*Incident is under investigation as to whether it is work related.* Employee claims to have been bitten by a spider on right hip (under waist line of pants) that EE saw on the door while exiting the dozer cab to check the machine. Dr. diagnosis was "cellulitis". Employee did not report to supervisor until late Wednesday afternoon. Dozer is fairly new and has been maintained.

Employee was by personal vehicle leaving work on 11/9/22 and reports something was blown into their right eye.

Employee was entering a payloader and as they reached to close the door, a gust of wind slammed the door shut smashing the employee's pinky finger in the door jamb resulting in a laceration to the tip of the finger.

EE was operating a Front End Loader positioning some HDPE Pipe, when setting the pipe down EE apparently disturbed a bee hive. EE was seen by a co-worker swatting the area around EE and EE exited the Loader quickly after putting it in park. EE indicated EE had been stung multiple times by the bees.

EE called said they thought they had passed out and needed medical attention. ERT Responded and took vitals which were normal. EE was taken to nurse for evaluation. Out of an abundance of caution EE was transferred to ER by Ambulance.

(Ground Conditions) Employee was walking through ankle deep water applying herbicide via backpack sprayer to invasive aquatic plants. During this process employee was bitten by a snake (water moccasin) on the left foot, inside area. Employee was wearing waterproof boot waders and only one fang punctured through wader boot. This bite was determined to be a dry bite.

5 man crew was working in a wetland harvesting plants to be relocated to a different reclamation program. harvesting began at approximately 8:30 am, around 10:45 am a snake approximately 8-10 inches in length bit employee on middle finger between the second knuckle and hand. Snake was submersed in water around the base of the plant.

Emp states that while outside a vehicle, a bee flew up inside EE's hardhat. While removing hardhat emp contacted bee and it stung EE on the middle finger of EE's right hand. Emp had an allergic reaction and was taken to Urgent Care. Emp was given Antihistamine, Pepcid and Solumedrol injection. Emp released back to work without restrictions.

6 man crew was planting vegetation in 18" of water. Worker was bit on left side by an alligator. Pre-job inspection showed no alligator presence. Spotter in place did not see gators until incident occurred. Employee taken to emergency room treated for injury on left hand, significant damage to thumb. Also cuts on torso under left arm and abrasions on back and stomach.

While performing inspections/PM EE felt something in left eye. EE did not report to supervisor and went to a walk in clinic the next day. A foreign object was found and removed from left eye. EE reported when returning for normal shift 12/11, Restrictions - No driving CMV - other accommodations have been made.

Employee walked onto ramp to enter office trailer when EE was bitten by a snake (water moccasin).

EE was getting out of work truck when a Black Widow spider bit EE on the left hand finger

A trespasser died on mine property. The driver lost control driving truck, struck the toe road berm, partially ejecting the driver and pinning driver under the truck.

Employee was found unconscious, waiting for medical results, possible dehydration, heart condition, or other. Employee regained consciousness when found by other employee. Doesn't remember what happened. Employee is currently returned to work and doing well.

While at home on 07-13-15 at approx. 8:00pm, a Mosaic Plant Maintenance Employee began to feel discomfort in his eyes accompanied by sensitivity to light. Employee notified his Supervisor at home to report this condition despite not having experienced any discomfort during the work day.

Emp. came to work and put on work boots. During tool box mtg emp. felt a burning/itching to his right foot, 4th toe. He felt swelling later during his work shift and removed his boot. Emp. took IBU for pain and swelling and applied ice. On 8/29/14 emp. taken to company Dr. for increase swelling and drainage. Emp. given an antibiotic and placed on restrictions.

While operating engine #3802 the engineer was bit by a spider on his lower right side. This was on Sunday night and he reported it on Monday night.

Emp. was on Battery Car. He became unresponsive at approx. 1:50 pm. his hard hat fell off and another employee ran to help him (Facundo Baca) Emp. was taken out of the mine on stretcher to be taken to ER, and was pronounced dead at 4:30 pm by OMI on the Hobbs Hwy in route to town.

Employee was working in a wetland extracting herbaceous plants and was bitten on the right hand by a snake.

Employee started having chest pains co-worker took him to the site nurse. The site nurse took hiss vitals and called the ambulance. The paramedics check him out and he refused to go in the ambulance. He called someone to pick him up and take him to the family doctor.

There was not an accident on the mine site. The incident was attributed to an individual's pre-existing personal health condition. An employee operating a crane was observed as non responsive. Our Emergency Response Plan was initiated. 911 was called. EMS arrived on the scene. The employee expired at a local hospital later the same day.

Emp. was working on level XMTR, when a bee got into outer shirt, stinging him on the left side of hip, at belt line. Then into right sleeve stinging him on the right arm. Emp. taken to company Dr. and given 2 antibiotics for infection.

Employee was in the process of replacing the east primary cyclone feed pump when the employee was stung by a bee in the upper right arm area. Employee went to supervisor to report bee sting. Emp. taken to company Dr. on 8/16/12 and given two antibiotics for infection.

Emp was working in area 8 when a ground fall in the old working annihilated the area. Crew had to travel through the salt dust to evacuate the mine. Emp. went to company Dr. on 3/28/12 given two prescription medications, no restrictions.

Emp. operating front end loader when he felt a sting on his left shoulder. Emp. taken to ER because he was feeling sick. Emp. given an antibiotic and Tetanus injection. No restrictions.

HEART ATTACK Employee reported to work and was going up stairs to clock in, 2 employees witnessed him collapse on landing. CPR was immediately administered without success. HEART ATTACK

Emp. was riding down the shaft and felt a pinch on his right leg. He later had swelling, irritation, itching and burning. Emp. went to ER related to bug bite. Emp. instructed to take 2 days off work and given antibiotics and pain meds.

Emp. cleaning trash out of compactor. Emp. started noticing pinching and itching to right upper inner leg. When he looked there were 2 red hard areas. Appeared to be bug bites. Emp. taken to Industrial Health Services. Antibiotic given and placed on restrictions.

Emp. felt discomfort to right ankle, got home and found ankle swollen and red area noted. Emp. went to ER and was given an antibiotic and pain prescription for possible spider bite.

According to hearing tests performed it has been determined that employee has sustained a hearing loss threshold shift in both ears.

After completing the task of relocating a water jack in the 20 Dragline mining area injured employee was in the process of rolling up the air hose and impact wrench when sand/dirt blew into his right eye. The employee was wearing company issued "Solara Safety Glasses - Smoke"

Field man was assisting in running matrix line with leader man. While working, he bent down(squatted) to see pipe alignment and felt sharp pain in right knee. Employee did NOT report the injury when it occurred and continued until shift was completed.

Emp finished assisting other employees in loading miner cable onto generator. He used his right forearm to wipe sweat off of his forehead when he felt a pain in the right side of his neck. Due to limited range of motion and pain, he went to company doctor. Placed on restrictions and given prescription.

Emp was sitting in a safety meeting and turned to his right to pass a paper to the person beside him and felt a pain in his lower back. When he went to stand up after the meeting was over the pain increased. Went to company doctor. Rcvd restrictions, PT, and prescription.

This is not work related: Employee had what appeared to be a heart attack underground. CPR administered as well as the use of AED. He was transferred to emergency room. Carlsbad EMS ambulance intercepted with Mine emergency vehicle on Hobbs Highway. Employee was pronouced dead in the emergency room at 8:53 am. MSHA notified on 7/31/08.

During the course of duty employee was stung on the neck by a wasp.

No actual accident occurred. Possible "wear and tear" of job could be cause of tear of tendon in employee's shoulder. Emp. reported that shoulder has been hurting for 1-2 yrs. no injury, went to company dr. on 4/8/08, placed on restricted duty.

Employee was bitten on the left forearm by an unknown insect while servicing Gulf #1 machine.

Upon getting into the cab of the backhoe; operator was stung by a wasp.

Employee put on his work gloves and was changing out ladder wire on the Gulf II dredge. Employee took his glove off & laid it to the side. When the employee put the glove back on he felt a sting on his left hand. Employee removed glove, inspected it and found nothing. Later that same day he noticed his hand swelling. Employee saw company nurse.

While grinding on elevated steel beam in high winds, a foreign body blew into employee's eye & became embedded. Employee returned to regular duty after treatment.
