Employee was cutting off excess threads on lacings to the underscreen belt. Employee was on their knees reaching up to the lacings with the grinder. When they came out from under the screen and stood up their back seized up from being in a awkward position resulting in back strain.

Employee was walking down to the pump house when they felt a sharp pain on left hand. That's when they noticed a Yellow jacket was biting their left hand. Went to ER and was given an injection.

Employee passed out, Ambulance came and could not find anything wrong. We drove the employee to the hospital, checked out and determined it was because lack of nutrition. This was reported to MSHA hotline but turned out to be a personal health issue and non work related.

Employee was complaining of chest pain, an ambulance was called. EKG normal & blood pressure high so employee was taken to the hospital.

EE woke with severe chest pain at approx. 3:30 a.m. and believed it to be really bad heart burn, he got ready for work and then drove to the office. Upon arrival, he started experiencing more of the pain with some tingling in leg and arm. When another EE arrived he asked her to call 911. EE was coherent and communicating with EMT's at departure from the mine.

Employee reported an injury of low back pain, a medical physican determined this to be non-work related. On 7/2/13 Utah Labor Commission determined a portion of injury was work related, a motion for dismissal was filed with the court of appeals. On 12/12/13 a binding agreement between the 2 parties was signed, that removes liability from the organization.

THE WIND WAS BLOWING EXTREMELY HARD THAT DAY, DUST GOT INTO THE SCALE HOUSE WHEN THE DOOR WOULD BE OPENED BY DRIVERS GETTING THEIR TICKETS. EE FELT DUST GET INTO HER EYES, BUT ONLY RUBBED THEM IN TRYING TO REMOVE THE DUST. BY MIDNIGHT HER EYES WERE IRRITATING HER SEVERELY.
