bit, rather than use the tool provided.

Miner was transversing the mill building. He failed to see a step and fell on his left side.
Employee was using a mig welding gun on a vessel/ tank and set the mig gun down to chip off slag. As he was doing this he accidently fired off the gun. His welding hood was up and the arc flashed his left eye.
Two men were breaking down 3"" pipe to reuse elbows, tees, coupling, etc. While performing this task the wrench slipped smashing one man's hand between it and the concrete ground. This injury caused swelling to the top part of his right hand.

Employee pinched his right index finger between a small piece of wood and the shelf that he was placing the wood onto, resulting in a laceration. The laceration required 5 stitches to close.
Employee strained right shoulder while carrying a piece of pipe. Employee did not initially seek medical treatment. When pain persisted, employee saw a physician on 9/6/2013 and was placed on restricted duty.
While kiln #2 was in the cool-off stage of a maintenance shut down, pulverized coal was inadvertently injected into the kiln. When the kiln was turned to prevent warping, the coal ignited resulting in an immediate release of energy.
Employee was cleaning a pile of material under a screw conveyor. He noticed a leak in the conveyor housing and attempted to stop the leak by inserting a rag into the hole. The moving screw inside the housing amputated the tips of his right index and middle fingers just below the fingernails.
Employee was lifting a bag of rags to throw in a truck when he strained his back.
Employee strained his back while lifting a mill liner by hand, rather than using a chain hoist as instructed. The mill liner weighed approximately 100 pounds. He was placed on restricted duty by his doctor.
While working on a load out structure,EE got into an altercation/fight w/his brother. Both employees were terminated & escorted off the mine property. Prior to be escorted offsite, ee was provider 1st aid treatment for a minor head laceration. It's our understanding ee went to the local ER and received stitches after being removed from the mine property.

Injured employee was holding a wooden stake to the ground while another employee was attempting to drive the stake into ground using a 16lb sledge hammer. The sledge hammer glanced off the stake striking the injured employee on the left hand/wrist.
Employee stepped on a rock and twisted right ankle resulting in a minor sprain. Doctor placed employee on restricted duty.
The new gas train on Kiln 1 was being inspected for leaks. A torch was placed in the kiln to burn any gas that was released. At some point, the torch in the kiln was extinguished and gas accumulated in the kiln. The accumulated gas ignited causing a loud bang and some damage to the clinker cooler. No flash or flames were seen by any employees.
Employee was cutting a plastic vinegar bottle with a pocket knife and punctured his left thumb with the point of the knife. He was taken to the ER and two stitches were used to close the puncture. No lost time/No restrictions.
EE was drilling when drill became hung up on metal bracing & torque of drill twisted left arm. On 10/31/12 EE asked for medical treatment by MD.
Employee was using a 20 pound sledge hammer to close a slide gate on the #3 finish mill feed belt and strained his back. Employee was placed on restricted duty and returned to work.
Employee was using a pry bar when it slipped, causing him to fall backwards and strike his elbow on the concrete.

Smashed middle finger of left hand while removing a forklift tire weighing appr. 80#. Tire assembly slipped and smashed finger between hub and wheel. Employee reported injury and didn't want to go to doctor. Finished working that day @ 18:00 Hours. Called company from emergency room @ sweetwater hospital @ 19:15 PM for permission to treat him.
Employee was walking up a set of stairs and slipped, causing a contusion on his right knee. The doctor put him on 1 week of restricted duty.
EE was helping co-worker move form onto slab. Form became caught on bolt, causing EE's left thumb to become caught between the form and the slab.
Employee was hooking up a rigging on a load. A cable choker fell from the top of the load. The shackle on the end of the cable choker swung and struck employee in the side of the hard hat. The inner band of the hard hat caused a laceration on the employees head. Employee was treated at ER for laceration, received 8 staples and was allowed to return to work.

Employee was walking to a job location and bumped his right knee on a safety gate. Employee did not report the incident until 12/2/2011. Employee went to his doctor, who removed fluid from the knee and requested an MRI.
Injured employee was watching another employee using a 5 pound hammer. Injured employee moved closer to gain better view of the work and was struck in the forehead with the hammer. Employee refused medical attention and continued working until the end of his shift. After his shift, he went to the emergency room and received stitches in his forehead.
EE was lifting 60# bags of cement and felt pain in mid back area.
Employee was replacing an access door on a vertical screw when his glove got caught by the screw. The tip of his left index finger was severed.
Employee was changing a 3"" hose on a cement pump. His right thumb was pinched and fractured by the brass hose coupling.

Employee was cleaning/repairing a cement sampler screw. This is a small screw aprox 1/2 inch diameter. Employee disconected power to the motor, but did not wait for the motor to stop rotating before accessing it. Employee's glove got caught on the screw, causing a small fracture to his left thumb. No time lost.
Employee was using a pry bar to close the door on a coal rail car. The pry bar slipped and he fell on his left wrist. The initial diagnosis was a left wrist sprain and no treatment was necessary. Employee had a follow-up doctors appointment on 12/29/2010 during which the doctor changed the diagnosis to a left wrist fracture making the incident reportable as of 12/29/2010.
Employee entered a barricaded area where other employees were conducting maintenance and fell through an open hole in the floor, falling approximately 8 feet. Employee experienced some back pain, but did not receive treatment. Employee's doctor determined on 2/18/2011 that surgery may be necessary.
Employee strained his left shoulder while working on the #2 finish separator. Employee initially declined medical attention, but when the shoulder pain did not go away he went to see his doctor. This injury became reportable on 11/10/2010 when the doctor diagnosed the injury as a torn rotator cuff. The employee was placed on restriced duty. No time lost to date.
Employee was walking down stairs and missed the last step. He lost his balance and reached for the handrail. His left thumb hit the handrail causing a fracture to the thumb.

Employee was loading hopper cars. While pulling the hose with a rope, the employee felt a sharp pain in his left bicep. He then noticed bruising of the area. The injury was diagnosed as a bicep tear. Employee was placed on restricted duty and instructed to wear a sling.
Employee felt groin pain when lifting a manhole cover. Employee initially thought it was a minor groin muscle strain and declined medical treatment. When pain persisted, employee saw a doctor on 4/17/2009.
Employee took his hard hat off and put it back on. He said something stuck his head. He checked his hat and found neither an insect or foreign object. He went to Abilene ER and was told not to wear his hard hat until 2/27/09 and keep his forehead clean and dry.
Employee stated that he had a tingling sensation in the fingers of his right hand which radiated up to his wrist. Employee performs truck bulk loading operations, which requires writing, typing and manipulation of switches.

Injured employee was was assisting with changing the inboard bearing on the primary crusher. He was removing a nut from a stud when the nut slipped from his hand and he tried to catch it. The nut, which weighed about 5 pounds, landed on the little finger of his right hand causing a hairline fracture. The diagnosis of his injury is a fractured distal phalanx of the right 5th finger.
Associate slipped while pushing wheelbarrow full of waste cement, strained lower back muscle.
Employee was removing a dust collector fan shaft for maintenance when the fan wheel fell on his left hand, resulting in a small laceration and hairline fracture of his left ring finger.

EE's cleaning up @ the end of shift,Tools were placed in box & lowered to ground floor of mill bldg with overhead hoist. Hoist opr. was raising the hook so the hoist could be parked. EE's gathered at the box, hoist did not stop and cable broke, block & hook fell stricking EE on hard hat. Possible skull fracture.

An employee with 36 years experience currently assigned with the Repair Department as an oiler was carrying a 5 gallon bucket of cleaning solvent up a set of stairs by the tire feed conveyor. The employee heard a ""funny popping sound"" followed by extreme pain from his left knee radiating to his ankle. Orthopedic Specialist diagnosed the injury as a left knee torn Miniscus.
Employee was lifting railroad track steel up over top of head and pulled muscles in back.

EE was tightening a clean-out cap with a 48"" pipe wrench when the wrench slipped off of the cap and his finger got in between the wrench and the pump housing.
Two EE's were moving a motor through a tight area and the EE that was injured, got his right thumb caught between the motor and a pipe. This resulted in a fracture and laceration to the EE's right thumb.
Employee was loading a bulk truck when cement leaked from the rubber hose and got in his left eye. This caused some burning and the employee was taken off work by the Doctor.
Employee was trying to crawl into access door opening when hand slipped and he hit his chin on the door opening. Short ladders are being fabricated and installed to provide a safer means of access.

EE had series of dr.'s visits with no problem found even on MRI until explor. surg. on 3/9/06 when EE first missed work. Small tear found in meniscus may have been preexisting. Knee had prev. operation. Ins. just agreed to accept claim.

EE WAS WELDING ON OUTSIDE LINERS INSIDE THE #2 RAW SEPARATOR AND RECEIVED A FLASH BURN TO HIS EYES.
Two men moving used Mill liners on pallet. Scraped left leg causing injury. Was taken to emergency room in Abilene, TX and received six stitches.Returned to work same day.

EMPLOYEE ALLEGES HE WAS TURNING AND PICKING UP SECONDARY HAMMERS WHEN HE FELT A PAIN IN HIS UPPER ABDOMEN. HE WAS ORIGINALLY MISDIAGNOSED AS HAVING A MUSCLE PULL. IT IS BEING DIAGNOSED AS A HERNOA WITH EE HAVING SURGERY ON 6/3/05.
EMPLOYEE ALLEGES HE WAS CUTTING CLOSING LINE ON NORTH CRANE TO REPLACE CABLES WHEN HE SLIPPED AND FELL BACK HITTING THE BUCKET WITH HIS BACK.
EMPLOYEE ALLEGES THAT WHILE WALKING DOWN THE STEPS BEHIND THE OFFICE HE SLIPPED AND FELL. HE WAS TAKEN TO THE DOCTOR AND HAS A BROKEN BONE ABOVE HIS ANKLE.

EE ALLEGES HE WAS HOLDING ONE END OF ELEVATOR DRIVE CHAIN WHILE ANOTHER EE WAS DRIVING OUT CONNECTIVE PIN WITH A BALL PEEN HAMMER. FOLLOWING A HAMMER BLOW A SMALL PIECE OF THE CONNECTING PIN BROKE OFF AND STRUCK EE IN THE RIGHT FOREARM.

EE ALLEGES HE WAS JACKHAMMERING OVERHEAD OR A SUPLHUR RING IN THE KILN. A ALRGE DUST CLOUD CAME DOWN AND EE STARTED THE FAN SUCKED THE DUST INTO HIS EYES. DR REMOVED A SMALL PARTICLE FROMHIS E YE AND SAID HE HAD A SCRATCH. THIS INCIDENT IS REPORTABLE BECAUSE A PRESCRIPTION MEDICATION WAS PRESCRIBED.

EMPLOYEE WAS TURNING A BARREL OF OIL OVER WITH BARREL RACK AND FELT A SHARP PAIN IN THE CENTER OF STOMACH. EMPLOYEE DID NOT MISS ANY TIME OR ISN'T ON RESTRICTED DUTY BUT INJURY REPORTED BECAUSE THE DIAGNOSIS IS AN UMBILICAL HERNIA. EMPLOYEE AT SOME TIME MAY HAVE IT REPAIRED.
EMPLOYEE ALLEGES THAT WHILE HE WAS LOADING WIRE, CONDUIT, ETC IN A PICKUP HE BUMPED HIS ARM/ ELBOW WHEN THE CONDUIT BOUNCED.

TWO EMPLOYEES WERE PRYING OUT DRIVE BEARING OFF OF #3 KILN INCLINE SCREW. THE BAR SLIPPED AND ONE EMPLOYEE PULLED A MUSCLE IN BACK.
EMPLOYEE ALLEGES HE WAS JACKHAMMERING ON A CURNER PIPE THAT WAS SITTING ON SAW HORSES. A CASTABLE CAME OFF THE PIPE CAUSING THE BURNER PIPE TO ROLL OFF OF THE HORSES TOWARD HIM. IT FELL STRIKI NG HIS RIGHT SHIN. HE HAD THE HAMMER IN HIS LEFT HAND AND WHEN HE PULLED BACK TO GET OUT OF THE WAY HE FELT A CHARLIE-HORSE IN THE LEFT SIDE OF HIS LOWER BACK.
