Employee was shoveling dirt near kiln area during strong winds. Wind blew material back into employee's face and into right eye. Material could not be effectively removed on site and employee visited doctor. Doctor was able to remove material. Resulted in scratched cornea in right eye.

Showing all 14EE took an OTC pain reliever, not provided by the company and had an allergic reaction to it, which included difficulty breathing, dizziness, weakness and an itchy body. Dicalite has an AEMT II that took care of EE until the ambulance arrived. EE was given oxygen and an IV and was transported to the hospital. The Vacaville office advised Dicalite to fill out this report.
Employee stated that there was a power bump in the system and they were running to see what happened and twisted their back.

EE HAD A RED SPOT IN HIS RIGHT EYE SO HE WENT TO THE INTERMOUNTAIN FAMILY PRACTICE & THEY SENTHIM TO A LOCAL EYE DR. DR SAID HE HAD A LACERATION TO HIS EYE. EE DOES NOT NOW HOW IT HAPPENED OR WHEN IT HAPPENED
EMPLOYEE WAS PUTTING TOGETHER POOL TIME BOXES & A FOREIGN PARTICLE FLEW INTO HIS LEFT EYE.
EE WAS TRANSPORTING PALLETS INTO PACK SHED WITH FORKLIFT, WHEN WIND BLEW MATERIAL OFF PALLETS INTO EYES.
