Struck against a moving object

Employee was involved in a motor vehicle accident in a company vehicle. The accident occurred in Redding, CA. Employee was injured in the accident, transported to the local ER, treated for injuries and diagnosed with a possible fracture of the C-3 vertebrae. The accident is still under investigation by the Redding PD.

Fall down stairs

Employee was walking up the stairs to change the time on the printer. EE tripped and fell landing on left wrist. EE was taken to the emergency room and treated for the sprained wrist.

Walking into the plant to start work, slipped and fell on the ice and snow in the parking lot. Injured right elbow.

Ingestion of radiations, caustics, toxic and noxious substances

EE took an OTC pain reliever, not provided by the company and had an allergic reaction to it, which included difficulty breathing, dizziness, weakness and an itchy body. Dicalite has an AEMT II that took care of EE until the ambulance arrived. EE was given oxygen and an IV and was transported to the hospital. The Vacaville office advised Dicalite to fill out this report.

EE was falling a tree and was hammering a wedge into the tree and when ee hit the wedge it flew out of the tree and hit ee in the nose.
