EE was operating a dozer (back blading) in a reclaim. area when he trammed over a section of shale rock causing him to be bounced in the cab jarring his neck and back. Lost work days due to the injury began on 5/1/12.

Cutting out old bed liner in 777D truck bed. Liner sprung upward and hit employee in left arm and face.
Employee left a message on voice mail 10/3/07 that he may not work the next day. A message was left on 10/4/07 that he had been scheduled for an MRI on his back. Emoloyee operates a D-10 Cat Dozer.
The 785 Cat. rock truck was being loaded by the 5230 Cato shovel. The bucket of the shovel struck the bed of the rock truck. The jolt from the bucket striking the bed injured the truck operator's back.
EE was using air tool the pressureized hose "jumped" and caught hand between air hose & machine.
The dozer trammed too close to the edge of the spoil bank. The spoil bank failed and the dozer dropped over a steep embankment about 30 ft high.
PICKED UP EE AT END OF SHIFT. EE COMPLAINED SAYING BACK HURT DUE TO THE ROCKING OF HOE(MACHINE)
WHILE PUSHING ROCK OUT OF NO. 12 COAL SEAM PIT, EE TRAMMED A CAT. D-11R DOZER OVER THE HIGHWALL. (DOZER FELL 120 FEET INTO SOFT MATERIAL; EE WAS NOT THROWN FROM CAB. SEATBELT CAUSED BRUISING TO HIP.)
EMPLOYEE WAS OPERATING A DOZER TO RECLAIM AN EXISTING HIGHWALL. WHILE PUSHING DIRT OVER RELAIM AREA, THE DOZER TURNED SIDEWAYS AND ROLLED OVER HILL. EMPLOYEE WAS WEARING SEATBELT.
OPERATING DOZER ON STEEP SLOPE, PULLING MUSCLE IN LEFT SIDE OF CHEST. OPR DID NOT REPORT ACCIDENT UNTIL 12:30 PM - 10/30/02.
EE STATED THAT HE HAD BEEN HAVING PAIN IN HIS RT. SHOULDER AND ELBOW. DR. ADVISED HIM THAT HITS WAS CAUSED OPERATING CONTROLS OF DOZER.
EE RAN OVER A LARGE ROCK. HE SAID WHEN THE DOZER CAME DOWN IT JARRED HIS BACK. I HAVE BEEN TRYING TO CONTACT EE AND HE'S BEEN UNAVAILABLE.
SOCKET LOOSE ON IMPACT HAMMER, AND SMACKED BUMPER TO SEAT AND CAUGHT RH FINGER ON BUMPER WHEN SEAT SOCKET. RIGHT HAND 2ND FINGER.
