Twisted left knee on slick uneven ground clearing and dragging brush.

Climbing into the cab of the road grader EE's knee popped. Strained or pulled left knee.

EE tripped over the pallet that EE was placing bags on, causing EE to fall and land on their right hand, dislocating the 5th middle phalanx.

Customer truck driver was disconnected from their lanyard while working on the back of their trailer. The driver lost footing and fell 5 feet to the ground below.

EE slipped and fell on the ice cutting the back of EE's head.

EE tripped over the concrete cover by the Soda Ash Silo, while hosing down the pad, injuring left shoulder.

EE got something in eye at the start of shift putting stickers on bags.

Threw out left shoulder using sledge hammer. Pulled muscles in both upper arm/shoulder.

Employee was involved in a motor vehicle accident in a company vehicle. The accident occurred in Redding, CA. Employee was injured in the accident, transported to the local ER, treated for injuries and diagnosed with a possible fracture of the C-3 vertebrae. The accident is still under investigation by the Redding PD.

Employee was walking up the stairs to change the time on the printer. EE tripped and fell landing on left wrist. EE was taken to the emergency room and treated for the sprained wrist.

Bringing paperwork to office slipped and fell on ice injuring right shoulder.

Walking into the plant to start work, slipped and fell on the ice and snow in the parking lot. Injured right elbow.

Employee was welding and grinding and got a foreign body in right eye.

Employee was burning brush and slipped and fell on the ice landing on EE's left shoulder.

Employee was clearing brush when a branch sprang back and hit EE in the face, knocking EE's hard hat and safety glasses off. Injury occurred to left eye.

employee tripped over bulk sack cord while filling bulk sacks. Fell on right knee & wrist on concrete floor

Employee was hooking up bulk sack at filling station and bumped knuckle on frame causing swollen knuckles.

Employee had arm extended all the way out and pulled sideways on a valve that was stuck and hurt right shoulder.

Pushing pallet of dica wrap onto a pallet and hurt left shoulder. (Popping noise in shoulder joint)

EE took an OTC pain reliever, not provided by the company and had an allergic reaction to it, which included difficulty breathing, dizziness, weakness and an itchy body. Dicalite has an AEMT II that took care of EE until the ambulance arrived. EE was given oxygen and an IV and was transported to the hospital. The Vacaville office advised Dicalite to fill out this report.

Possibly exposed to arc welding from fellow worker; working on the opposite side of blower housing. Employee was wearing welding/cutting goggles.

EE was falling a tree and was hammering a wedge into the tree and when ee hit the wedge it flew out of the tree and hit ee in the nose.

EE was whipping a pallet bag like you would a trash bag to open it, when EE whipped it EE was standing on part of the bag and as EE followed thru with the motion EE felt like it separated EE's left shoulder.

Employee was feeding straw into straw blower for reclamation work and hurt left shoulder.

Climbing onto back of pickup to go to lunch, slipped and landed wrong.

Employee was welding at the palletizer and got a piece of metal in eye.

Employee was putting 25lb bags of product into boxes and strained left abdomen.

Employee was wrapping pallets and got a foreign object in his right eye.

Employee was fabricating a roll for the soda ash belt conveyor. While machining surface of roll on lathe a flying metal chip embedded in his left eye.

Employee was working on a forklift and got a small piece of metal in his left eye.

Employee was welding inside of pipe, and when moving forward to continue welding, he came in contact with a previous weld and burned his left elbow.

Employee had heat prostration.

Employee was pushing ice out of bowl of scraper with his foot. The piece of ice broke causing him to fall and land on his chin.

Employee was cutting strap on new bulk sack pallet with his pocket knife and accidentally cut inside of his forearm.

Employee was trimming trees and burning brush piles. He stepped on the bank and slipped in the snow falling backwards, catching himself with his arm.

Employee was laying on his stomach removing valves on the 988 Loader when he over extended stomach/abdominal muscles.

Slipped on snow covered ice.

Employee gathered 12 1lb sample bags of DE and put them in a garbage bag. She put the bag over her shoulder straining her upper left scapular area.

Employee was making conveyor paddle. When making adjustment, hand slipped into saw blade.

Injured was putting a 24 lb bag of diatomaceous earth on a pallet and his elbow popped.

EE was holding limb with hand and hit thumb with swede.

EE was getting out of water truck for lunch break and slipped on bottom step and twisted right knee, skinned up left knee and shin.

EE was changing springs on the #44 trailer. He was cutting spring bolts and a piece of metal got in left eye.

EE was installing new roll pipe on pacing conveyor, he was pushing roll thru hole on conveyor. He exteneded his arms out to push pipe thru puther and hurt his lower back.

Employee lifted a conveyor motor and strained his shoulder and chest. Later on that evening he had a heart attack.

Hit top of head on the catwalk while climbing ladder.

Repetitive packing of 10 lb and 25 lb bags, pounding 25 lb into boxes, use of air hammer.

He was climbing into the back of a service truck. His foot slipped on a step and he fell to the floor on his right shoulder.

EE WAS CARRYING A 4" PIPE DOWN STAIRS WHEN SOMEONE GRABBED THE OTHER END TO HELP HIM, THROWING HIM OFF BALANCE ON THE STAIRS.

Went to get jumper cables for apick up when walking back with cables slipped on ice and fell down.

BAG OF DIATOMACEOUS EARTH TORE OPEN WHILE FILLING.

LOADING TRASH IN BACK OF DUMP TRUCK SLIPPED AND FELL.

WHILE MOVING BELTS ON SCREEN MOTOR, FINGER WAS CAUGHT BETWEEN BELT AND PULLEY.

A PIECE OF GRINDING FLEW INTO RIGHT EYE WHILE HE WAS WEARING EYE PROTECTION.

CLAIMING REPETITIVE MOTION CAUSED HIS ARM TO SWELL AND HURT. HE WAS MAKING 25# HANDIPAKS.

EE WAS DRAGGING PIPE AND GOT HIS THUMB CAUGHT IN FLANGE OF 8" PIPE.

WHILE WALKING UP THE WAREHOUSE STEPS. EE FELL AND HIT HIS HAND ON THE HANDRAIL. HE BROKE HIS RIGHT HAND.

WHILE JACKING UP THE BULK TRUCK (EE FORGOT TO BLOCK THE TIRES) THE TRUCK ROLLED KICKING OUT THE JACK. THE JACK HIT EE'S RIGHT ARM PUSHING INTO THE TRUCK CUTTING HIS RIGHT ELBOW.

SUB CONTRACTOR TRUCKER FELL OFF HIS TRUCK WHILE TARPING HIS LOAD. HE INHURED HIS RIBS AND ELBOW.

CUTTING METAL IN VISE WITH A HAND HACKSAW. CUT THROUGH METAL STRIKING LEFT HAND ON THE VISE.

SUB-CONTRACTOR TRUCKER FELL FROM OWN LADDER WHILE STRAIGHTENING HIS V-BOARDS

EE WAS WRAPPING A PALLET WHEN A FOREIGN OBJECT FLEW INTO THE CORNER OF HIS LEFT EYE. HE WAS WEARING HIS SAFETY GLASSES AT THE TIME.

AFTER FILLING WATER TRUCK, EE WENT TO GET INTO THE TRUCK AND SLIPPED IN THE MUD. HE WENT TO DOCTOR ON 6/4/01. DOCTOR SAID HE WAS FINE. ON 6/21 AND 8/8 HE WENT BACK TO THE DOCTOR AND WAS DIAGNO SED AS HAVING A HERNIA. THE COMPANY WAS NOTIFIED ON 12/4/ BY THE DOCTORS OFFICE THAT THE EVENTS HAD OCCURRED. AS OF 12/10 EMPLOYEE HAS NOT NOTIFIED EMPLOYER.

EE HAD A RED SPOT IN HIS RIGHT EYE SO HE WENT TO THE INTERMOUNTAIN FAMILY PRACTICE & THEY SENTHIM TO A LOCAL EYE DR. DR SAID HE HAD A LACERATION TO HIS EYE. EE DOES NOT NOW HOW IT HAPPENED OR WHEN IT HAPPENED

EE WAS CLOSING ISOLATION LOUVERS WHEN FOREIGN MATERIAL FROM ABOVE WAS DISTURBED BY CO-WORKER AND MATERIAL GOT INTO HIS LEFT EYE.

EE SLIPPED IN WET DIRT AT BOTTOM OF CONTROL STAIRS.

EE SLIPPED ON THE STEPS AND HURT HIS LOWER BACK.

EMPLOYEE WAS PUTTING TOGETHER POOL TIME BOXES & A FOREIGN PARTICLE FLEW INTO HIS LEFT EYE.

CLAIMS EXPOSURE TO NUISANCE DUST CAUSED SINUS PROBLEMS. OPENED DOORS OF ENVIRONMENTAL CAB TO CHEW AND SMOKE TOBACCO. MINIMAL EXPOSURE TO DUST. EE DID NOT REPORT THIS FOR 9 MONTHS.

EE WAS TRANSPORTING PALLETS INTO PACK SHED WITH FORKLIFT, WHEN WIND BLEW MATERIAL OFF PALLETS INTO EYES.

EE WAS GETTING SAMPLE OUT OF #2 PACKER AND MATERIAL GOT INTO BOTH OF HIS EYES.

EE WAS GREASING THE D9, WHEN HE SLIPPED ON SOME MUD ON DOZER ARM, AND FELL BACKWARDS, PULLING HIS RIGHT KNEE.

EE JAMMED HIS RING FINGER OF HIS LEFT HAND INTO A DICALITE BAG WHILE PALLETIZING.

CHANGING FILTER ON WATER PULLEY. HIS AIR HOSE HUNG UP AND HE SLIPPED AND FELL.

THE EE WAS INSTALLING CONVEYING BLOWER TRANSITION, AND WAS GOING AROUND TO OPPOSITE SIDE AND SLIPPED ON ICE, CAUSING INJURY TO RIGHT KNEE.
