While tramming dozer along haul road, employee drove over shot rock on the haul road causing the front end of the dozer to slam against the ground which cause the air ride seat suspension to bottom out.

Moving the head on the drill up & down for mechanic to work on and the steel hammer came back and pinned EE's left elbow between it and the drill.

Drill bit blocked up, Employee raised tower and separated steel to remove all pressure to dig out air holes. Drill steel still had pressure and blew air and debris into Employee's eyes and face. He did not wear his safety glasses.

This employee was jack hammering ice & frozen material from a crusher when he was struck in the left eye with a piece of debris.

THIS EMP. & OTHERS WERE INSTALLING BOOM ON 285 DEMAG. GRINDING/WELDING/BURNING WAS TAKING PLACE - METAL SLIVER GOT INTO RT EYE.

While operating dozer, building a roadway, the operator went over large rocks & bounced violently in the cab of dozer causing a neck sprain. EE worked thru 3/12/04 & his first day of lost time was 3/15/04.

GRINDING BOLT FROM D9N DOZER - FB INTO LT EYE. WORKED 9/22 & 9/23 - OFF 9/24/03. SAFETY GLASSES WERE WORN DURING GRINDING.

EE WAS CUTTING/BURNING APART OLD TANK WHEN HE WAS BURNED BY SPARKS.

THIS WELDER WAS HOLDING A PIECE OF STEEL WHILE ANOTHER WELDER WAS WELDING TO REPAIR SHOVEL BULLET AND WAS RETAILS ON LG. BOULDER. BOULDER MOVED BUCKET SLID AND CAUGHT 2 FINGERS BETWEEN BUCKET AND BOULDER.

USING CHIPPING HAMMER TO CHIP SLAG OFF & A PIECE OF SLAG (FOREIGN BODY) GOT IN LT. EYE.

EE WAS BURNING A BOLT HEAD OFF A SCREEN. HOT SLAG FELL INTO RT. EAR, BURNING EARDRUM.

EMPLOYEE WAS HEATING PIN IN DOZER TO REMOVE IT WHEN OIL RING EXPLODED AND SENT DIRT INTO BOTH EYES.

HURT HIS BACK WHILE RUNNING DOZER.

WHILE SQUEEZING ELECTRIC HOLDER, FINGER BENT DOWN, STAYING THERE, NOT ABLE TO MOVE.
