While assisting with a repair and working in a kneeling position went to stand and his right knee locked up. He was in pain but completed his shift. He continued working and completed an initial report the following work day. He sought medical attention February 1st. Diagnosed as a meniscus tear and placed on a work restriction beginning Feb 3rd.
Injured stated he was standing outside the shop area talking to employees when he noticed something felt like it went in his eye. He immediately washed out his eye. However, two (2) days after the event, he sought medical attention where an abrasion was observed and was given a prescription for eye ointment. Note: This incident was not reported to managment until 11/5/2010.
As the injured was going to install a roof bolt into the top. As he twisted to install the glue and bolt he heard and felt a pop in his back.
Down on knees winching. As EE was getting up his knees gave out.
Employee was walking over to machine buggy to get parts, and said he twisted his right knee causing pain and swelling.
Employee was leaned over talking to his foreman (who was in a vehicle). When employee straightened up, he stated he felt a sharp pain in his lower back. The employee has not missed any work. After a follow-up visit with his physician, it was recommended that the employee start physicial therapy on Monday 11-10-08.
Employee was working underneath a water truck and was stung by a bee. Shortly thereafter he started to exhibit anaphylaxix systems. He was taken to the hopital, treated with injection and intravenous medications. Following treatment he was released and sent home. He returned to work on his next scheduled shift.
Employee was holding hood door open while checking oil. A gust of wind blew hood out of his hand causing hood to fall hitting him in back of head & neck. He was wearing his hard hat.
Employee bent over to pick up something and could not straighten up. Diagnosis is back spasms.
Employee was bending over to tighten a clamp on a pipe bonder when he felt pain in his back. Employee did not miss any work until 09-17-07. Accident has been under investigation and has been determined to be reportable.
Employee was repairing a return stopping when a piece of an Omega block went into his eye beside his safety glasses. Employee was in a high volume of air.
Non EE received injuries that proved to be fatal after he wrecked an ATV on a section of road that is not a part of the mine. ICG Knott County does not believe submission of this report is required by the Mine Act or regulations and is submitting it under protest.
In accordance with the information we have received thus far, EE evidently died of a massive heart attack instantly because no one present could detect a pulse from the time it happened through EE's departure in ambulance.
Employee stated that he was stung by a wasp.
As truck was staged in front of bin waiting to dump, the loader attempting to clear off bin pulled in front of truck to clean bin and backed into the waiting truck. No injury reported - Employee refused medical treatment.
Injured stated he was in process of cutting weeds using a weed eater. While doing so, he encountered some bees and was stung 6 or 7 times. Shortly thereafter, he had difficulty breathing and was taken to he hospital for an apparent allergic reaction.
EE stated that he started to turn around, and he felt pain in his left knee. This ee stated that he has had cartilage removed from both knees when he was in grade school. He is schedule to have an mri on 12-16-05.
Employee came to work with Dr's excuse & a prescription for Cecebrex & Vikaden. Foreman told employee he could not operate equipment while on Vikaden. Employee stated that he went to Dr. for pain in his shoulder, wrist and right foot. He also stated tha his right foot was still giving him problems. The Dr. wrote to not use right foot on pedal that Miner Operator has to use.
