Employee tripped and fell over a pallet while carrying containers of D.E.F.

employee was lifting a crate into a loader bucket

Employee was climbing down ladder to exit dragline when EE's foot slipped and EE struck EE's left elbow on a ladder wrung. Employee attended PT for elbow pain. MRI was performed in May 2025 showing a torn ligament. Employee did not miss any time from work until June 17, 2025.

Employee slipped and fell on ice.

The front tires of the truck lifted off the ground while ascending a ramp. Employee struck right leg off something in the cab.

Employee was cleaning tracks on drill rig with track shovel. Hit something hard with shovel and heard/felt left shoulder pop.

Carpel tunnel of right hand due to repetitive motion throughout EE's years working with the company.

WHILE WALKING FROM EMPLOYEE PARKING LOT TO EQUIPMENT IN THE DARK, THEY SLIPPED AND FELL TWISTING RIGHT KNEE.

EE was walking across uneven ground on mine site and slipped on ice and fell to ground landing on back.

Fueling 785 B truck - standing on ladder. Ladder slipped and employee twisted right knee.

Drill bit blocked up, Employee raised tower and separated steel to remove all pressure to dig out air holes. Drill steel still had pressure and blew air and debris into Employee's eyes and face. He did not wear his safety glasses.

Repairing cutting edge on excavator. Sledge hammer cracked & piece of hammer impaled left thigh.
