A third-party trucking contract driver was found unresponsive near the mine exit. Trained CPR providers from the mine responded and performed CPR until EMS arrived on scene. The driver was pronounced deceased by EMS staff.

A non-miner customer truck driver was knocked unconscious when a gust of wind blew their trailer doors open and slammed them, headfirst, onto the ground. The driver received medical attention for a concussion.

Injured had just dismounted their haul truck with both feet on the ground. When the employee turned to walk away the left knee buckled. Diagnosed with a left knee strain.

A customer truck driver was found unresponsive in the cab of the driver's truck on the US Silica customer pickup dock. The onsite emergency responder and 911 were notified. CPR was conducted until the ambulance arrived. The driver was then transported to a local hospital and was pronounced deceased due to a health related incident.

A deceased body was discovered in the South Ottawa Pit. The person is not a mine employee. The Sheriff and MSHA were notified immediately upon discovery and the investigation is ongoing. The birthdate of the decedent is unknown.

EE was operating a front end loader in the clay shed. EE opened the door and thought it latched open. EE reached out to get some food and the wind blew the door shut striking EE's left hand between thumb and pointer figure. EE required 8 stitches.

Employee stood up after opening a rail car gate and felt a "pop" in knee. Employee initially refused medical treatment. Employee requested medical treatment on May 9th at which time surgery was recommended. On June 4th 2018 employee went off work for surgery.

A Service Worker suffered a fatal heart attack at a residential property owned by the company and adjacent to the active mine site. The Service Worker was removing an inactive LP gas tank when he fell ill. EMS was called by his coworker, and the victim was rushed to a local hospital. The fatality was due to natural causes and was not related to any mining activities.

Employee was struck in the eye by debris while working either on a door in the shop or while working on packing for a pump, both jobs were on separate days, the employee is not sure when it happened exactly. Employee reported incident to his supervisor Sunday February 28th and stated that he wasn't sure if it happened Friday February 26th or Saturday February 27th.

As employee broke for lunch he started to feel pain slowly come on in his neck and left arm. The employee has a serious personal illness and has not been to work since 7/18/2007. The company received paperwork from the employee's doctor that said he is off of work for this injury not his personal illness on 8/2/2007.

Employee popped his right knee out of place doing maint. work on a loader. He indicated that the damage was from a pre-existing condition caused by a skiing accident and that the "popping" of his knee happened often. Injury was ruled a first aid at the time-no lost work days. Employee went for surgery on April 26, 2007 causing the first aid to become a LWC on April 27, 2007.

THE OTHEREMPLOYEE TRIED TO SHAKE HIM AND GOT NO RESPONSE FROM HIM. THE EMPLOYEE HAD NO PULSE AND WAS NOT BREATHING. THE EMPLOYEE HAD SOMEONE CALL 911 AND BEGIN CPR.
